Renální epiteloidní angiomyolipom (ReAML) je vzácným podtypem angiomyolipomu. Jedná se o benigní onemocnění ledvin, u kterého byly hlášeny případy agresivního chování a malignity. Tyto tumory představují 8 % operovaných angiomyolipomů ledviny. Diagnostika se opírá o histologický nález. Cílem práce je podat přehled informací o této problematice, histologickém obrazu a kazuistice 42leté ženy operované na našem pracovišti, u které bylo prokázáno multicentrické či metastatické postižení lymfatické uzliny retroperitonea.
Renal epithelioid angiomyolipoma (ReAML) is a rare subtype of angiomyolipoma. It is a benign kidney disease, in which cases of aggressive behavior and malignancy have been reported. These tumors represent 8% of operated kidney angiomyolipomas. Diagnosis is based on histological findings. The aim of this text is to review the information on this issue, the histological picture and the case report of a 42-year-old woman operated at our department who was found to have multicentric or metastatic involvement of the retroperitoneal lymph node.

Angiomyolipomas (AMLs) are mesenchymal tumours derived from perivascular epithelioid cells. Although AMLs are generally known as benign and extremely rare epithelioid variants of AML, they may be potentially aggressive. Here we present an adrenal epithelioid AML and the literature review. A 64-year-old female patient was diagnosed with a left adrenal mass detected incidentally on ultrasonography. Preoperative abdominal CT (computed tomography) showed a 95×68 mm heterogeneous contrast enhancement mass lesion in the left adrenal gland. The lesion was hormone inactive in the endocrinological evaluation, and left laparoscopic adrenalectomy was performed. The patient was discharged on the 2nd postoperative day. Pathology was reported as epithelioid subtype AML. The patient has no local recurrence or metastasis in the 18-month follow-up period and imaging. Adrenal epithelioid AML is an extremely rare and potentially aggressive variant. According to the literature, open or laparoscopic adrenalectomy seems to be suitable option for disease management.

PEComy (perivaskulární epiteloidní tumory) jsou poměrně vzácné mezenchymové nádory z perivaskulárních epiteloidních buněk. Jedná se převážně o nádory s benigní biologickou povahou, mohou však mít i maligní potenciál. Vyskytují se v jakékoli anatomické lokalizaci a postihují 4krát častěji ženy většinou mladšího a středního věku. Až 20 % případů je náhodným nálezem na zobrazovacích vyšetřeních. Diagnózu PEComu potvrzuje histologický nález epiteloidních buněk s imunohistochemickým průkazem exprese myoidních a melanocytárních markerů. Jediným kauzálním řešením je chirurgická léčba. Na svízelnost diagnostiky PEComů poukazuje kazuistika pacientky s maligním angiomyolipomem jater, která podstoupila transplantaci jater pro biopticky verifikovanou diagnózu hepatocelulárního karcinomu.
PEComas (perivascular epithelioid cell tumors) are rare mesenchymal tumours composed of epithelial perivascular cells. Most of these tumours have indolent behaviour, but occasionally they can have malignant potential. PEComas occur at any anatomical location and are 4 times more frequent in younger and middle-aged women. The diagnosis of PEComas is established incidentally in about 20% patients as a result of abdominal imaging. A typical microscopic feature of PEComas is the presence of perivascular epithelioid cells co-expressing both muscle and melanocytic markers. The only causal therapy is surgery. Our case report points at The diagnostic challenge of PEComas in, it describes a patient with malignant hepatic angiomyolipoma, who underwent liver transplantation for a bioptically proven diagnosis of hepatocellular carcinoma.
Cílem mé práce je prezentace kazuistiky pacientky s již dříve diagnostikovaným angiomyolipomem levé ledviny, která přichází na urologickou pohotovost pro akutně vzniklou bolest v oblasti levé ledviny při práci s teleskopickou pilou. Zjištěna ruptura tumoru s retroperitoneálním krvácením, jež řešena operační revizí a nefrektomií.
The goal of my work is to present a case report of the pacient firstly diagnosed with angiomyolipoma of the left kidney in past, who is now arriving to urological emergency due to acute made-pain in the left kidney, while was working with a telescopic saw. We found the ruptura of tumour with retroperitoneal bleeding has been already solved by operated revision and nephrectomy.

