Nádory appendixu a pseudomyxom peritonea (PMP) jsou vzácné. Nejčastějším zdrojem PMP jsou perforované epiteliální nádory appendixu. Toto onemocnění je charakterizováno přítomností částečně na povrchy adherujícího mucinu různého stupně konzistence. Samotné apendikální mukokély jsou taktéž velmi vzácné a zpravidla jejich léčba zahrnuje pouze prostou apendektomii. Cílem této práce bylo vytvořit aktuální přehled doporučení pro diagnostiku a léčbu těchto malignit podle aktuálních doporučení skupiny PSOGI (The Peritoneal Surface Oncology Group International) a Modré knihy České onkologické společnosti ČLS JEP.
Appendiceal tumors and pseudomyxoma peritonei (PMP) are rare tumors. Perforated epithelial tumors of the appendix are the most common source of PMP. This disease is characterized by the presence of mucin of varying degrees of consistency, partially adherent to the surfaces. Appendiceal mucoceles themselves are also very rare and usually their treatment involves only a simple appendectomy. The aim of this study was to provide an up-to-date review of the recommendations for the diagnosis and treatment of these malignancies according to the current guidelines of The Peritoneal Surface Oncology Group International (PSOGI) and the Blue Book of the Czech Society for Oncology of the Czech Medical Association of J. E. Purkyně (ČOS ČLS JEP).
OBJECTIVES: The aim of this study is to evaluate the results of treatment of diffuse malignant peritoneal mesothelioma (DMPM) by cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) at a single center. METHODS: We conducted a retrospective single-center observational cohort study of consecutive patients with DMPM treated by CRS-HIPEC at the Department of Surgery I of the University Hospital in Olomouc, Czech Republic. RESULTS: Data on a total of 16 patients were processed. The study group of 16 patients had six (37.5 %) women. The mean age was approximately 62 years. Complete cytoreduction was achieved in all patients (100 %) (CC0: 75 %, CC1: 25 %). All patients underwent a closed form of HIPEC with cisplatin and doxorubicin for 90 min. The mean hospital stay was 13.5 days, including 4.38 days in the ICU (13.5 ± 5.07 and 4.38 ± 1.49, respectively). Major postoperative complications (CD grades 3–4) occurred in four patients (25 %). In-hospital mortality was 6.25 %. In the study group, the median overall survival was 20 months, and the median disease-free survival was 10.3 months. CONCLUSIONS: Also under the conditions at our specialized center, CRS-HIPEC is considered as an effective, affordable, and safe therapy with OS, DFS, morbidity, and mortality rates comparable to those reported in the literature (Tab. 5, Fig. 2, Ref. 28).

Maligní mezoteliom pleury patří k nejagresivnějším maligním onemocněním s velmi špatnou prognózou. Optimální je multimodální způsob léčby, který zahrnuje tři modality − chemoterapii, radioterapii a cytoreduktivní chirurgii. Možností, jak zlepšit dosavadní výsledky léčby mezoteliomu, je zařazení dalších metod léčby, např. hypertermické peroperační intrapleurální chemoterapie či fotodynamické terapie. Na I. chirurgické klinice Fakultní nemocnice Olomouc se kombinace hypertermické peroperační chemoterapie spolu s cytoreduktivní chirurgií v rámci multimodální terapie maligního mezoteliomu využívá od roku 2017. Autoři uvádějí případ 47letého muže s maligním epiteloidním mezoteliomem pleury vlevo. Po neoadjuvantní chemoterapii byla u tohoto pacienta provedena extrapleurální pneumonektomie s peroperačním intrakavitárním podáním hypertermické chemoterapie. Pooperační průběh byl komplikován nestabilitou oběhu a později tenzním chováním fluidotoraxu se známkami srdečního selhání. Pacient je v současnosti 8 měsíců po operaci a bez známek recidivy nemoci.
Malignant pleural mesothelioma is one of the most aggressive malignancies with a very poor prognosis. Multimodal treatment consisting of three modalities – chemotherapy, radiotherapy and cytoreductive surgery is optimal. The use of new therapeutic modalities, such as hyperthermic intraoperative intrapleural chemotherapy or photodynamic therapy can improve the current results of mesothelioma treatment. Combined hyperthermic intraoperative chemotherapy and cytoreductive surgery as part of multimodal therapy of malignant mesothelioma has been used since 2017 at the 1st Department of Surgery of University Hospital Olomouc. The authors report the case of a 47-year-old man with malignant epithelioid mesothelioma of the left pleural cavity. Following neoadjuvant chemotherapy, this patient underwent extrapleural pneumonectomy with intraoperative intracavitary hyperthermic chemotherapy. The postoperative period was complicated by circulatory instability and later by tension fluidothorax with signs of heart failure. Currently, 8 months have elapsed from the surgery and the patient remains without any signs of recurrence.

BACKGROUND: Cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) is a treatment modality for peritoneal surface malignancies with efficacy reported in many trials. Discrepancies, however, in the indication criteria, the extent of the surgical procedure, HIPEC regimens and toxicity evaluation represent a problem when comparing this method with other therapeutic modalities. METHODS: We describe the initial experience with CRS/HIPEC using different chemotherapy regimens (oxaliplatin, cisplatin, mitomycin C and doxorubicin) at the Comprehensive Oncology Centre Olomouc. RESULTS: A perioperative mortality of 2% and perioperative morbidity of 11%, according to Clavien-Dindo were observed. Interestingly, all these patients underwent HIPEC with oxaliplatin 460 mg/m2. The median duration of admission to hospital was 6 days in the intensive care unit (range 2-28 days) and 7 days in the surgical ward (range 1-21 days). Hospital admission did not exceed 2 weeks in 75% of patients. These results are consistent with the published results of large centres performing this treatment modality mainly due to pre-operative preparation of patients and pre-treatment and post-treatment management of HIPEC/CRS toxicity. Evaluation of the efficacy in terms of time to progression and overall survival (OS) is limited by the short follow up period. CONCLUSION: CRS/HIPEC performed is a safe method with low perioperative mortality.
