Východisko: Sekvenční protokolární biopsie transplantovaných ledvin poskytují důležité informace o histomorfologických změnách funkčně stabilních štěpů a jejich vývoji v čase. V současné době jsou diskutovány jejich indikace z pohledu přínosu pro pacienta a rizika možných komplikací. Cfl: Jedním z cílů naší retrospektivní monocentrické studie, primárně zaměřené na detekci subklinických histologických změn u štěpů se stabilizovanou funkcí, bylo zhodnocení výskytu komplikací protokolárních biopsií a jejich vlivu na prognózu ledvinných štěpů a pacientů. Metodika: V souboru 424 protokolárních biopsií provedených během prvního roku po transplantaci byl retrospektivně vyhodnocen celkový výskyt biopťických komplikací. U klinicky závažných i nevýznamných komplikací byl posouzen následný terapeutický postup a dopad na další potransplantační průběh. Výsledky: U žádného pacienta nedošlo v souvislosti s biopsií k úmrtí či ke ztrátě ledvinného štěpu, nebylo nutno přistoupit ani k chirurgické revizi z důvodu nestavitelného krvácení. Ostatní závažné komplikace se vyskytly pouze ve třech případech (0.7 %), kdy u dvou pacientů bylo indikováno podání erytrocytárního koncentrátu při hemodynamicky významném perirenálním hematomu a v jednom případě byla provedena superselektivní embolizace arteriálního raménka parenchymové píštěle. Drobné komplikace byly zaznamenány v 27 případech (6.4 %). bez dalšího nepříznivého vlivu na pacienta a transplantovanou ledvinu. Závěry: Incidence komplikací sekvenčních protokolárních biopsií transplantovaných ledvin byla na našem pracovišti nízká, srovnatelná s daty prezentovanými jinými transplantačními centry na obdobných souborech pacientů. Domníváme se. že přesná znalost histologického obrazu a jeho vývoje u konkrétního pacienta je nezbytnou podmínkou nastavení adekvátní imunosupresivní terapie a představuje významný přínos pro zlepšení prognózy štěpu vysoce převažující možná rizika této procedury.
Introduction: Sequential protocol biopsies of transplanted kidneys provide important information about histomorphological changes in functionally stable grafts and its evolution over time. There are currently discussed their indications in terms of benefit to the patient and the risk of possible complications. Aim: One of the objectives of our retrospective monocentric study, primarily aimed at detecting subclinical histological changes in grafts with stable function, was to evaluate the incidence of complications of protocol biopsies and their influence on the pro gnosis of renal grafts and patients. Methods: The set of 424 protocol biopsies performed during the first year after kidney transplantation was retrospectively evaluated for the overall incidence of biopsy complications. In clinically severe and minor complications the accepted therapeutic approa ch and the impact on the following post-transplant course was reviewed. Results: No patient died in connection with the procedure and there was no graft loss. There was no need to proceed to surgical revision due to unstoppable bleeding. Other serious complications occurred in only three cases (0.7%) when two patients were indicated for blood transfusion due to hemodynamically significant hematoma and in one patient the superselective arterial embolisation of parenchymal fistula was performed. Minor complications were reported in 27 cases (6.4%), without any adverse ef fect on the prognosis of the patient and transplanted kidney. Conclusions: The incidence of complications of sequential protocol biopsies of transplanted kidneys at our center was low and comparable with the data presented by other transplant centers on a comparable group of patients. We believe that accurate knowledge of the histological picture and its development in a particular patient is a necessary condition for setting adequate immunosuppressive therapy and represents a significant contribution to improving the prognosis of graft, which greatly exceeds the potential risks of the procedure.

Východisko: Nefrotoxicita kalcineurinových inhibitorů představuje důležitý faktor významně limitující dlouhodobé přežití transplantovaných ledvin. Cíl: Cílem naší práce bylo posoudit výskyt subklinické a klinicky manifestní nefrotoxicity kalcineurinových inhibitorů v sekvenčních protokolárních biopsiích transplantovaných ledvin a porovnat cyklosporin a takrolimus z hlediska jejich schopnosti ovlivnit rozvoj toxických změn. Metodika: V souboru 158 pacientů bylo provedeno celkem 424 protokolárních biopsií během prvního roku po transplantaci. Ve skupinách se subklinickou a klinicky manifestní nefrotoxicitou byl sledován typ podávaného kalcineurinového inhibitoru a vývoj histologického Skóre nefrotoxicity kalcineurinových inhibitorů v průběhu prvního roku po transplantaci a skupiny byly vzájemně porovnány. Výsledky statistických testů byly označeny jako signifikantní při hladině významnosti P < 0,05. Výsiedky: Subklinické nefrotoxicita se vyskytla ve 3. týdnu u 17 (10,8 %) pacientů s perzistencí nálezu po redukci dávky kalcineurinového inhibitoru ve 3. měsíci u 14 (9,9 %) a v 1. roce u 12 (9,7 %) nemocných. Klinicky manifestní nefrotoxicita byla pozorována u 16 (10.1 %). 13 (9,2 %) resp. 11 (8.9 %) pacientů. Nefrotoxicita se vyskytla nezávisle na užitém kalcineurinovém inhibitoru a vedla jak v případě cyklosporinu. tak takrolimu k podobnému nárůstu Skóre nefrotoxicity kalcineurinových inhibitorů (1,22 ± 1.04 resp. 1.51 ± 0.80) na konci jednoročního sledování (P < 0.05). Závěry: Subklinické i klinicky manifestní nefrotoxicita postihuje v časném potransplantačním období srovnatelné procento ledvinných štěpů. Vyskytuje se nezávisle na typu užitého kalcineurinového inhibitoru, ve vysokém procentu perzistuje v opakované biopsii a vede k progresi toxických změn na konci jednoročního sledování.
