BACKGROUND: Achondroplasia (ACH) is one of the most prevalent genetic forms of short-limbed skeletal dysplasia, caused by gain-of-function mutations in the receptor tyrosine kinase FGFR3. In August 2021, the C-type natriuretic peptide (CNP) analog vosoritide was approved for the treatment of ACH. A total of six other inhibitors of FGFR3 signaling are currently undergoing clinical evaluation for ACH. This progress creates an opportunity for children with ACH, who may gain early access to the treatment by entering clinical trials before the closure of their epiphyseal growth plates and cessation of growth. Pathophysiology associated with the ACH, however, demands a long observational period before admission to the interventional trial. Public patient registries can facilitate the process by identification of patients suitable for treatment and collecting the data necessary for the trial entry. RESULTS: In 2015, we established the prospective ACH registry in the Czechia and the Slovak Republic ( http://www.achondroplasia-registry.cz ). Patient data is collected through pediatric practitioners and other relevant specialists. After informed consent is given, the data is entered to the online TrialDB system and stored in the Oracle 9i database. The initial cohort included 51 ACH children (average age 8.5 years, range 3 months to 14 years). The frequency of selected neurological, orthopedic, or ORL diagnoses is also recorded. In 2015-2021, a total of 89 measurements of heights, weights, and other parameters were collected. The individual average growth rate was calculated and showed values without exception in the lower decile for the appropriate age. Evidence of paternal age effect was found, with 58.7% of ACH fathers older than the general average paternal age and 43.5% of fathers older by two or more years. One ACH patient had orthopedic limb extension and one patient received growth hormone therapy. Low blood pressure or renal impairment were not found in any patient. CONCLUSION: The registry collected the clinical information of 51 pediatric ACH patients during its 6 years of existence, corresponding to ~ 60% of ACH patients living in the Czechia and Slovak Republic. The registry continues to collect ACH patient data with annual frequency to monitor the growth and other parameters in preparation for future therapy.

Východiska: Gastrointestinální stromální tumory (GIST) jsou vzácné maligní mezenchymální tumory s incidencí 1/100 000 obyvatel. Představují pouze 5 % gastrointestinálních nádorů. Nejčastěji je nalézáme v žaludku (60-70 %). Do rekta je jich situovaných < 5 %. V případě lokalizovaných, resekabilních tumorů je základem léčby chirurgická resekce. V závislosti na velikosti a lokalizaci tumoru v rektu volíme mezi lokální excizí, resekcí rekta s anastomózou nebo abdominoperitoneální amputací s trvalou stomií. Na rozdíl od karcinomů je metastazování GIST do lymfatických uzlin vzácné, a proto není z onkologického hlediska lymfadenektomie v podobě mezorektální excize vyžadována. Neoadjuvantní léčba s využitím inhibitorů tyrozinkináz (tyrosine kinase inhibitors -TKI) je doporučovaná např. u tumorů větších 5 cm, v případě prorůstaní do okolních orgánů nebo při infiltraci sfinkterů za účelem dosažení resekability a méně mutilujícího či kontinentního zákroku. V případě pozitivní resekční linie se můžeme pokusit o reresekci. Při CD117 pozitivitě lze zvážit adjuvantní léčbu TKI, kterou nasazujeme také po resekcích GIST středního a vysokého rizika maligního chování. U neresekabilních a metastazujících GIST je taktéž indikovaná léčba TKI. Materiál a metody: Analýza dat získaných z registru GIST k 1. 1. 2017, kdy do registru přispívalo 10 center z ČR. Výsledky: Z analyzovaných 1 095 pacientů byla primární lokalizace GIST v rektu ve 45 případech (4,1 %). Průměrný věk pacientů byl 60 let. V souboru bylo signifikantně více mužů (68,9 %; p = 0,0007) a symptomatických pacientů (62,2 %; p = 0,034). Operaci podstoupilo 82 % pacientů. Z toho lokální excize byla provedena ve 37,8 %, resekce rekta s anastomózou ve 29,7 % a abdominoperineální amputace ve 29,7 %. V souboru bylo nejvíce tumorů velikosti 2-5 cm a necelá polovina vykazovala vysoké riziko maligního chování. Záznam o podání systémové léčby byl u 73 % pacientů. Kompletní remise byla dosažena u 80 % pacientů s GIST rekta. Medián přežití byl 11,3 roku a 5leté přežití je 90,6 %. Závěr: Navzdory úspěchům léčby TKI zůstává jedinou potenciálně kurativní metodou u GIST rekta chirurgická R0 resekce. Nejen vzhledem k nízké frekvenci výskytu těchto nádorů je správná diagnostika a léčba náročná. Proto by měli být pacienti s touto problematikou soustředěni do specializovaných center.
