Outer membrane vesicles (OMVs) are nanoscale proteoliposomes secreted from the cell envelope of all Gram-negative bacteria. Originally considered as an artifact of the cell wall, OMVs are now recognized as a general secretion system, which serves to improve the fitness of bacteria and facilitate bacterial interactions in polymicrobial communities as well as interactions between the microbe and the host. In general, OMVs are released in increased amounts from pathogenic bacteria and have been found to harbor much of the contents of the parental bacterium. They mainly encompass components of the outer membrane and the periplasm including various virulence factors such as toxins, adhesins, and immunomodulatory molecules. Numerous studies have clearly shown that the delivery of toxins and other virulence factors via OMVs essentially influences their interactions with host cells. Here, we review the OMV-mediated intracellular deployment of toxins and other virulence factors with a special focus on intestinal pathogenic Escherichia coli. Especially, OMVs ubiquitously produced and secreted by enterohemorrhagic E. coli (EHEC) appear as a highly advanced mechanism for secretion and simultaneous, coordinated and direct delivery of bacterial virulence factors into host cells. OMV-associated virulence factors are not only stabilized by the association with OMVs, but can also often target previously unknown target structures and perform novel activities. The toxins are released by OMVs in their active forms and are transported via cell sorting processes to their specific cell compartments, where they can develop their detrimental effects. OMVs can be considered as bacterial "long distance weapons" that attack host tissues and help bacterial pathogens to establish the colonization of their biological niche(s), impair host cell function, and modulate the defense of the host. Thus, OMVs contribute significantly to the virulence of the pathogenic bacteria.

Druhá kazuistika se zabývá případem akutní gastroenteritidy u batolete vyvolané třemi patogeny najednou – duální virovou infekcí (rotaviry a noroviry typ I) současně s Escherichia coli O 157:H7 a komplikované rozvojem hemolyticko-uremického syndromu s nutností dialýzy a krevních transfuzí.
The second case report deals with a case of acute gastroenteritis in a toddler caused simultaneously by three pathogens – dual viral infection (rotaviruses and noroviruses type I) together with Escherichia coli O157:H7 and complicated by the development of haemolytic-uremic syndrome with the necessity of dialysis and blood transfusions.

Shiga toxin-produkující Escherichia coli (STEC) jsou celosvětovými původci průjmového onemocnění a nejčastější příčinou vzniku hemolyticko-uremického syndromu (HUS) u dětí. Manifestace infekce STEC je různorodá. Zahrnuje průjmová onemocnění různé tíže, od nekomplikovaného průjmu přes hemoragickou kolitidu, až po rozvoj závažného systémového onemocnění – HUS, který patří mezi nejčastější příčiny akutního poškození ledvin zejména u malých dětí. Vzhledem k možné progresi průjmu v život ohrožující stav by měli být všichni lékaři ošetřující pacienty s průjmovým onemocněním obeznámeni s epidemiologií, patogenezí a klinickými příznaky infekce způsobené těmito bakteriemi. Článek pojednává o historii, epidemiologii, klinických aspektech a diagnostice infekcí STEC.
Shiga toxin-producing Escherichia coli (STEC) are an important cause of diarrhea and the major cause of typical form of hemolytic-uremic syndrome (HUS) worldwide. Manifestation of STEC infection can be diverse, from uncomplicated diarrhea through hemorrhagic colitis, to a serious systemic disease – HUS, that is the most common cause of acute kidney injury, especially in young children. Due to possible progression of diarrhea to life-threatening illness, all physicians treating patients with diarrhea should get familiar with the epidemiology, pathogenesis and clinical symptomes of infection caused by the organism. This article describes the history, epidemiology, pathogenesis, clinical aspects, and diagnostic methods of STEC infections.

