Na přelomu 60.–70. let 20. století je na pole plastické chirurgie uveden nový aloplastický materiál – Wichterleho gel. Český vědec, profesor Otto Wichterle, se svým týmem vyvinul v roce 1961 hydrofilní gel z polymerů, který byl díky své hydrofilii, chemické, tepelné a tvarové stabilitě snášen lépe než gely hydrofobního charakteru a splňoval tak lépe nároky kladeny na protetický materiál. V plastické chirurgii se začal využívat k augmentaci a rekonstrukci prsů. K úspěchu přispěla nenáročná předoperační příprava. Materiál byl implantovaný v celkové anestezii submamárním přístupem nad sval, kde byl fixován stehem k fascii. Po operaci byl aplikován fixační korzetový obvaz. Implantovaný materiál se zpočátku osvědčil dobrým pooperačním průběhem s minimálními komplikacemi. V pozdějším pooperačním období se ale projevily závažné komplikace, především infekce a kalcifikace. Dlouhodobé výsledky jsou prezentovány kazuistikami. Dnes se již tento materiál nepoužívá a je nahrazen modernějšími implantáty.
During the period of 1960s and 1970s, a new alloplastic material – Wichterle gel – was introduced in the field of plastic surgery. In 1961, a Czech scientist, prof. Otto Wichterle, had developed, along with his research team, a hydrophilic gel made of polymers, which fulfilled the high standards for prosthetic materials due to its hydrophilic, chemical, thermal and shape stability that provided a better tolerance in the body compared with other hydrophobic gels. Plastic surgeons had started to use the gel for breast augmentations and reconstructions. Success of the gel had been reinforced due to its easy preoperative preparation. The material had been implanted during general anaesthesia via submammary approach over the muscle fixed with a stitch to the fascia. Fixing corset bandage was applied after the surgery. The implanted material had proved to be suitable for postoperative processes with a minimum of complications. In the later postoperative period, however, serious complications occurred – mainly infections and calcifications. Long-term results are presented by case reports. Today, this material is no longer used and it is replaced by more modern implants.

BACKGROUND: The optimal surgical approach to reconnecting bowel ends safely after resection is of great importance. OBJECTIVES: This project is focused on assessment of the perianastomotic microcirculation quality in the short postoperative period when using three different anastomosis techniques in experimental animal. METHODS: The experimental study involved 27 young female domestic pigs divided into three subgroups of 9 animals according to each surgical method of anastomosis construction in the sigmoid colon region: by manual suture, by stapler, or by gluing. Blood microcirculation in the anastomosis region was monitored using Laser Doppler Flowmetry (LDF). Anastomosis healing was evaluated by macroscopic and histological examination. RESULTS: Evaluation of the microcirculation in the anastomosis region showed the smallest decrease in perfusion values in animals reconstructed by suturing (Δ= -38.01%). A significantly more profound drop was observed postoperatively after stapling or gluing (Δ= -52.42% and Δ= -59.53%, respectively). All performed anastomoses healed without any signs of tissue and function pathology. CONCLUSIONS: Sewing, stapling, and gluing techniques for bowel anastomosis each have a different effect on regional microcirculation during 120 min. postoperatively. Nevertheless, the final results of anastomosis healing were found without of any pathology in all experimental animals managed by above mentioned anastomotic techniques.
