AIM: The objective of this study was to evaluate the efficacy of diabetic macular edema (DME) therapy using subthreshold micropulse laser (SMPL) with a wavelength of 577 nm during a long-term monitoring period of 5 years. METHODS: The cohort included the total number of 52 eyes of 34 patients with DME. All underwent the standard laser treatment for the diabetic retinopathy outside the macula and DME treatment with SMPL. Subsequent check-ups were followed every 3 months in the first year of treatment, and every 4 to 6 months in the following years. The treatment was combined neither with focal macular laser nor with anti-VEGF therapy. RESULTS: The mean central retinal thickness (CRT) was 345.9 μm SD 122.6 μm at the beginning of the monitoring. At the end of the follow-up period five years after treatment it was 256.4 μm SD 98.4 μm. The mean CRT decreased by 89.5 μm SD 153.6 μm during 5 years. At the beginning of the monitoring, before treatment with SMPL, the best corrected visual acuity (BCVA) was 70.0, SD 10.1 ETDRS letters. One year after therapy, BCVA was 72, SD 10.0 letters, two years later it was 71.4, SD 10.4 letters and decreased to 66.9, SD 12.1 letters after 5 years. The mean BCVA decreased by merely 3.1, SD 10.9 letters during 5 years. CONCLUSION: Based on our long-term observations, the DME treatment with SMPL appears to be an effective method for reducing DME and protecting BCVA against rapid worsening.

Diabetický edém makuly je hlavnou príčinou poklesu zrakovej ostrosti u pacientov s diabetom v rozvinutých krajinách. Etiopatogeneticky ide o multifaktoriálne ochorenie, v ktorom zohráva úlohu zvýšená produkcia cievneho endotelového rastového faktoru, oxidačný stres, zápal a rôzne metabolické pochody, čo v konečnom dôsledku vedie k zrúteniu hematoretinálnej bariéry, strate pericytov, narušeniu tesných spojení endotelových buniek a vzniku edému makuly. Po dobu viac ako 30 rokov bola zlatým štandardom v liečbe diabetického edému makuly laserkoagulácia. V posledných rokoch sa do prvej línii liečby diabetického edému makuly dostali antirastové faktory, ale kortikosteroidy majú dôležité postavenie u určitej skupiny pacientov. Využívajú sa pre svoj protizápalový, antiapoptotický, antiedematózny a antiangiogénny účinok. Táto liečba môže byť efektívna u pseudofakických očí refraktérnych na liečbu blokátormi cievneho endotelového rastového faktoru alebo u recidivujúcich edémov, je metódou voľby u tehotných a kojacich žien alebo u pacientov, ktorí nie sú schopní z rôznych dôvodov podstúpiť početné mesačné intravitreálne aplikácie. V súčasnosti sa používa triamcinolon acetonid, intravitreálny implantát dexametazónu a intravitreálny implantát fluocinolónu. Dlhodobo používaný triamcinolon acetonid intravitreálne je tzv. off-label liečba v tejto indikácii. Vedľajšie účinky používania intravitreálnych steroidov sú vznik a progresia katarakty a zvýšenie vnútroočného tlaku, nežiadúcim účinkom intravitreálnej liečby, našťastie zriedkavým, je endoftalmitída, odlúčenie sietnice a krvácanie. Pomaly uvoľňujúce intravitreálne implantáty dexametazónu a fluocinolónu redukujú výskyt vedľajších účinkov a znižujú počet aplikácií v manažmente pacienta s diabetickým edémom makuly.
Diabetic macular edema is the main cause of decreased vision in diabetic patients in developing countries. Pathogenesis is multifactorial, upregulation of vascular endothelial growing factor, oxidative stress, inflammation and different metabolic pathways lead to breakdown of hematoretinal barrier, loss of pericytes, endothelial cells tight junction disruption and formation of macular edema. For more than 30 years, lasercoagulation has been the gold standard in the treatment of diabetic macular edema. Although antivascular endothelial growing factors are the first – line treatment option in eyes with DME, corticosteroids have the important role as well. The main advantage of corticosteroids is their antiinflammatory, antiapoptotic, antiedematous and antiangiogenic effects. This treatment could be especially effective in pseudophakic patients poorly responsive or refractive to antiVEGF treatment, as well as method of choice in pregnant and lactating women or in patients unwilling to come for numerous monthly injections. Currently available triamcinolone acetonide has been used for many years, but is not approved for this indication yet. Intravitreal-steroid-related side effects are cataract and elevation of intraocular pressure corticosteroids are triamcinolon acetonide, dexamethasone and fluocinolone acetonide intravitreal implants, injection – related side effects are endophthalmitis, retinal detachment and bleeding. Sustained release of intravitreal implants of dexamethasone and fluocinolon reduce complications and decreased number of injections in the management of patients with diabetic macular edema.

