Navzdory klesající incidenci a účinné prevenci představuje karcinom hrdla dělohy v České republice nadále veliký společenský a medicínský problém. Ročně se v naší zemi diagnostikuje více než 700 případů zhoubného onemocnění hrdla dělohy, přičemž asi 300 pacientek ročně tomuto onemocnění podlehne. Mezioborová spolupráce, zkušenosti jednotlivých aplikujících odborníků (lékařů, sester, medicínských fyziků a asistentů atd.), moderní přístrojové vybavení i dostupnost inovativních léků mají obrovský dopad na stále se zlepšující prognózu našich pacientek. Přehledový článek sumarizuje aktuální postupy a doporučení v nechirurgické léčbě karcinomu hrdla dělohy.
Cervical cancer remains a significant social and medical problem in the Czech Republic despite a decrease in incidence and effective prevention measures. Every year, over 700 cases of malignant cervix disease are diagnosed in the country, and about 300 patients die from the disease. However, interdisciplinary cooperation, the expertise of individual healthcare professionals (such as doctors, nurses, medical physicists, and assistants), modern equipment, and the availability of innovative drugs have contributed to an improving prognosis for patients. This review article highlights the current procedures and recommendations for non-surgical treatment of cervical cancer.
V rámci paliativní léčby cholangiocelulárního karcinomu byla donedávna široce využívána chemoterapie, zejména režimy cisplatina + gemcitabin a FOLFOX. V poslední době došlo k výraznému rozšíření možností léčby cholangiocelulárního karcinomu se specifickými mutacemi – u nádorů s fúzí genu FGFR byla prokázána účinnost tyrosinkinázového inhibitoru FGFR1–3 pemigatinibu, u nádorů s aktivační mutací IDH1 byl účinný ivosidenib a u HER2-pozitivních nádorů byl pozorován přínos anti-HER2 léčby (pertuzumab + trastuzumab, zanidatamab). Ukazuje se rovněž, že část pacientů s pokročilým cholangiocelulárním adenokarcinomem by mohla být úspěšně léčena checkpoint inhibitory (CPI): v této souvislosti přinesla robustní data studie TOPAZ-1, ve které vedlo přidání CPI durvalumabu k chemoterapii cisplatina + gemcitabin k signifikantnímu prodloužení přežití bez progrese i celkového přežití pacientů, aniž by přitom došlo k významnějšímu nárůstu toxicity léčby.
Until recently, chemotherapy has been widely used in the palliative treatment of cholangiocellular carcinoma, especially cisplatin + gemcitabine and FOLFOX regimens. Lately, there has been a significant expansion of treatment options for cholangiocellular carcinoma with specific mutations – in tumours with the FGFR gene fusion, efficacy of the FGFR1–3 tyrosine kinase inhibitor pemigatinib has been shown, in tumours with activating IDH1 mutations, ivosidenib has been beneficial, and in HER2-positive tumours, the efficacy of anti-HER2 therapy (pertuzumab + trastuzumab, zanidatamab) has been observed. It also seems, that a proportion of patients with advanced cholangiocellular adenocarcinoma could be successfully treated with checkpoint inhibitors (CPIs): in this context, the TOPAZ-1 trial provided robust data, in which the addition of the CPI durvalumab to cisplatin + gemcitabine chemotherapy led to a significant increase in progressionfree survival as well as overall survival of the patients, without a marked increase in treatment toxicity.

PURPOSE: The present systematic review and network meta-analysis (NMA) compared the current different neoadjuvant chemotherapy (NAC) regimes for bladder cancer patients to rank them. METHODS: We used the Bayesian approach in NMA of six different therapy regimens cisplatin, cisplatin/doxorubicin, (gemcitabine/cisplatin) GC, cisplatin/methotrexate, methotrexate, cisplatin, and vinblastine (MCV) and (MVAC) compared to locoregional treatment. RESULTS: Fifteen studies comprised 4276 patients who met the eligibility criteria. Six different regimes were not significantly associated with a lower likelihood of overall mortality rate compared to local treatment alone. In progression-free survival (PFS) rates, cisplatin, GC, cisplatin/methotrexate, MCV and MVAC were not significantly associated with a higher likelihood of PFS rate compared to locoregional treatment alone. In local control outcome, MCV, MVAC, GC and cisplatin/methotrexate were not significantly associated with a higher likelihood of local control rate versus locoregional treatment alone. Nevertheless, based on the analyses of the treatment ranking according to SUCRA, it was highly likely that MVAC with high certainty of results appeared as the most effective approach in terms of mortality, PFS and local control rates. GC and cisplatin/doxorubicin with low certainty of results was found to be the best second options. CONCLUSION: No significant differences were observed in mortality, progression-free survival and local control rates before and after adjusting the type of definitive treatment in any of the six study arms. However, MVAC was found to be the most effective regimen with high certainty, while cisplatin alone and cisplatin/methotrexate should not be recommended as a neoadjuvant chemotherapy regime.

