Porucha glukózové tolerance, manifestace diabetes mellitus a dekompenzace diabetu 1. i 2. typu je všeobecně známým nežádoucím účinkem terapie kortikosteroidy. Kortikosteroidy patří k často předepisovaným lékům a jejich užívání se v některých situacích nevyhnou ani pacienti s poruchou glukózové tolerance nebo s již rozvinutým diabetem 2. typu ani pacienti s diabetem 1. typu. Patogenetické mechanismy ovlivnění glukózového metabolismu při léčbě kortikosteroidy nejsou na buněčné úrovni doposud zcela objasněné a existuje řada teorií vysvětlujících tento nežádoucí metabolický účinek. Je zřejmé, že jde o působení na několika úrovních a existují i interindividuální rozdíly v projevech tohoto nežádoucího účinku. Článek na základě dostupných informací rozebírá možné mechanismy vzniku hyperglykemie působením kortikosteroidů a vliv kortikoidní léčby na další tělesné systémy. Zhodnoceny jsou i terapeutické postupy vůči takto vzniklému diabetes mellitus.

Glucose tolerance impairment, a manifestation of diabetes mellitus and a decompensation of both type 1 and 2 diabetes mellitus are commonly known unwanted effects of the corticosteroid treatment. The corticosteroids belong to frequently prescribed drugs and their use is inevitable even in patients with the glucose tolerance impairment or already manifested type 2 diabetes mellitus as well as in patients with type 1 diabetes mellitus in some circumstances. The pathogenetic mechanism influencing the glucose metabolism during the corticosteroid treatment at the cellular level has not been completely explained yet and there exist different theories explaining this unwanted metabolic effect. It is evident, that the influence is multilevel and that there exist interindividual differences in the manifestations of this unwanted effect. Based on available information, the paper analyzes possible mechanism of hyperglycemia induced by the corticosteroid treatment as well as the effect of the corticosteroid treatment on other body systems. Therapeutical procedures in diabetes mellitus induced in this way are reviewed.