Těhotenství je spojeno s vyšším rizikem progrese diabetických změn na očním pozadí, největší riziko je přitom ve druhém trimestru gravidity a přetrvává ještě rok po jejím ukončení. Mezi rizikové faktory progrese diabetické retinopatie (DR) během těhotenství patří délka trvání onemocnění, nedostatečná metabolická kontrola diabetu mellitu (DM), závažnost DR před otěhotněním, přítomnost dalších vaskulárních onemocnění a těhotenství samotné. Doporučení pro screening diabetických změn v těhotenství se liší, vhodné je vyšetření na počátku gravidity a poté upravovat intervaly kontrolních vyšetření dle závažnosti nálezu. Pro dosažení co nejvyšší kvality péče je velmi důležitá mezioborová spolupráce oftalmologa, gynekologa a diabetologa. Autoři prezentují kazuistiku gravidní ženy s DM 1. typu, u které došlo během těhotenství k progresi DR a diabetického makulárního edému (DME) na obou očích. Žena se léčila pro DM 24 let v intenzifikovaném inzulinovém režimu. Při očním vyšetření ve 23. týdnu druhé gravidity (první těhotenství muselo být pro diagnózu zamlklého těhotenství předčasně ukončeno) jsme diagnostikovali pokročilou proliferativní DR a pokročilý DME bilaterálně. Sítnici obou očí jsme ošetřili panretinální fotokoagulací, přesto stav progredoval, na levém oku byl komplikován hemoftalmem. Ve 28. týdnu gravidity jsme provedli pars plana vitrektomii (PPV) levého oka, DME se ovšem na obou očích dále zhoršoval, proto bylo těhotenství ve 31. týdnu plánovaně ukončeno pro riziko ztráty zrakové ostrosti matky. Zraková ostrost levého oka se zlepšila, ale po porodu se nově objevil hemoftalmus na pravém oku. Opět jsme indikovali operační řešení hemoftalmu cestou PPV s uspokojivým výsledkem. Pacientka nadále zůstává v naší péči.
Pregnancy is associated with increased risk of progression of diabetic retinopathy (DR), the greatest risk of worsening occurs during the second trimester of pregnancy and persists as long as one year after the childbirth. The risk factors include duration of the diabetes, insufficient metabolic control, severity of DR at the time of conception and presence of coexisting vascular disease, such as arterial hypertension, and pregnancy itself. The recommendations for retinopathy screening in pregnancy vary significantly. A dilated fundus exam should be done in the beginning of pregnancy, the next follow-up throughout pregnancy depends on the severity of ocular findings. The cooperation of multi-disciplinary team consisting of ophthalmologist, obstetrition and endocrinologist is essential to provide the best health care. The authors present a case report of a pregnant woman with type 1 diabetes mellitus (DM), who had a progression of DR and diabetic macular edema (DME) in both eyes during pregnancy. She has had DM for 24 years and has been treated with insulin. The patient was examined at the 23rd week of the second pregnancy (first pregnancy was terminated because of missed miscarriage). The diagnosis of advanced proliferative DR and advanced DME in both eyes was made so we performed panretinal laser photocoagulation of the retina of both eyes. Despite that the ocular findings got worse and we found vitreous haemorrhage in the left eye. We performed pars plana vitrectomy (PPV) of the left eye at the 28th week of pregnancy, nevertheless the DME got worse in both eyes, so we recommended to terminate the pregnancy at the 31st week because of the risk of loss of vision. The visual acuity of the left eye improved, but suddenly there was vitreous haemorrhage in the right eye after the delivery. We indicated PPV of the right eye, the outcome of the surgery was satisfying. We still take care about this patient.