BACKGROUND: The advent of immune-checkpoint inhibitors has challenged previous treatment paradigms for advanced urothelial carcinoma (UC) in the post-platinum setting as well as in the first-line setting for cisplatin-ineligible patients. In this study, we investigated the effectiveness of pembrolizumab as first-line treatment for cisplatin-ineligible UC. METHODS: Data from patients aged ≥ 18 years with cisplatin-ineligible UC and receiving first-line pembrolizumab from January 1st 2017 to September 1st 2022 were collected. Cisplatin ineligibility was defined according to the Galsky criteria. Thirty-three Institutions from 18 countries were involved in the ARON-2 study. RESULTS: Our analysis included 162 patients. The median follow-up time was 18.9 months (95%CI 15.3-76.9). In the overall study population, the median OS was 15.8 months (95%CI 11.3-32.4). The median OS was significantly longer in males versus females while no statistically significant differences were observed between patients aged < 65y versus ≥ 65y and between smokers and non-smokers. According to Recist 1.1 criteria, 26 patients (16%) experienced CR, 32 (20%) PR, 39 (24%) SD and 55 (34%) PD. CONCLUSIONS: Our data confirm the role of pembrolizumab as first-line therapy for cisplatin-unfit patients. Further studies investigating the biological and immunological characteristics of UC patients are warranted in order to optimize the outcome of patients receiving immunotherapy in this setting.

Uroteliální karcinom je nejčastějším histologickým typem zhoubných nádorů močového měchýře. V léčbě neinvazivního uroteliálního karcinomu je standardem provedení transuretrální resekce s případnou instalací chemoterapie či vakcíny Bacillus Calmette-Guérin (BCG). Základním kurativním výkonem u nemetastazujícího invazivního uroteliálního karcinomu je provedení radikální cystektomie, které by měla předcházet neoadjuvantní chemoterapie s cisplatinou. U pacientů, kde radikální cystektomie možná není, je indikován záchovný protokol. Základem systémové léčby lokálně pokročilého či metastazujícího uroteliálního karcinomu je chemoterapie založená na cisplatině. Režimy bez podání cisplatiny mají horší léčebné výsledky. V posledních letech dochází k dalšímu rozvoji systémové terapie. Uroteliální karcinom močového měchýře patří mezi nádory ovlivnitelné imunoterapií. Největší přínos prokázaly protilátky proti kontrolním bodům imunitní reakce. V paliaci lze tuto léčbu indikovat jednak u pacientů po předchozím podání chemoterapie založené na platině, jednak v první linii u nemocných neschopných podstoupit chemoterapii založenou na platině. Novějším postupem je koncept udržovací imunoterapie po předchozím podání chemoterapie založené na platině. Dále se do léčebné strategie dostávají i nové tyrosinkinázové inhibitory či konjugované molekuly. Problémem je nejednoznačnost prediktivních faktorů imunoterapie.
Urothelial cancer is the most common histological type of bladder cancer. The standard of the treatment of non-invasive urothelial cancer is transurethral resection with possible installation of chemotherapy or Bacillus Calmette-Guerin (BCG) vaccine. The curative therapy for non-metastatic invasive urothelial cancer is radical cystectomy combined by neoadjuvant chemotherapy based on cisplatin. In patients where radical cystectomy is not possible, a maintenance protocol is indicated. Chemotherapy based on cisplatin is the basis of systemic treatment of locally advanced or metastatic urothelial cancer. Regimens without cisplatin administration have worse treatment outcomes. In recent years, systemic therapy has been further developed. Bladder urothelial carcinoma is among the tumors that can be treated by immunotherapy. The checkpoints inhibitors of the immune response showed the greatest benefit. In the treatment of metastatic urothelial cancer checkpoints inhibitors can be indicated both in patients who have previously received platinum-based chemotherapy, and in the first line in patients unable to undergo platinum-based chemotherapy. A more recent approach is the concept of maintenance immunotherapy after prior administration of platinum-based chemotherapy. Furthermore, new tyrosine kinase inhibitors or conjugated molecules are also entering the treatment strategy. The problem is the ambiguity of the predictive factors of immunotherapy.

