AIMS: Sub-analysis of a retrospective nation-wide observational analysis of heart failure (HF) epidemiology reported to the Czech National Registry of Reimbursed Health Services between 2012 and 2018 aimed at angiotensin-converting enzyme inhibitors (ACEI), angiotensin-II-receptor antagonists (ARB) and angiotensin receptor blocker/neprilysin inhibitor (ARNI) use. METHODS AND RESULTS: ACEi and ARBs were generally used in 87.6% of all HF patients in 2012 (n=154 627); 84.5% in 2013 (n=170 861); 83.5% in 2014 (n=186 963); 81.6% in 2015 (n=198 844); 80.1% in 2016 (n=205 793); 78.0% in 2017 (n=212 152) and in 76.7% in 2018 (n=219 235). In a sub-analysis of patients with a medical procedure and/or examination using an I50.x ICD code accounted for in the given year, ACEi and ARBs were generally used in 99.3% in 2012 (n=63 250); 96% in 2013 (n=62 241); 95.2% in 2014 (n=64 414); 93.3% in 2015 (n=65 217); 91.8% in 2016 (n=65 236); 90.1% in 2017 (n=65 761) and in 88.6% in 2018 (n=66 332). In 2018, the majority of patients with HF were prescribed ramipril (n=49 909; 17.5%) and perindopril (n=44 332; 15.5%). The mostly prescribed ARBs in 2018 were telmisartan (n=18 669; 6.5%); losartan (n=13 935; 4.9%) and valsartan (n=4 849; 1.7%). In 24.5% of cases, ACEIs and ARBs were prescribed in a fixed combination with another drug. ARNI became gradually more prescribed from 2018 (n=9 659 in November 2020). CONCLUSION: In an analysis of ACEIs, ARBs and ARNIs utilization in all patients treated for heart failure in the given year in the whole country, we found a comparable rate of drug prescription in comparison with specific heart failure registries. This indicates a good translation of current standard of care into common clinical practice. Ramipril and perindopril remained the mostly prescribed ACEIs and telmisartan became the mostly prescribed ARB. Since 2018, ARNIs began to be widely prescribed.

Chronické srdeční selhání (CHSS), tak jako každé chronické onemocnění, představuje medicínskou i socioekonomickou zátěž. Pacientů s CHSS přibývá a naší snahou je v praxi aplikovat co nejefektivnější způsob péče, optimálně s využitím telemedicínských postupů. Klíčová je u CHSS včasná diagnostika, nastavení na terapii a redukce rehospitalizací. U pacientů, u kterých nad rámec běžné péče připojíme i telemedicínské sledování (telefonická konzultace, přenos fyziologických dat, přenos dat z implantabilních zařízení, telekonzultace), a zejména pokud jsme schopni poskytnout zázemí dobře fungujícího telemedicínského centra, které je schopné na data promptně reagovat, dokážeme včas zhoršení zdravotního stavu rozpoznat a zareagovat na něj. Telemedicína přináší benefit ve všech aspektech péče, nejvíce však v rámci redukce rehospitalizací, což souvisí s redukcí morbidity i mortality, a tyto postupy jsou efektivní i při šetření nákladů na péči u pacientů s CHSS.
Chronic heart failure (HF), like any chronic disease, is a medical and socioeconomic burden. The number of patients with HF is increasing and our effort is to apply the most effective way of care in practice, preferably together with using telemedicine. Early diagnosis, therapy establishment and reduction of rehospitalizations play the key role in HF management. For patients for whom we add telemedicine (telephone consultation, physiological data transfer, data transfer from implantable devices, teleconsultation) and especially if we are able to provide the background of a well-functioning telemedicine center that is able to respond promptly to the data, we can recognize and respond to the deterioration in health status in a timely manner. Telemedicine has benefit in all aspects of care mentioned above, but mostly in the reduction of re-hospitalizations, which is related to the reduction of morbidity and mortality. Telemedicine in HF is also cost-effective.