Introduction: Calcineurin inhibitors nephrotoxicity represents important risk factor markedly limiting long-term renal graft survival. Aim: The aim of our study was to assess the incidence of subclinical and clinically manifest nephrotoxicity of calcineurin inhibitor s in sequential protocol biopsies of transplanted kidneys and compare cyclosporine and tacrolimus in their toxic effects. Methods: A total of 424 protocol biopsies were performed in the set of 158 patients within the first year after transplantation. In the groups with subclinical and clinically manifest nephrotoxicity, the type of calcineurin inhibitor used and development of histo logical Calcineurin inhibitors nephrotoxicity score were studied, and the groups were compared. The results of statistical tests were considered to be significant at a level of P < 0.05. Results: In the third week, subclinical nephrotoxicity occurred in 17 (10.8 %) patients with persistence after calcineurin inhibitor do se reduction in the third month in 14 (9.9 %) and after 1 year in 12 (9.7 %) patients. Clinically manifest nephrotoxicity was seen in 16 (10.1 %), 13 (9.2 %) and 11 (8.9 %) patients, respectively. Nephrotoxicity occurred irrespective of the calcineurin inhibitor u sed and in both cyclosporine and tacrolimus led to a similar increase in Calcineurin inhibitors nephrotoxicity score (1.22 ± 1.04 and 1.51 ± 0.80, respectively) at the end of one-year follow-up (P < 0.05). Conclusions: The incidence of subclinical and clinically manifest nephrotoxicity in the early post-transplant period is comparable and it appears independently of the calcineurin inhibitor used. These changes persist in repeated biopsies in a large proportion of ca ses and lead to significant progression of toxic damage at the end of one-year follow-up.

Východisko: Dlouhodobá aplikace kalcineurinových inhibitorů (Cl) u pacientů po transplantaci ledviny je díky jejich nefrotoxickému potenciálu často spojena s rozvojem ireverzibilního poškození štěpu. Cn: Cílem naší práce bylo vyhodnotit přínos opakovaného stanovení sérového kreatininu (Skr) a glomerulární filtrace (GF) v detekci časných projevů nefrotoxicity Cl během prvního roku po transplantaci. Metodika: V souboru 158 nemocných bylo v prvním roce po transplantaci provedeno 424 protokolárních biopsií ledvinných štěpů. Ze souboru byly vyčleněny 3 sledované skupiny - skupina kontrolní s normálním histologickým nálezem, skupina s histologickými projevy toxicity s normální a stabilizovanou funkcí štěpu a skupina s klinicky manifestní toxicitou s dysfunkcí štěpu. Ve skupinách byl hodnocen vývoj Skr, GF a histologického obrazu během jednoročního sledování a skupiny byly vzájemně porovnány. Výsledky statistických testů byly označeny jako signifikantní při hladině významnosti P < 0.05. Výsledky: Normální histologický nález byl pozorován ve 3. týdnu pouze u 30 pacientů (19.0 %). Histologické projevy toxicity se vyskytly ve á. týdnu u 33 (20,1 %) pacientů s perzistencí nálezu po redukci dávky Cl ve 3 měsíci u 27 (19,0 %) a v 1. roce u 23 (18,5 %) nemocných. Více než 50 % těchto toxických změn bylo klinicky němých s normální hodnotou Skr a bez signifikantních změn GF (P < 0,05). Závěry: Nefrotoxicita Cl postihuje významné procento ledvinných štěpů již v prvním roce po transplantaci. Skr dobře neodráží stupeň toxického poškození štěpu v tomto období a limitované jsou i možnosti hodnocení změn GF. Protokolární biopsie se jeví jako vhodný prostředek detekce toxicity Cl a kontroly její perzistence.
Introduction: The long-term administration of calcineurin inhibitors (CI) in patients after kidney graft transplantation is due to their nephrotoxic potential frequently accompanied by irreversible graft damage. Aim: The purpose of our study was to determine the contribution of repeated measeurement of serum creatinine (Scr) and glomerular filtration rate (GFR) in the detection of early nephrotoxic changes of CI in the course of the first year after transplantation. Methods: 424 protocol biopsies of transplanted kidneys were conducted in the set of 158. Three groups were detached from the initial cohort – comparison group with normal histological findings, group with histological signs of toxicity but normal and stabilised graft function and group with histological evidence of toxicity and an impaired graft function. All of the studied groups were monitored for the development of Scr, GFR and histological picture in the course of the one-year monitoring, and cross-comparisons of the groups were carried out. Test results were labelled as significant where the level of statistical significance P < 0.05 was achieved. Results: In the third week normal histological findings was seen only in 30 patients (19.0%). Histological signs of toxicity occurred in the third week in 33 (20.1%) patients, with persistence after CI dose reduction in the third month in 27 (19.0%) and in the first year in 23 (18.5%) patients. More than 50% of the toxic changes were clinically silent with normal level of Scr and without significant changes in GFR (P < 0.05). Conclusions: CI nephrotoxicity affects a significant percentage of renal grafts as early as in the first year after transplantation. Scr and GFR does not adequately reflect the degree of the toxic graft damage. Protocol biopsies seem to be a good tool for the detection of CI toxicity and for the control of its persistence.