Chirurgická léčba představuje bohužel jedinou léčebnou modalitu, která poskytuje pacientům s nádory žlučníku / žlučových cest šanci na dlouhodobé přežívání. Standardem léčby pacientů s lokálně pokročilým či metastazujícím onemocněním je v tuto chvíli kombinace ciplatiny s gemcitabinem, když většina cílené léčby bohužel u tohoto typu nádoru nepřinesla očekávané výsledky. V dalších liniích léčby jsou obvykle indikovány kombinace s oxaliplatinou nebo taxanem. Durvalumab je první molekulou ze skupiny imunoterapie, která prokázala účinnost u nádorů žlučníku / žlučových cest a získala registraci pro tuto indikaci na základě klinické studie TOPAZ-1. Chemoterapie v kombinaci s durvalumabem splnila primární cíl studie, když došlo ke statisticky signifikantnímu prodloužení mediánu celkového přežití léčených pacientů. Durvalumab představuje novou léčebnou možnost u pacientů s nádory žlučníku / žlučových cest, kterou je vhodné zvážit v první linii paliativní léčby.
Surgery represents unfortunately only treatment option of the patients with gallbladder / biliary tract tumors, which provides the chance for long term survival. The gold standard of the treatment of the locally advanced or metastatic disease is the combination of cisplatin with gemcitabine, when targeted therapy didn't provide expected results. In further lines of the palliative treatment with chemotherapy combinations with oxaliplatin or taxane are usually indicated. Durvalumab is the first molecule from the group of immunotherapy, which showed effectivity in the treatment of the gallbladder / biliary tract cancer and gained registration in this indication based on the clinical trial TOPAZ-1. Chemotherapy with durvalumab fulfilled the primary endpoint of the trial, when median overall survival of the patients was improved with statistical significance. Durvalumab represents new treatment option for the patients with gallbladder / biliary tract cancer and should be considered in the first line of the palliative treatment.

Tento článek sumarizuje současná data týkající se použití olaparibu v terapii metastazujícího karcinomu prsu u pacientů s mutací v genech BRCA1, BRCA2: jeho mechanismus účinku, zásadní registrační studii OlympiAD, výsledky finální analýzy LUCY, jež hodnotila olaparib v klinické praxi, a také recentní data z adjuvantní studie OlympiA, na jejichž podkladě byl olaparib nově zařazen do mezinárodních léčebných postupů jako první cílený lék v terapii časného karcinomu prsu s mutací BRCA.
This article summarizes current data regarding the use of olaparib in metastatic breast cancer in patients with a BRCA1, BRCA2 mutation, its mechanism of action, data from the main registration study OlympiAD, final analysis of LUCY, which evaluated olaparib in clinical practice, and also recent results from the adjuvant study OlympiA, based on its results olaparib was included in treatment guidelines internationally as the first targeted drug in the therapy of early breast cancer with BRCA mutation.

Imunoterapie jako samostatná léčebná modalita nebo v kombinaci s chemoterapií se stala novým zlatým standardem léčby nemocných s recidivujícími či metastazujícími nádory hlavy a krku s vyčerpanými možnostmi lokoregionální terapie. V naší kazuistice prezentujeme pacienta s recidivou nádoru hlasivky po kurativní radioterapii a následně první linii paliativní léčby cisplatinou s 5-fluorouracilem s rychlou progresí onemocnění, druhou linií imunoterapie nivolumabem, po které následovala opět progrese, a teprve po třetí linii s docetaxelem došlo k dlouhodobé kompletní remisi onemocnění. Tuto skutečnost si vysvětlujeme buď opožděnou aktivací imunitního systému, nebo imunomodulačním účinkem docetaxelu, jelikož dlouhodobá kompletní remise po třetí linii paliativní léčby docetaxelem se nedá úplně očekávat.
Immunotherapy as monotherapy or in combination with chemotherapy represents the new gold standard for the treatment of the relapsed or metastatic head and neck cancer where local therapy is not possible. In our case report we present the patient with laryngeal cancer relapse after radical radiotherapy and first line palliative chemotherapy cisplatin and 5-fluorouracil with rapid disease progression followed by second line palliative treatment with immunotherapy nivolumab, again with progression and finally followed by third line palliative treatment with docetaxel with long term complete remission. This excellent result can be explained by delayed activation of the immune system or immunomudulatory effect of the docetaxel, since long term remission cannot be expected solely after third line docetaxel palliative treatment.

BACKGROUND: Testicular cancer is associated with excellent prognosis and cure is achieved in most patients with advanced cancer treated with cisplatin-based chemotherapy. However, testicular cancer survivors are at increased risk of accelerated atherosclerosis, which significantly contributes to their late morbidity and mortality. Atherosclerosis is associated with a higher proportion of fat mass and especially with increased amount of visceral fat. We explored the effects of cisplatin-based chemotherapy on body composition during and after the treatment. PATIENTS AND METHODS: We studied 30 testicular cancer patients before chemotherapy, after the second cycle of chemotherapy and three months after the end of chemotherapy. Body composition parameters were evaluated using bioelectrical impedance analysis (BIA). RESULTS: Three months after the end of chemotherapy the fat mass had increased from 22.04±7.15% to 23.92±7.33% (P=0.026) and visceral fat volume had increased by 17% from 2.36±1.75l to 2.77±1.94l (P=0.013). In the whole sample there was a decrease in muscle mass after the second cycle of chemotherapy (-1.33 ± 2 kg on average; P=0.005). The changes in body composition varied according to distinct baseline fat mass. CONCLUSION: Cisplatin-based chemotherapy was associated with increase of fat mass, visceral fat, and body mass index. We also observed decrease in muscle mass and total body water. Our results suggest that BIA could help to target preventative measures to avert the acceleration of atherosclerosis in patients treated with cisplatin-based chemotherapy.
