Dupilumab představuje biologické léčivo, které inhibuje signalizaci interleukinů (IL) 4 a 13, což jsou klíčové faktory u zánětlivých onemocnění typu 2 včetně atopické dermatitidy. Dostupná data z randomizovaných klinických studií i z běžné klinické praxe potvrzují účinnost a bezpečnost dupilumabu v léčbě atopické dermatitidy. Z léčby mohou výrazně profitovat pacienti s těžší formou atopické dermatitidy primárně nevhodní pro systémovou imunosupresivní léčbu i léčbu inhibitory Janusových kináz, tj. pacienti s proběhlým či probíhajícím maligním onemocněním nebo pacienti se závažnějším infekčním onemocněním v anamnéze nebo se závažnějším infektem právě probíhajícím. Článek představuje pacienta s geneticky prokázaným Loyesovým-Dietzovým syndromem a těžkou formou atopické dermatitidy s infekční komplikací, který je úspěšně léčen dupilumabem.

Dupilumab is a biologic that inhibits interleukins (IL) 4 and 13 signaling, which are key factors in type 2 inflammation in type 2 inflammatory diseases including atopic dermatitis. Available data from randomized clinical trials as well as from routine clinical practice confirm the efficacy and safety of dupilumab in the treatment of atopic dermatitis. Patients with a more severe form of atopic dermatitis who are primarily unsuitable for systemic immunosuppressive treatment or treatment with Janus kinase inhibitors, i.e. patients with past or ongoing malignant disease or patients with a history or ongoing more serious infection, can benefit significantly from the treatment. The article presents a patient with genetically proven Loyes-Dietz syndrome and a severe form of atopic dermatitis with an infectious complication, who is successfully treated with dupilumab.