Cíl práce: Enterohemoragické Escherichia coli (EHEC) jsou celosvětovými původci průjmu, krvavého průjmu a hemolyticko-uremického syndromu (HUS). Zatímco role EHEC v etiologii HUS v České republice byla popsána v nedávné publikaci, výskyt, vlastnosti a epidemiologie EHEC vyvolávajících průjmová onemocnění nejsou dostatečně známé. V této studii jsme proto analyzovali sérotypy, stx genotypy a faktory virulence kmenů EHEC izolovaných v letech 1965–2013 od pacientů s průjmem, krvavým průjmem a jejich rodinných kontaktů. Dále jsme charakterizovali diagnosticky důležité fenotypy kmenů EHEC, jejich citlivost k antimikrobiálním agens, sezonnost a distribuci podle krajů. Materiál a metody: Kmeny od pacientů byly zaslány jako izoláty E. coli určené séroskupiny do NRL pro E. coli a shigely k průkazu Stx. V souvislosti s epidemiologickým šetřením byly zaslány do NRL vzorky humánního a nonhumánního původu. Sérotypy byly určeny konvenční a molekulární sérotypizací, stx genotypy a non-stx faktory virulence pomocí PCR, a fenotypy a antimikrobiální citlivost pomocí standardních metod. Epidemiologická souvislost izolátů od zvířete a pacienta byla potvrzena metodou pulzní gelové elektroforézy (PFGE). Výsledky: Z 50 kmenů EHEC pocházelo 24 od pacientů s průjmem bez krve, 19 od pacientů s krvavým průjmem, 6 od rodinných kontaktů a 1 od epidemiologicky souvisejícího zvířete. Případy EHEC infekcí se vyskytovaly během celého roku, s vrcholem v květnu až říjnu a nejvíce ve Středočeském a Královéhradeckém kraji. Hromadný výskyt infekce EHEC byl zaznamenán ve 3 rodinách a v 1 případě byl prokázán přenos EHEC infekce z ovce na pacienta. Kmeny EHEC patřily k 5 sérotypům, z nichž více než polovinu tvořily sorbitol-neokyselující (NSF) O157:H7/NM[fliCH7] a třetinu kmeny O26:H11/NM[fliCH11]; sérotypy O111:NM[fliCH8], O118:NM[fliCH25] a O104:H4, stejně jako sorbitol-okyselující (SF) kmeny O157:NM[fliCH7], byly vzácné. Z celkem 7 identifikovaných stx genotypů se všech 7 vyskytlo u NSF EHEC O157, 2 u EHEC O26 a O111, a 1 u EHEC O118, O104 a SF O157. Všechny až na 1 kmen produkovaly Stx. Geny kódující další faktory virulence včetně toxinů (EHEC-hlyA, cdt-V, espP) a adhezinů (eae, efa1, iha, lpf, sfpA) byly přítomny u všech kmenů a jejich spektrum bylo sérotyp-specifické. Nejčastější z nich byl eae kódující adhezin intimin a EHEC-hlyA kódující EHEC hemolyzin. Všechny kmeny EHEC kromě SF O157 obsahovaly terE, kódující rezistenci k teluritu. Všechny kmeny kromě NSF EHEC O157 a EHEC O118 okyselovaly sorbitol a produkovaly ß-D-glukuronidázu. Většina EHEC (89,8 %) byla citlivá ke všem 12 sledovaným antimikrobiálním agens. Závěr: EHEC se vyskytují jako původci průjmu a krvavého průjmu v ČR. Pouze systematický screening těchto patogenů ve stolicích pacientů však umožní odhalit jejich skutečný podíl na etiologii průjmových onemocnění (a rovněž HUS) v ČR, a zařadit tak tato data do evropského kontextu. Onemocnění vyvolaná EHEC jsou hlášena do Evropského centra pro prevenci a kontrolu nemocí (ECDC) v rámci surveillance Food and Waterborne Diseases.
Aim: Enterohemorrhagic Escherichia coli (EHEC) is the cause of diarrhea, bloody diarrhea, and haemolytic uremic syndrome (HUS) worldwide. The role of EHEC in the etiology of HUS in the Czech Republic has recently been described, but the prevalence, characteristics, and epidemiology of EHEC causing diarrhea have not been fully known. Therefore, this study analyzed the serotypes, stx genotypes, and virulence factors in EHEC strains isolated in 1965–2013 from patients with diarrhea or bloody diarrhea and their family contacts. In addition, we characterized diagnostically relevant phenotypes of EHEC strains, their antimicrobial susceptibility, seasonal trends, and distribution by administrative region. Material and methods: Serogrouped E. coli isolates from patients were referred to the National Reference Laboratory (NRL) for E. coli and Shigella for the detection of Stx. Specimens of both human and non-human origin were referred to the NRL for epidemiological investigation. Serotyping was performed by conventional and molecular methods, PCR was applied to stx genotyping and identification of non-stx virulence factors, and standard methods were used for phenotypic analysis and antimicrobial susceptibility testing. The epidemiological link between the human and animal isolates was confirmed using pulsed-field gel electrophoresis (PFGE). Results: Of 50 EHEC strains, 24 were recovered from patients with diarrhea without blood, 19 from patients with bloody diarrhea, six from family contacts, and one from an epidemiologically linked animal. EHEC cases were reported during the whole year, with peaks in May through October, most often in the Central Bohemian and Hradec Králové Regions. EHEC outbreaks occurred in three families: in one of them sheep-to-human transmission of EHEC was detected. The EHEC strains were assigned to five serotypes, with more than half of them being non-sorbitol fermenting (NSF) O157:H7/NM[fliCH7] and a third being strains O26:H11/NM[fliCH11]; serotypes O111:NM[fliCH8], O118:NM[fliCH25], and O104:H4, similarly to sorbitol-fermenting (SF) strains O157:NM[fliCH7], were rare. Of seven stx genotypes identified, all were present in NSF EHEC O157, two in each of EHEC O26 and O111, and one in each of EHEC O118, O104, and SF O157. All but one strain were Stx producers. Genes encoding other virulence factors including toxins (EHEC-hlyA, cdt-V, and espP) and adhesins (eae, efa1, iha, lpf, and sfpA) were detected in all strains and their occurrence was serotype specific. The most common of these genes were eae encoding adhesin intimin and EHEC-hlyA encoding EHEC hemolysin. All EHEC strains but SF O157 harboured terE encoding tellurite resistance. All strains except NSF EHEC O157 and EHEC O118 fermented sorbitol and produced ß-D-glucuronidase. Most (89.8%) EHEC strains were susceptible to all 12 antimicrobials tested. Conclusion: EHEC strains cause diarrhea and bloody diarrhea in the Czech Republic. Nevertheless, only a systematic screening of the stool from patients with diarrhea can make it possible to elucidate their actual role in the etiology of diarrheal diseases (as well as HUS) in the Czech Republic and to consider the data in the European context. EHEC cases are reported to the European Centre for Disease Prevention and Control (ECDC) within the Food and Waterborne Diseases Surveillance Network.