Tento přehledový článek stručně shrnuje možnosti využití imunohistochemie při vyšetřování především renálních karcinomů a základní molekulárně genetické znaky vybraných neoplázií. Článek však v žádném případě nelze brát jako univerzální návod pro diagnostiku renálních tumorů. Renální karcinomy dokážou mít velmi variabilní morfologický vzhled a to i v rámci jedné léze (nádorová heterogenita) a často velmi nepředvídatelný a neuniformní imunohistochemický profil. Některé renální neoplázie jsou diagnostikovány striktně na podkladě molekulárně-genetických vlastností, bez ohledu na morfologický vzhled.
Pro správnou léčbu renálních nádorů je nutná co nejpřesnější diagnóza. Stále nejpřesnější je histologické vyšetření, ale v poslední době se zlepšuje i diagnostika pomocí zobrazovacích metod, jak v určení histologického typu, tak při zjišťování malých nádorů ledvin. Z celkového počtu nádorů ledvin připadá 20 % na benigní nádory, zbytek tvoří maligní tumory, které vyžadují agresivnější způsob léčby. Nejdůležitější je rozlišení benigních a maligních lézí, což se pomocí zobrazovacích technik daří s poměrně vysokou přesností. Pokračuje i snaha o rozlišení jednotlivých typů maligních nádorů; výsledky zatím nejsou plně uspokojivé, ale jednotlivé zobrazovací metody se stále zdokonalují.
For the correct treatment of renal tumors, the most accurate diagnosis is necessary, histological examination is still the most accurate, but recently the diagnosis has been improved by imaging methods both in determining the histological type and in the detection of small kidney tumors. Of the total number of kidney tumors, 20% are benign tumors, the rest are malignant tumors that require a more aggressive treatment. The most important is to distinguish between benign and malignant lesions, which is achieved by using imaging techniques with relatively high accuracy. The effort continues to distinguish between individual types of malignant tumors, where the results are not yet fully satisfactory, but individual imaging methods are being improved.

Prezentujeme kazuistiku objemného recidivujícího angiomyolipomu podkovovité ledviny u mladé pacientky s podezřením na syndrom tuberózní sklerózy. Stav byl řešen opakovanou cytoredukční operací v kombinaci se systémovou léčbou mTOR inhibitory.
We present a case report of gigantic recurrent angiomyolipoma of the horseshoe kidney in a young patient with suspected tuberous sclerosis syndrome. The condition was solved by repeated cytoreductive surgery in combination of systemic treatment with mTOR inhibitors.

CONTEXT: Little is known about the natural history of sporadic angiomyolipomas (AMLs); there is uncertainty regarding the indications of treatment and treatment options. OBJECTIVE: To evaluate the indications, effectiveness, harms, and follow-up of different management modalities for sporadic AML to provide guidance for clinical practice. EVIDENCE ACQUISITION: A systematic review of the literature was undertaken, incorporating Medline, Embase, and the Cochrane Library (from 1 January 1990 to 30 June 2017), in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. No restriction on study design was imposed. Patients with sporadic AML were included. The main interventions included active surveillance, surgery (nephron-sparing surgery and radical nephrectomy), selective arterial embolisation, and percutaneous or laparoscopic thermal ablations (radiofrequency, microwaves, or cryoablation). The outcomes included indications for active treatment, AML growth rate, AML recurrence rate, risk of bleeding, post-treatment renal function, adverse events of treatments, and modalities of follow-up. Risk of bias assessment was performed using standard Cochrane methods. EVIDENCE SYNTHESIS: Among 2704 articles identified, 43 were eligible for inclusion (zero randomised controlled trials, nine nonrandomised comparative retrospective studies, and 34 single-arm case series). Most studies were retrospective and uncontrolled, and had a moderate to high risk of bias. CONCLUSIONS: In active surveillance series, spontaneous bleeding was reported in 2% of patients and active treatment was undertaken in 5%. Active surveillance is the most chosen option in 48% of the cases, followed by surgery in 31% and selective arterial embolisation in 17% of the cases. Selective arterial embolisation appeared to reduce AML volume but required secondary treatment in 30% of the cases. Surgery (particularly nephron-sparing surgery) was the most effective treatment in terms of recurrence and need for secondary procedures. Thermal ablation was an infrequent option. The association between AML size and the risk of bleeding remained unclear; as such the traditional 4-cm cut-off should not per se trigger active treatment. In spite of the limitations and uncertainties relating to the evidence base, the findings may be used to guide and inform clinical practice, until more robust data emerge. PATIENT SUMMARY: Sporadic angiomyolipoma (AML) is a benign tumour of the kidney consisting of a mixture of blood vessels, fat, and muscle. Large tumours may have a risk of spontaneous bleeding. However, the size beyond which these tumours need to be treated remains unclear. Most small AMLs can be monitored without any active treatment. For those who need treatment, options include surgical removal of the tumour or stopping its blood supply (selective embolisation). Surgery has a lower recurrence rate and lower need for a repeat surgical procedure.

