Ovarian cancer (OC) is one of the leading causes of cancer-related deaths in women. Most patients are diagnosed with advanced epithelial OC in their late 60s, and early-onset adult OC diagnosed ≤30 years is rare, accounting for less than 5% of all OC cases. The most significant risk factor for OC development are germline pathogenic/likely pathogenic variants (GPVs) in OC predisposition genes (including BRCA1, BRCA2, BRIP1, RAD51C, RAD51D, Lynch syndrome genes, or BRIP1), which contribute to the development of over 20% of all OC cases. GPVs in BRCA1/BRCA2 are the most prevalent. The presence of a GPV directs tailored cancer risk-reducing strategies for OC patients and their relatives. Identification of OC patients with GPVs can also have therapeutic consequences. Despite the general assumption that early cancer onset indicates higher involvement of hereditary cancer predisposition, the presence of GPVs in early-onset OC is rare (<10% of patients), and their heritability is uncertain. This review summarizes the current knowledge on the genetic predisposition to early-onset OC, with a special focus on epithelial OC, and suggests other alternative genetic factors (digenic, oligogenic, polygenic heritability, genetic mosaicism, imprinting, etc.) that may influence the development of early-onset OC in adult women lacking GPVs in known OC predisposition genes.

PURPOSE: Germline pathogenic variants in CHEK2 confer moderately elevated breast cancer risk (odds ratio, OR ∼ 2.5), qualifying carriers for enhanced breast cancer screening. Besides pathogenic variants, dozens of missense CHEK2 variants of uncertain significance (VUS) have been identified, hampering the clinical utility of germline genetic testing (GGT). EXPERIMENTAL DESIGN: We collected 460 CHEK2 missense VUS identified by the ENIGMA consortium in 15 countries. Their functional characterization was performed using CHEK2-complementation assays quantifying KAP1 phosphorylation and CHK2 autophosphorylation in human RPE1-CHEK2-knockout cells. Concordant results in both functional assays were used to categorize CHEK2 VUS from 12 ENIGMA case-control datasets, including 73,048 female patients with breast cancer and 88,658 ethnicity-matched controls. RESULTS: A total of 430/460 VUS were successfully analyzed, of which 340 (79.1%) were concordant in both functional assays and categorized as functionally impaired (N = 102), functionally intermediate (N = 12), or functionally wild-type (WT)-like (N = 226). We then examined their association with breast cancer risk in the case-control analysis. The OR and 95% CI (confidence intervals) for carriers of functionally impaired, intermediate, and WT-like variants were 2.83 (95% CI, 2.35-3.41), 1.57 (95% CI, 1.41-1.75), and 1.19 (95% CI, 1.08-1.31), respectively. The meta-analysis of population-specific datasets showed similar results. CONCLUSIONS: We determined the functional consequences for the majority of CHEK2 missense VUS found in patients with breast cancer (3,660/4,436; 82.5%). Carriers of functionally impaired missense variants accounted for 0.5% of patients with breast cancer and were associated with a moderate risk similar to that of truncating CHEK2 variants. In contrast, 2.2% of all patients with breast cancer carried functionally wild-type/intermediate missense variants with no clinically relevant breast cancer risk in heterozygous carriers.

Východiska: Karcinom prsu je komplexní, multifaktoriální onemocnění, na jehož vzniku se podílejí i genetické faktory. Kromě relativně vzácných patogenních variant ve vysoce či středně penetrantních nádorových predispozičních genech ovlivňují riziko vzniku karcinomu prsu i početné nízce penetrantní alely, které vnímáme jako faktory polygenní dědičnosti. Nízce penetrantní alely mají samostatně zanedbatelný význam, avšak jejich kumulativní účinek může dosáhnout klinicky významných hodnot a lze jej vyjádřit pomocí skóre polygenního rizika (polygenic risk score – PRS), které se v současnosti zvažuje jako možný nástroj pro přesnější odhad absolutního a kumulativního rizika onkologického onemocnění u konkrétní osoby. Cíl: V zahraničí již bylo vyvinuto několik setů pro jednonukleotidový polymorfizmus (single nucleotide polymorphism – SNP) pro stanovení PRS za účelem využití metody v individualizaci rizika v klinické praxi. Následující text si klade za cíl osvětlit základy stanovení PRS a jeho interpretaci jako možného predikčního nástroje. Využití PRS by však mělo být vždy podmíněno genetickým vyšetřením patogenních variant v nádorových predispozičních genech.
Background: Breast cancer is a complex, multifactorial disease influenced by many genetic factors. Besides the relatively rare pathogenic variants in high or moderate penetrant cancer predisposition genes, breast cancer risk is modified by numerous low risk alleles considered to be polygenic genetic factors. While the risks associated with individual polygenic loci are negligible, its cumulative effect can reach clinically significant values and it can be expressed as a polygenic risk score (PRS). PRS is recently considered to be a possible tool improving assessment of absolute and cumulative risks at the individual level. Purpose: Several single nucleotide polymorphism sets for PRS assessment have recently been developed and prepared for their implementation into clinical practice. The following text aims to explain the fundamental principles of the PRS assessment and its interpretation as a candidate prediction tool. The use of the PRS should always depend on genetic analysis of pathogenic variants in cancer predisposition genes including its current limitations.

