Outer membrane vesicles (OMVs), nanoparticles released by Shiga toxin-producing Escherichia coli (STEC), have been identified as novel efficient virulence tools of these pathogens. STEC O157 OMVs carry a cocktail of virulence factors including Shiga toxin 2a (Stx2a), cytolethal distending toxin V (CdtV), EHEC hemolysin, flagellin, and lipopolysaccharide. OMVs are taken up by human intestinal epithelial and microvascular endothelial cells, the major targets during STEC infection, and deliver the virulence factors into host cells. There the toxins separate from OMVs and are trafficked via different pathways to their target compartments, i.e., the cytosol (Stx2a-A subunit), nucleus (CdtV-B subunit), and mitochondria (EHEC hemolysin). This leads to a toxin-specific host cell injury and ultimately apoptotic cell death. Besides their cytotoxic effects, STEC OMVs trigger an inflammatory response via their lipopolysaccharide and flagellin components. In this chapter, we describe methods for the isolation and purification of STEC OMVs, for the detection of OMV-associated virulence factors, and for the analysis of OMV interactions with host cells including OMV cellular uptake and intracellular trafficking of OMVs and OMV-delivered toxins.

Shiga toxin-producing Escherichia coli (STEC) is an important foodborne pathogens, known to cause enteric infections especially diarrhea, mainly attributed to Shiga toxins (Stxs). The use of certain antibiotics for treating this infection is controversial, owing to an increased risk for producing Stxs (Stx 1 and Stx 2). Increased antibiotic resistance is also thought to be involved in the pathogenesis of STEC diseases. The purpose of this study was to analyze the effects of antibiotics on induction of Stx 1 and Stx 2 in clinical STEC isolates and to investigate the relationships between increased resistance and Stx production. Fifteen clinical isolates were treated with sub minimum inhibitory concentrations (Sub MIC) of clinically used antibiotics (ciprofloxacin, fosfomycin, tigecycline, and meropenem), and the changes in expression levels of stx1 and stx2 genes were estimated using qRT-PCR. The expressions of Shiga toxins were found to be increased up to 6.5- and eightfold under ciprofloxacin and tigecycline Sub MIC, respectively. Fosfomycin had weak induction effect of up to twofold, whereas meropenem had the weakest influence on such expression. Resistant isolates were found to be more prone to increased expression of toxins.

INTRODUCTION: Shiga toxin 2a (Stx2a) induces hemolytic uremic syndrome (STEC HUS) by targeting glomerular endothelial cells (GEC). OBJECTIVES: We investigated in a metabolomic analysis the response of a conditionally immortalized, stable glomerular endothelial cell line (ciGEnC) to Stx2a stimulation as a cell culture model for STEC HUS. METHODS: CiGEnC were treated with tumor necrosis factor-(TNF)α, Stx2a or sequentially with TNFα and Stx2a. We performed a metabolomic high-throughput screening by lipid- or gas chromatography and subsequent mass spectrometry. Metabolite fold changes in stimulated ciGEnC compared to untreated cells were calculated. RESULTS: 320 metabolites were identified and investigated. In response to TNFα + Stx2a, there was a predominant increase in intracellular free fatty acids and amino acids. Furthermore, lipid- and protein derived pro-inflammatory mediators, oxidative stress and an augmented intracellular energy turnover were increased in ciGEnC. Levels of most biochemicals related to carbohydrate metabolism remained unchanged. CONCLUSION: Stimulation of ciGEnC with TNFα + Stx2a is associated with profound metabolic changes indicative of increased inflammation, oxidative stress and energy turnover.

Outer membrane vesicles (OMVs) are important virulence tools of enterohaemorrhagic Escherichia coli (EHEC), but other biological functions of these nanostructures are unknown. We tested the hypothesis that modulation of OMV production enables EHEC to resist the intrahost environment during infection by investigating if simulated human gastrointestinal conditions affect OMV production in EHEC O157:H7 and O104:H4. All the conditions tested including a low pH, simulated ileal and colonic media, presence of mucin, intestinal epithelial cell lysate or antimicrobial peptides, as well as iron limitation, significantly increased OMV production by these pathogens. Accordingly, a maximum vesiculation in EHEC O104:H4 was observed immediately after its isolation from a patient's intestine, and rapidly decreased during passages in vitro. Most of the simulated intrahost conditions also upregulated the OMV-associated Shiga toxin 2a (Stx2a), the major EHEC virulence factor, and, as a result, OMV cytotoxicity. The data indicates that upregulation of OMV production by the human gastrointestinal milieu contributes to EHEC survival and adaptation within the host during infection. Moreover, the intrahost increase of vesiculation and OMV-associated Stx2a may augment EHEC virulence.