Background: Gastrointestinal stromal tumours (GIST) are rare malignant mesenchymal tumours with an incidence of 1 in 100,000. They represent only 5% of gastrointestinal tumours. The GISTs are mainly located in the stomach (60-70%) and in the rectum in < 5% of cases. In the case of localized, resectable tumours, the treatment is surgical resection. Depending on the size and localization of the tumour in the rectum, either a local excision, rectal resection with anastomosis, or abdominoperitoneal amputation with permanent stoma can be performed. In contrast to carcinomas, the metastasis of GISTs into lymph nodes is rare; therefore, from an oncological point of view, lymphadenectomy in the form of mesorectal excision is not required. Neoadjuvant treatment using tyrosine-kinase inhibitors (TKI) is recommended for tumours larger than 5 cm and in case of tumours infiltrating surrounding organs or sphincters in order to achieve complete resectability, less mutilating and continent procedure. In GISTs with a positive resection line, re-resection can be attempted. Adjuvant TKI therapy can be considered in cases of CD117 positivity and after resections of GISTs with medium and high-risk malignant behaviour. The TKI treatment is also indicated in cases of unresectable and metastatic GISTs. Methods: Data obtained from the GIST registry by the 1st January 2017, when 10 centres in the Czech Republic were contributing to the registry, were analysed. Results: We analysed 1,095 patients out of which 45 (4.1%) had GIST localized in the rectum. The average age of the patients was 60 years. There were significantly more males (68.9%; p = 0.0007) and symptomatic patients (62.2%; p = 0.034). In total, 82% of the patients underwent surgery. Local excision was performed in 37.8%, resection of the rectum with anastomosis in 29.7%, and Miles operation in 29.7%. In the cohort, most tumours were 2-5 cm in size and almost half of the tumours presented a high risk of malignant behaviour. Systemic treatment was reported in 73% of patients. A complete remission was achieved in 80% of patients with GIST of the rectum. The median survival rate was 11.3 years and the 5-year survival rate is 90.6%. Conclusion: Despite the success of TKI treatment, the only potentially curative method of rectal GISTs is a surgical R0 resection. Given the relatively rare frequency of these tumours, proper diagnosis and treatment is demanding. Therefore, these patients should be preferably treated in specialised centres.
INTRODUCTION: Pirfenidone, an antifibrotic drug, slows-down the disease progression in idiopathic pulmonary fibrosis (IPF) over 12 months, however limited data on the decline of lung function and overall survival (OS) in real-world cohorts on longer follow-up exists. PATIENTS/METHODS: Of the enrolled Czech IPF patients (n = 841) from an EMPIRE registry, 383 (45.5%) received pirfenidone, 218 (25.9%) no-antifibrotic treatment and 240 (28.5%) were excluded (missing data, nintedanib treatment). The 2- and 5-yrs OS and forced vital capacity (FVC) and diffusing lung capacity for carbon monoxide (DLCO) were investigated at treatment initiation and 6, 12, 18 and 24 months' follow-up. RESULTS: During a 2-yr follow-up, less than a quarter of the patients progressed on pirfenidone as assessed by the decline of ≥10% FVC (17.0%) and ≥ 15% DLCO (14.3%). On pirfenidone, the DLCO (≥10%) declines at 6, 12, 18 and 24 months' and DLCO (≥15%) declines at 6, 18 and 24 months' follow-up were associated with increased mortality. The DLCO decline showed higher predictive value for mortality than FVC decline. In patients with no-antifibrotics, FVC and DLCO declines were not predictive for mortality. Pirfenidone increased 5-yrs OS over no-antifibrotic treatment (55.9% vs 31.5% alive, P = 0.002). CONCLUSION: Our study observed the 2-yrs sustained effect of pirfenidone on the decline of lung function and survival in the real-world patient's IPF cohort. DLCO decline of ≥10% shows a potential as a mortality predictor in IPF patients on pirfenidone, and should be routinely evaluated during follow-up examinations.

INTRODUCTION: Prognostic factors of idiopathic pulmonary fibrosis (IPF) currently recognized include changes in vital capacity and radiologic findings. However, most of the prognostic studies in IPF are based on clinical studies with preselected IPF populations. Therefore, we decided to analyze the factors influencing IPF prognosis based on the real-practice data from our IPF registry. METHODS: Data of 514 subjects consecutively entered since 2012 into Czech EMPIRE IPF registry were analyzed. RESULTS: Median age of our patient cohort was 67 years (50-82). Median overall survival (OS) of the cohort was 63.1 months. The clinical course of IPF according to FVC (forced vital capacity) changes was stabilized in 32.8% of patients (29.7% according to DLCO [diffuse lung capacity] changes), slowly progressive in 39.5% (45%), rapidly progressive in 23.5% (20.7%); and 1.7% patients had at least one acute exacerbation during follow-up. Deterioration in FVC of ≥10% at month 12 and in DLCO of ≥15% at months 12, 18, and 24 influenced the OS significantly. The fast progressors defined by the DLCO decline rate had higher risk of death compared to those defined by the FVC change over time. In multivariate analysis, age ≥70 years, interstitial HRCT scores ≥3, and change in DLCO of ≥15% at month 12 were confirmed as factors negatively influencing OS. CONCLUSIONS: DLCO changes over time were shown as a better predictor of mortality compared with FVC changes in our study. In our opinion it is necessary to implement the DLCO analysis into clinical trials and routine practice.