BACKGROUND: The distribution of ovarian tumour characteristics differs between germline BRCA1 and BRCA2 pathogenic variant carriers and non-carriers. In this study, we assessed the utility of ovarian tumour characteristics as predictors of BRCA1 and BRCA2 variant pathogenicity, for application using the American College of Medical Genetics and the Association for Molecular Pathology (ACMG/AMP) variant classification system. METHODS: Data for 10,373 ovarian cancer cases, including carriers and non-carriers of BRCA1 or BRCA2 pathogenic variants, were collected from unpublished international cohorts and consortia and published studies. Likelihood ratios (LR) were calculated for the association of ovarian cancer histology and other characteristics, with BRCA1 and BRCA2 variant pathogenicity. Estimates were aligned to ACMG/AMP code strengths (supporting, moderate, strong). RESULTS: No histological subtype provided informative ACMG/AMP evidence in favour of BRCA1 and BRCA2 variant pathogenicity. Evidence against variant pathogenicity was estimated for the mucinous and clear cell histologies (supporting) and borderline cases (moderate). Refined associations are provided according to tumour grade, invasion and age at diagnosis. CONCLUSIONS: We provide detailed estimates for predicting BRCA1 and BRCA2 variant pathogenicity based on ovarian tumour characteristics. This evidence can be combined with other variant information under the ACMG/AMP classification system, to improve classification and carrier clinical management.

The MRE11, RAD50, and NBN genes encode for the nuclear MRN protein complex, which senses the DNA double strand breaks and initiates the DNA repair. The MRN complex also participates in the activation of ATM kinase, which coordinates DNA repair with the p53-dependent cell cycle checkpoint arrest. Carriers of homozygous germline pathogenic variants in the MRN complex genes or compound heterozygotes develop phenotypically distinct rare autosomal recessive syndromes characterized by chromosomal instability and neurological symptoms. Heterozygous germline alterations in the MRN complex genes have been associated with a poorly-specified predisposition to various cancer types. Somatic alterations in the MRN complex genes may represent valuable predictive and prognostic biomarkers in cancer patients. MRN complex genes have been targeted in several next-generation sequencing panels for cancer and neurological disorders, but interpretation of the identified alterations is challenging due to the complexity of MRN complex function in the DNA damage response. In this review, we outline the structural characteristics of the MRE11, RAD50 and NBN proteins, the assembly and functions of the MRN complex from the perspective of clinical interpretation of germline and somatic alterations in the MRE11, RAD50 and NBN genes.

Hepatocellular carcinoma (HCC) mainly stems from liver cirrhosis and its genetic predisposition is believed to be rare. However, two recent studies describe pathogenic/likely pathogenic germline variants (PV) in cancer-predisposition genes (CPG). As the risk of de novo tumors might be increased in PV carriers, especially in immunosuppressed patients after a liver transplantation, we analyzed the prevalence of germline CPG variants in HCC patients considered for liver transplantation. Using the panel NGS targeting 226 CPGs, we analyzed germline DNA from 334 Czech HCC patients and 1662 population-matched controls. We identified 48 PVs in 35 genes in 47/334 patients (14.1%). However, only 7/334 (2.1%) patients carried a PV in an established CPG (PMS2, 4×NBN, FH or RET). Only the PV carriers in two MRN complex genes (NBN and RAD50) were significantly more frequent among patients over controls. We found no differences in clinicopathological characteristics between carriers and non-carriers. Our study indicated that the genetic component of HCC is rare. The HCC diagnosis itself does not meet criteria for routine germline CPG genetic testing. However, a low proportion of PV carriers may benefit from a tailored follow-up or targeted therapy and germline testing could be considered in liver transplant recipients.