Chronické srdeční selhání je významným medicínským a socioekonomickým problémem. Nemocní s chronickým srdečním selháním jsou zatíženi vysokou morbiditou a vyžadují systematickou péči, která zahrnuje opakovanou edukaci stran režimových opatření, postupnou uptitraci terapie do maximálních tolerovaných dávek, péči o pacienty s mechanickými srdečními podporami či paliativní péči. Opakované hospitalizace pro srdeční selhání významně zhoršují kvalitu života pacientů, nepříznivě ovlivňují jejich mortalitu a jsou také ekonomickou zátěží pro zdravotní systém. Vzhledem k narůstající prevalenci srdečního selhání je pro poskytování náležité systematické péče nezbytná spolupráce na více úrovních zdravotního systému, především na úrovni kardiologů a praktických lékařů.
Chronic heart failure is a major medical and socioeconomic problem. Patients with chronic heart failure are burdened by high morbidity and require systematic care, which includes repeated education of the regimen, gradual uptitration of therapy to the maximum tolerated doses, care for patients with mechanical cardiac support or palliative care. Repeated hospitalizations for heart failure significantly worsen the quality of life of patients, adversely affect their mortality and are also an economic burden on the health system. Given the growing prevalence of heart failure, cooperation at multiple levels of the health system, especially at the level of cardiologists and general practitioners, is essential for the provision of appropriate systematic care.

Chronické srdeční selhání (CHSS) je v populaci časté a je spojeno se špatnou prognózou navzdory pokrokům v léčbě. V poslední dekádě probíhaly studie s kardiovaskulárními výslednými parametry u pacientů s diabetes mellitus 2. typu (DM2T) užívajících inhibitory sodíko-glukózového kontransportéru 2 (SGLT2i). U tří těchto inhibitorů SGLT2 (empagliflozin, canagliflozin a dapagliflozin) byly prokázány příznivé účinky na snížení počtu hospitalizací pro srdeční selhání. V poslední době navíc bylo zjištěno, že dapagliflozin snížil riziko zhoršení srdečního selhání nebo úmrtí z kardiovaskulárních příčin u pacientů s chronickým srdečním selháním se sníženou ejekční frakcí levé komory, nezávisle na přítomnosti diabetu 2. typu. Probíhá a brzy bude prezentována řada dalších studií u pacientů se srdečním selháním se sníženou nebo zachovanou ejekční frakcí levé komory. Tento souborný dokument shrnuje nedávné důkazy z klinických studií a diskutuje roli inhibitorů SGLT2 při léčbě srdečního selhání, než budou výsledky probíhajících studií u různých populací pacientů s CHSS.
Heart failure (HF) is common and associated with a poor prognosis, despite advances in treatment. Over the last decade cardiovascular outcome trials with sodium-glucose co-transporter 2 (SGLT2) inhibitors in patients with type 2 diabetes mellitus have demonstrated beneficial effects for three SGLT2 inhibitors (empagliflozin, canagliflozin and dapagliflozin) in reducing hospitalisations for HF. More recently, dapagliflozin reduced the risk of worsening HF or death from cardiovascular causes in patients with chronic HF with reduced left ventricular ejection fraction, with or without type 2 diabetes mellitus. A number of additional trials in HF patients with reduced and/or preserved left ventricular ejection fraction are ongoing and/or about to be reported. The present position paper summarises recent clinical trial evidence and discusses the role of SGLT2 inhibitors in the treatment of HF, pending the results of ongoing trials in different populations of patients with HF.
BACKGROUND AND AIMS: The prognosis of patients with heart failure (HF) is still generally unfavorable. HF with reduced ejection fraction (HFrEF) patients reach target medication doses in very low percentages in daily clinical practice. HF disease management programs (DMP), including nurse and telemedicine support that facilitate achieving target medication doses, may improve the unfavorable prognosis. METHODS: We retrospectively analyzed the data of 738 patients with HFrEF who were followed in a single HF center during the years 1975-2011, for 6.4 (median) years. DMP, nurse and telemedicine support is established at this center. RESULTS: The group achieved left ventricle (LV) recovery after the HF treatment. The median LV ejection fraction improved from 25.0% at baseline to 50.0% at the time of the latest data collection. The proportion of NYHA II, III and IV classes decreased from 27.6%, 30.2% and 29.7% to 26.6%, 7.2% and 0.1%, respectively while the proportion of NYHA class I increased from 12.5% to 66.1%. Median NT-proBNP decreased from 975.0 to 324.0 pg/mL. The survival of the patient group was favorable; 79.7% survived 18.1 years after diagnosis of HF. A high percentage of the patients received recommended target or higher than target doses of angiotensin-converting enzyme inhibitors (82.0%) and beta-blockers (78.1%). CONCLUSION: The established pharmacotherapy resulted from an effective DMP and this contributed to the favorable prognosis.

