Hypoglykémie u pacienta s diabetes mellitus 1. typu a Addisonovou chorobou - ako na ne?

Prezentujeme prípad 59-ročného pacienta s diabetes mellitus 1. typu a adrenokortikálnou insuficienciou v rámci autoimunitného polyglandulárneho syndrómu 2. typu s klinicky závažnými hypoglykémiami. Vychádzajúc zo znalostí o kontraregulačných mechanizmoch pri hypoglykémii, o špecifickej etiopatogenéze vyššie uvedených ochorení a s ohľadom na vek pacienta sme do liečby priradili rastový hormón, výsledkom čoho bolo výrazné zníženie frekvencie, ako aj intenzity hypoglykémií. Prípad tak demonštruje potrebu komplexného pohľadu na pacienta s diabetom, ktorý je možný len vtedy, pokiaľ bude diabetológia integrálnou súčasťou endokrinológie.

TA 59-year-old male with diabetes mellitus type 1 and adrenocortical insufficiency as a part of autoimmune polyglandular syndrome type 2 presented with severe hypoglycemic episodes. Based on the knowledge of contraregular mechanisms in hypoglycemia, specific etiopathogenesis of forementioned disorders and patient age, we initiated treatment with growth hormone. As a result, the frequency as well as the intensity of hypoglycemic episodes decreased significantly. The case demonstrates need of complex assessment of the patient with diabetes mellitus. We firmly believe that this could be possible only when diabetology is an integral part of endocrinology.