Karcinom ovaria je onemocnění s významnou dědičnou komponentou. Dědičnou mutaci v klinicky relevantních genech nacházíme v naší populaci u každé čtvrté pacientky. Proto je ke genetickému vyšetření zárodečných mutací indikována každá pacientka s tímto onemocněním (epiteliální zhoubný nádor vaječníku, vejcovodu nebo peritonea), bez ohledu na histopatologický typ tumoru ovaria či věk v době diagnózy. Genetické vyšetření má pro samotnou pacientku význam prognostický i prediktivní. U téměř každé páté nemocné (až 20 % všech pacientek s karcinomem ovaria) diagnostikujeme přítomnost patogenní zárodečné mutace v genech BRCA1 a BRCA2. Nosičky mutací v genech BRCA1 a BRCA2 zpravidla lépe odpovídají na léčbu platinovými deriváty, a tato léčba může být uplatněna i ve vyšších liniích. Genetické vyšetření zárodečných mutací navíc umožňuje cílené genetické poradenství v rodině pacientky, identifikaci osob v riziku a odpovídající dispenzarizaci nosičů mutace s cílem prevence / časné diagnostiky souvisejících nádorových onemocnění. Zárodečná, ale i somatická mutace genů BRCA1 a BRCA2 může také hrát roli v rozvaze o indikaci konkrétního inhibitoru poly-ADP-ribózapolymerázy (PARPi). Somatické mutace v BRCA1 a BRCA2 nacházíme u dalších 5-9 % žen s touto diagnózou. Souběžné vyšetření zárodečných a somatických mutací umožní zefektivnit identifikaci pacientek, které jsou potenciálními kandidátkami pro jeden z typů udržovací léčby PARPi, a zároveň odhalit zdravé příbuzné ve vysokém riziku vzniku především karcinomu prsu a ovaria.
Ovarian cancer is a disease with a significant hereditary component. Hereditary mutations in clinically relevant genes (including BRCA1 and BRCA2) are found in one in four patients in our population. Therefore, genetic testing for germline mutations is indicated for every patient with this disease (epithelial ovarian, fallopian tube or peritoneal cancer), regardless of the histopathological type of ovarian tumour or age at diagnosis. Nearly one in five patients (up to 20% of all patients with ovarian cancer) carries a pathogenic germline mutation in the BRCA1 and BRCA2 genes. Genetic testing has prognostic and predictive value for the patient herself. Carriers of mutations in the BRCA1 and BRCA2 genes generally respond better to treatment with platinum derivatives, and this treatment can be applied in higher lineages. In addition, genetic testing of germline mutations allows targeted genetic counselling in the patient`s family, identification of individuals at risk and appropriate surveillance management of the patient and her positively tested healthy relatives. Germline as well as somatic mutations in BRCA1 and BRCA2 genes may also play a role in the consideration of the indication of a specific poly-ADP-ribose polymerase inhibitor (PARPi). Somatic mutations in BRCA1 and BRCA2 are found in an additional 5-9% of women with ovarian cancer. Simultaneous testing for germline and somatic mutations will allow for more efficient identification of patients who are potential candidates for one type of PARPi maintenance therapy, while also detecting healthy relatives at high risk of developing breast and ovarian cancer in particular.

Karcinom ovaria patří k nejčastějším gynekologickým nádorovým onemocněním, mezi nimiž dlouhodobě zaujímá první místo v mortalitě. Genetická predispozice pro vznik karcinomu ovaria je neobvykle vysoká, v ČR dosahuje 30 % všech případů. Vysoká prevalence dědičných mutací v klinicky významných genech odůvodňuje dle současných doporučení testování přítomnosti zárodečných mutací u všech pacientek s karcinomem ovaria. Hlavními predispozičními geny dědičné formy karcinomu ovaria v ČR jsou BRCA1 a BRCA2, nicméně s vysokým rizikem vzniku tohoto onemocnění jsou spojeny zárodečné mutace i v dalších predispozičních genech vč. RAD51C a RAD51D. Mutace v každém z těchto dvou genů se u nás vyskytují u 1 % pacientek s karcinomem ovaria. Identifikace zárodečných mutací v genech RAD51C a RAD51D má kromě významu preventivního potenciálně také význam prognostický. Cílem této přehledové práce je shrnout dosavadní poznatky týkající se klinického významu RAD51C a RAD51D, rizika vzniku karcinomu ovaria a dalších nádorových onemocnění a klinických doporučení pro nosičky mutací.
Ovarian cancer is one of the most common gynecologic cancers with the highest mortality rate over a long period. Genetic predisposition to ovarian cancer is unusually high. In the Czech Republic, causal mutation in any ovarian cancer predisposition gene is identified in approximately 30% of the ovarian cancer patients. Therefore, according to the current guidelines, all ovarian cancer patients should be provided with genetic testing. The BRCA1 and BRCA2 are the two major ovarian cancer predisposition genes. Nevertheless, mutations in other predisposition genes, including RAD51C and RAD51D, are associated with high ovarian cancer risk. Mutations in RAD51C and RAD51D are found in 1% of ovarian cancer patients in each respective gene. Currently, identification of germline mutation in RAD51C and RAD51D is primarily of preventive importance but it potentially could make a prognostic difference. The aim of this review is to summarize the recent RAD51C and RAD51D knowledge, including the biological function, cancer risks associated with germline mutations, and recommendations for mutation carriers.