Cíl práce: Tato studie byla zpracována ve spolupráci se Státní veterinární správou (SVS) s cílem monitorovat výskyt Shiga-toxin produkujících izolátů Escherichia coli ve sterech z jatečně upravených těl prasat a skotu. Materiál a metody: Od června do srpna roku 2013 odebraU pracovníci SVS 168 stěrů ze skotu a 318 z prasat na 157 různých porážkách v ČR Základní zpracování vzorků bylo provedeno ve Státních veterinárních ústavech Praha, Jihlava a Olomouc podle metodického postupu koordinovaného Národní referenční laboratoří (NRL) pro oblast Escherichia coh v rámci rezortu Ministerstva zemědělství. Laboratorní zpracování vzorků vycházelo z normy ISO TS 13136. Výsledky: Z celkem 486 odebraných stěrů bylo detekováno 22 pozitivních vzorků. Získáno bylo 22 izolátů shigatoxigenních E. coli (STEC) a 1 kmen s charakteristikou enterohemoragických E. coli (EHEC). U žádného izolátů nebyly zjištěny geny typické pro enteroagregativní E. coh (EAggEC). Většina získaných kmenů STEC pocházela z prasat (15 kmenů). Gen stx1, byl detekován 2krát (stx1a, stx1d), gen stx2 13krát (12krát stx2e, 1krát Stx2a). Ze vzorků stěrů skotu bylo získáno 8 izolátů STEC. jedenkrát se jednalo o gen stx, (subtyp a), 6krát byl zjištěn gen stx2 (4krát stx2e, 1krát stx2a a 1krát stx2c). Jeden izolát nesl současně geny pro oba typy toxinů stx1a a stx2a. Vždy po jednom izolátů byly detekovány séroskupiny 091,0113,0146 popisované ve spojitosti s onemocněním lidi. U žádného z těchto kmenu ale nebyly detekovány další faktory virulence, typické pro kmeny vyvolávající závažná onemocnění. Závěr: Z výsledků této studie vyplývá celková prevalence výskytu shigatoxigenních E. coli ve sterech z povrchu jatečně upravených těl prasat a skotu 4,5 % a v případě kmenů EHEC pouze 0,2 %. Syrové maso pocházející z tuzemských chovů zřejmě nepředstavuje v souČasnosti významný zdroj STEC pro člověka.
Objectives: This study was performed in cooperation with the State Veterinary Administration (SVA) in order to monitor the occurrence of Shiga toxin-producing Escherichia coli isolates in swabs from the carcasses of pigs and cattle at slaughterhouses. Material and Methods: From June to August 2013, SVA staff took 168 swabs from cattle and 318 from pigs at 157 different slaughters in the Czech Republic. Basic processing of the samples was carried out in the State Veterinary Institutes (SVIs) in Prague, Jihlava and Olomouc according to the methodical process coordinated by the National reference laboratory (NRL) for Escherichia coli (Czech Ministry of Agriculture). The procedure was based on the guideline ISO TS 13136. Results: Out of the 486 swabs, twenty-two positive samples were detected. There were a total of 22 isolates of Shiga toxigenic E. coli (STEC) and 1 strain with the characteristic of enterohemorrhagic E. coli (EHEC). Genes typical for enteroaggregative E. coli (EAggEC) were not found in any of the isolates. Most STEC strains originated from pigs. The stx, gene was detected twice (stxi1a, stx1d) and the stx2 gene 13 times (12 times stx2e, once Stx2a). Seven STEC isolates were detected from samples of cattle origin. One strain was stx1 (stx1a) -positive, the stx2 gene was found 6 times (4 stx2e, 1 stx2a aud 1 stx2c). One isolate carried simultaneously both stxja and stx2a. Each of the serogroups 091,0113 and 0146 described as etiological agents of severe disease in humans were detected only once. None of these strains harbored additional virulence factors typical for strains causing serious illness. Conclusion: Results of this study show the overall prevalence of Shiga toxigenic E. coli of 4.5% and 0.2% of enterohemorrhagic strains in the studied samples. Raw meat originating from local farms does not currently represent an important source of STEC for humans.

A large outbreak caused by a rare Shiga toxin-producing Escherichia coli serotype O104:H4 occurred in Germany in May to July 2011. The National Reference Laboratory for E. coli and Shigella investigated the stool sample from an American tourist with bloody diarrhea who arrived in the Czech Republic from Germany where she consumed salads with raw vegetable a week ago. Using culture of the enriched stool on extended-spectrum β-lactamase agar, we isolated E. coli strain which belonged to serotype O104:H4 as determined by conventional and molecular serotyping. The strain contained the major virulence characteristics of enterohemorrhagic E. coli (stx (2) encoding Shiga toxin 2) and enteroaggregative E. coli (aggA encoding aggregative adherence fimbriae I). This unique combination of virulence traits demonstrated that this strain belongs to the hybrid enteroaggregative hemorrhagic E. coli clone which caused the German outbreak. Using advanced culture and molecular biological approaches is the prerequisite for identification of new, unusual pathogens.

V příspěvku je uvedena stručná kazuistika hemolyticko-uremického syndromu (HUS) po prodromálním průjmu u dvouletého chlapce. Původcem byl enterohemoragický kmen Escherichia coli O145:H28 s produkcí Shiga toxinu 2. Stejný kmen byl zjištěn i u matky a 7letého bratra pacienta, kteří byli asymptomatickými nosiči. Jedná se o první záchyt EHEC tohoto sérotypu jako etiologického agens HUS v České republice.
A brief case report of diarrhea-positive hemolytic-uremic syndrome (HUS) in a two-year-old boy is presented. The causative agent was an enterohemorrhagic Escherichia coli (EHEC) O145:H28 strain producing Shiga toxin 2. Identical strains were isolated from stool samples of the patient's mother and seven-year-old brother who were asymptomatic carriers. This is the first report of EHEC O145:H28 as a cause of HUS in the Czech Republic.