Úvod: Angiomyolipom jater je vzácný mezenchymální tumor. Skládá se z krevních cév, tukové tkáně a svalových buněk. Podíl jednotlivých složek je variabilní. Nejčastější lokalizací angiomyolipomu je ledvina. Doposud bylo popsáno jen okolo 300 případů jaterní lokalizace. Jeho biologická povaha je nejistá. Většina pacientů je asymptomatická. Pro podobnost s jinými lézemi na zobrazovacích vyšetřeních je často stanovena diagnóza angiomyolipomu až po resekci ložiska. Kazuistika: 64letá pacientka byla vyšetřovaná pro tlakové bolesti epigastria bez další symptomatiky. Pacientka byla bez známých nádorových onemocnění u blízkých příbuzných. Zobrazovací vyšetření ozřejmila lézi charakteru hepatocelulárního karcinomu jater bez korelátu v testovaných laboratorních onkomarkerech. Pacientce byla provedena levostranná lobektomie jater. Na řezu bylo patrné tuhé šedavé ložisko. Histologická a imu-nohistochemická vyšetření potvrdila diagnózu angiomyolipomu. Pooperační průběh byl bez komplikací. Pacientka je sledována jaterní poradnou. Je bez známek recidivy a bez obtíží. Závěr: Angiomyolipom jater je onemocnění vzácné. Na prezentovaném případu je patrné, že diagnostika této léze je obtížná. Přestože se ukázala být pracovní diagnóza nesprávná, léčebný postup byl volen správně a s dobrým výsledkem. Dlouhodobé sledování pacientky je indikováno.
Introduction: Hepatic angiomyolipoma is a rare mesenchymal tumor. It consists of vessels, fatty tissue and muscle tissue. These can appear in various ratios. While the kidney is the most common localization of angiomyolipoma, only about 300 cases have been described in the liver so far. It is a tumor of uncertain behavior. Most of the patients suffering from the lesion is asymptomatic. It is often preoperatively misdiagnosed using various imaging methods given its similarity to other hepatic tumors.Case report: Our 64 years old female patient was being examined for dull abdominal pains with no other symptoms. Her close relatives suffered from no malignancies. Imaging exams showed a liver lesion highly suspicious for hepatocellular carcinoma. However, the patient showed no elevation of typical oncomarkers. We performed left lateral sectionectomy. A grey solid focal lesion was found in the resected tissue. Histological and immunohistochemical evaluation determined the diagnosis of angiomyolipoma. The postoperative period was uncomplicated. The patient has been followed at an office for hepato-pancreato-biliary diseases, with no signs of recurrence until the present.Conclusion: Hepatic angiomyolipoma is a rare disease. The diagnostic process can be challenging as illustrated by the presented case. Even though the working diagnosis proved false, the chosen treatment was appropriate and delivered good results. Long-term postoperative follow-up is required.