Úvod: Echokardiografie hraje vedoucí roli v diagnostice diastolické funkce levé komory srdeční. Dosud jsme měli k dispozici pouze omezená data o vlivu věku na různé parametry diastolické funkce. Metodika: Retrospektivně jsme analyzovali databázi echokardiografických nálezů pacientů vyšetřených na našem pracovišti. Do studie jsme zahrnuli pouze pacienty se sinusovým rytmem, normální ejekční frakcí levé komory, bez chlopenní vady nebo jakékoliv jiné signifikantní echokardiografické patologie. Manuálním prohledáním lékařských záznamů jsme pacienty rozdělili do tří skupin na základě jejich anamnézy - skupina zdravých pacientů, skupina pacientů s arteriální hypertenzí a skupina s ischemickou chorobou srdeční. Výsledky: Zařazeno bylo celkem 999 pacientů průměrného věku 60,1 ± 14,4 roku, 48,5 % byli muži. Skupina zdravých pacientů zahrnovala 363 subjektů, skupina s arteriální hypertenzí 429 subjektů a skupina s ischemickou chorobou srdeční 207 subjektů. Transmitrální poměr E/A vlivem věku postupně klesal z průměru 1,65 u pacientů mladších 30 let na 0,78 u pacientů starších 80 let. Septální a laterální rychlosti mitrálního anulu v dopplerovském tkáňovém zobrazení (e´ sep a e´ lat) s věkem poklesly, stejně jako septální a laterální e´/a´ poměr. Septální, laterální a průměrný poměr E/e´ s věkem rostly shodně jako decelerační čas vlny E (DtE). S výjimkou deceleračního času vlny E nebyly χ2 testem nalezeny žádné statisticky významné rozdíly v měřených parametrech mezi třemi srovnávanými skupinami. Závěr: Echokardiografické parametry diastolické funkce se s přibývajícím věkem fyziologicky zhoršují. Na základě této studie mohou být určeny přesnější hraniční hodnoty měřených parametrů diastolické funkce pro různé věkové skupiny, než jsou doposud dostupné.
Introduction: Echocardiography plays a pivotal role in the diagnosis of left ventricular diastolic function. There has been only limited data on effects of age on different parameters of left ventricular diastolic function. Methods: We retrospectively searched and analysed a computerised echocardiography database data of subjects investigated at our department. We enrolled only subjects with sinus rhythm, normal left ventricular ejection fraction, without any valvular disease or any other significant echocardiographic pathology. By a subsequent manual search of hospital records we divided subjects into three groups based on their clinical history - group of healthy patients, group of patients with arterial hypertension and a group with coronary artery disease. Results: We enrolled a total of 999 subjects, mean age 60.1 ± 14.4 years, 48.5% were men. There were 363 healthy subjects, 429 subjects with arterial hypertension and 207 subjects with coronary artery disease. Transmitral E/A ratio decreased with age from mean 1.65 in subjects aged under 30 years to 0.78 in subjects over 80 years of age (p < 0.001). Tissue doppler velocities of septal and lateral mitral anulus (Em sep and Em lat) decreased with age, as did the septal and lateral e´/a´ ratio. The septal, lateral and average E/Em ratio increased with age, as did the E wave deceleration time (DtE) (all parameters with p < 0.001). Except for DtE, Chi-square goodness of fit test did not find any statistically significant differences in the measured para- meters between the three compared groups. Conclusions: Echocardiographic parameters of diastolic function physiologically deteriorate with age. Based on this study, more exact cut-off values can be adopted for different age groups and measured diastolic parameters, than are currently available.