BACKGROUND: Publication of comprehensive clinical care guidelines for Duchenne muscular dystrophy (DMD) in 2010 was a milestone for DMD patient management. Our CARE-NMD survey investigates the neuromuscular, medical, and psychosocial care of DMD patients in Europe, and compares it to the guidelines. METHODS: A cross-sectional survey of 1677 patients contacted via the TREAT-NMD patient registries was conducted using self-report questionnaires in seven European countries. RESULTS: Survey respondents were 861 children and 201 adults. Data describe a European DMD population with mean age of 13.0 years (range 0.8-46.2) of whom 53% had lost ambulation (at 10.3 years of age, median). Corticosteroid medication raised the median age for ambulatory loss from 10.1 years in patients never medicated to 11.4 years in patients who received steroids (p < 0.0001). The majority of patients reported receiving care in line with guidelines, although we identified significant differences between countries and important shortcomings in prevention and treatment. Summarised, 35% of patients aged≥ nine years received no corticosteroid medication, 24% of all patients received no regular physiotherapy, echocardiograms were not performed regularly in 22% of patients, pulmonary function was not regularly assessed in 71% of non-ambulatory patients. Patients with regular follow-up by neuromuscular specialists were more likely to receive care according to guidelines, were better satisfied, and experienced shorter unplanned hospitalization periods.

Cílem našeho článku je zhodnotit výsledky léčby gliálních nádorů WHO stupně III v České republice. Hodnocení bylo provedeno na vzorku pacientů získaném z registru gliálních nádorů DoIT. Ke statistickému zpracování byly k dispozici údaje od 205 pacientů s gliomy WHO stupně III diagnostikovaných v období 1. 1. 2007–31. 12. 2012 v 10 centrech. Analýzou nebyl prokázán prognostický vliv pohlaví. Naopak byl prokázán pozitivní efekt věku pacienta, oligodendrogliální komponenty nádoru (medián přežití – 45,3 měsíce), velikosti resekce (medián přežití – 25,4 měsíce), chemoradioterapie nad pouhou radioterapií (medián přežití – 33,1 vs. 11,6 měsíce) a nakonec pozitivní synergický efektu kombinace maximální resekce a chemoradioterapie na celkové přežití (medián přežití – 36 měsíců). Ve všech centrech byl temozolomid využíván jako chemoterapeutikum první volby; PCV byla v první linii chemoterapie anaplastických oligo‑gliomů použita jen jednou. Ze závěru také vyplynulo, že předpokladem pro zavedení „individualizované terapie“ gliomů WHO stupně II je implantace cytogenetického vyšetření prognostických markerů (MGMT, 1p/19q, IDH, ATRX) do standardní laboratorní diagnostiky. Gliomy WHO stupně III jsou velmi heterogenní skupina nádorů s rozdílnou prognózou a odpovědí na léčbu. K pochopení problematiky je potřebný komplexní pohled, s pochopením spojitostí mezi chirurgickou, onkologickou léčbou a terapií vedlejších projevů. Prakticky všechna komplexní onkologická centra si vytvořila své interdisciplinární neuroonkologické týmy, jejichž členy jsou i neurologové a neurochirurgové, kteří se takto podílejí na stanovování celkové strategie léčby gliomů WHO stupně III.
The aim of our paper is to evaluate treatment results for WHO grade III gliomas in the Czech Republic. We performed an assessment of a sample of patients from the glioma tumor DoIT registry. Data on 226 patients diagnosed with WHO grade III gliomas between 1 January 2007 and 31 December 2012 in ten centers were available for statistical analysis. We failed to prove an effect of gender on overall survival. However, a positive effect of patient age, oligodendroglial tumor component (median survival – 45.3 months), extent of resection (median survival – 25.4 months), chemoradiotherapy vs. radiotherapy alone (median survival – 33.1 vs. 11.6 months) was shown as well as a synergic effect of total tumor resection plus chemoradiotherapy on overall survival (median survival – 36 months). Temozolomide was the first‑choice chemotherapy regimen in all centers; PCV chemotherapy was initially administered in one case of oligo‑glioma only. The analysis also implies that implementation of cytogenetic investigation of prognostic markers (MGMT, 1p/19q, IDH, ATRX) into standard laboratory procedures is required if individualized therapy of WHO grade III glioma is to take place. WHO grade III gliomas are a very heterogeneous group of tumors with different prognosis and treatment response. Virtually all comprehensive cancer centers have appointed neuro‑oncologic teams including neurologists and neurosurgeons who thus participate in establishing the general management strategy for WHO grade III gliomas.