Enterohemoragické kmeny Escherichia coli, které byly poprvé popsány v USA v roce 1983, jsou důležitými, celosvětově rozšířenými zoonotickými patogeny s významným epidemickým potenciálem. Vedle nekomplikovaného průjmu mohou vyvolávat u dětí i dospělých závažné komplikace, a to hemolyticko-uremický syndrom a vzácně trombotickou trombocytopenickou purpuru. Hemolyticko-uremický syndrom je nejčastější příčinou akutní renální insuficience v dětském věku. V článku jsou vedle etiologie, epidemiologie, klinického obrazu, komplikací, léčby a prevence zmíněna i nejnovější data z Národní referenční laboratoře pro Escherichia coli a shigely v Praze o výskytu a komplikacích této infekce v České republice.
Enterohaemorrhagic Escherichia coli first described in the United States in 1983, are important, worldwide spread zoonotic pathogens with a significant outbreak potential. Besides uncomplicated diarrhoea, they can cause severe complications in children and adults including haemolytic – uremic syndrome and rarely thrombotic thrombocytopenic purpura. Haemolytic – uremic syndrome is the most frequent cause of acute renal failure in children. In this article we review present knowledge about etiology, epidemiology, clinic, treatment and prevention of these infections including new data from National Reference Laboratory for Escherichia coli and shigella in Prague about the occurrence of enterohaemorrhagic Escherichia coli in the Czech Republic.

V příspěvku je uvedena stručná kazuistika hemolyticko-uremického syndromu (HUS) po prodromálním průjmu u dvouletého chlapce. Původcem byl enterohemoragický kmen Escherichia coli O145:H28 s produkcí Shiga toxinu 2. Stejný kmen byl zjištěn i u matky a 7letého bratra pacienta, kteří byli asymptomatickými nosiči. Jedná se o první záchyt EHEC tohoto sérotypu jako etiologického agens HUS v České republice.
A brief case report of diarrhea-positive hemolytic-uremic syndrome (HUS) in a two-year-old boy is presented. The causative agent was an enterohemorrhagic Escherichia coli (EHEC) O145:H28 strain producing Shiga toxin 2. Identical strains were isolated from stool samples of the patient's mother and seven-year-old brother who were asymptomatic carriers. This is the first report of EHEC O145:H28 as a cause of HUS in the Czech Republic.

V příspivku je uvedena stručná kazuistika diarea-pozitivního hemolyticko-uremického syndromu u dvouleté dívky, který skoneil úmrtím pacientky. Puvodcem bylo enterohemoragické Escherichia coli (EHEC) O26 s produkcí Shiga toxinu 2. V elánku je zduraznino, že toto vážné onemocniní mohou, kromi nejeastiji uvádi- ného sérotypu E. coli O157:H7, vyvolat i kmeny dalších séroskupin: O26, O111, O103 a O145. V NRL pro E. coli a shigely SZÚ jsme za ideálních okolností schopni zjistit poítomnost EHEC do 24, resp. do 48 hodin.
We report a fatal case of diarrhea-associated hemolytic uremic syndrome (HUS) in a 2 year-old girl, caused by enterohemorrhagic Escherichia coli (EHEC) O26 producing Shiga toxin (Stx) 2. This case demonstrates that in addition to E. coli O157:H7 which is the most common cause of HUS, strains of other serogroups (O26, O111, O103, O145) can also cause this severe disease. Stool specimens from patients with suspected EHEC infections should be sent to NRL for E. coli and Shigellae at NIPH for molecular and phenotypic detection of stx/Stx that is feasible within 24-48 h.