Cílem práce je retrospektivní hodnocení dvouletých zkušeností s mikropulzní cyklofotokoagulací (MP CPC) v terapii různých typů glaukomů. Soubor pacientů a metodika: Soubor pacientů tvořilo 47 nemocných, z toho 16 mužů a 31 žen. Průměrný věk mužů byl 58,9 let (rozpětí 35-78 let), průměrný věk u žen byl 64,7 let (rozpětí 33-86 let). U tří pacientů byly ošetřeny obě oči, takže bylo hodnoceno nakonec 50 očí. Léčbu podstoupili nemocní v různých stádiích glaukomového onemocnění s různými typy primárního i sekundárního glaukomu. Nejčastěji byli zastoupeni pacienti s primárním glaukomem s otevřeným úhlem (POAG): 26 nemocných, dále pacienti se sekundárním pseudoexfoliativním glaukomem (PEXG): 9 nemocných, 4 nemocní se sekundárním pigmentovým glaukomem (PG), 3 nemocní s primárním glaukomem s uzavřeným úhlem (PACG), 2 nemocní s normotenzním glaukomem (NTG), 3 nemocní se syndromem Posner-Schlossman, po jednom pacientovi se syndromem Cogan-Reese, neovaskulárním glaukomem a sekundárním posttraumatickým glaukomem. Před operací podstoupili na operovaném oku 3 nemocní laserovou iridotomii, 4 pacienti trabekulektomii, 4 nemocní implantaci implantátu EX-PRESS®, 3 pacienti implantaci implantátu EX-PRESS® i trabekulektomii a 1 nemocný transsklerální cyklofotokoagulaci. Při operaci jsme zvolili energii 2000 až 2250 mW infračerveného laseru s vlnovou délkou 810 nm, který využívá mikropulzní systém emise laserové energie. K výkonu jsme použili novou aplikační sondu MP3. Jako úspěch terapie jsme stanovilipokles nitroočníhotlaku (NOT) o 30 % z jeho původních hodnot. Výsledky: Naše výsledky korelovali s výsledky většiny dostupných studií, kdy jsme dosáhli pokles hodnot nitroočního tlaku o minimálně 30 % u 53,4 % očí. K selhání efektu terapie došlo u 9 očí (18 %), u kterých jsme následně volili jiný terapeutický postup. Závěr:MP CPC je neincizní laserová terapie s minimem komplikací. Vyznačuje se vysokým bezpečnostním profilem a předvídatelností výsledků. V našem souboru jsme zaznamenali u 53,4 % léčených očí pokles NOT minimálně o 30 %. Využití není limitováno typem glaukomového onemocnění. V případě nedostatečného efektu je možné ji opakovat.
The objective of this study was a retrospective assessment of two-year experience with micropulse cyclophotocoagulation (MP CPC) in the therapy of various types of glaucoma.Material and method: The cohort of patients consisted of 47 people, out of which 16 men and 31 women. An average age of males was 58.9 years (the range from 35 to 78 years), and an average age of females was 64.7 years (the range from 33 to 86 years). Both eyes were treated in three patients, and therefore the total number of assessed eyes was 50. Patients who underwent the therapy were in different stages of glaucoma disease, with various types of primary and secondary glaucoma. Most of the patients were those with primary open angle glaucoma (POAG): 26 patients, followed by patients with secondary pseudoexfoliative glaucoma (PEXG): 9 patients; diagnoses of the remaining patients: 4 patients – secondary pigmentary glaucoma (PG), 3 patients - primary angle closure glaucoma (PACG), 2 patients normal tension glaucoma (NTG), 3 patients - Posner-Schlossman syndrome, 1 patient - Cogan-Reese syndrome, 1 patient - neovascular glaucoma and 1 patient – secondary traumatic glaucoma. On the operated eye prior to the surgery 3 patients underwent laser iridotomy, 4 patients trabeculectomy, 4 patients EX-PRESS® implant, 3 patients EX-PRESS® implant and trabeculectomy and 1 patient transscleral cyclophotocoagulation. During surgery we opted for from 2,000 to 2,250 mW infrared laser with 810 nm wavelength which uses a micropulse system of laser energy emission. For the procedure we used a new MP3 application probe. 30% drop in intraocular pressure (IOP) compared to the baseline IOP values was set as a success. Results: Our results correlated with the most of available studies when we achieved drop in the values of intraocular pressure by the minimum of 30% in 53.4% of the eyes. The effect of therapy failed in 9 eyes (18%) where we subsequently selected a different therapeutic procedure. Conclusion: MP CPC is a non-incisional laser treatment with minimum complications. It is characterized by a high safety profile and predictability of results. In our study we achieved drop in the values of intraocular pressure by the minimum of 30% in 53.4% of treated eyes. Its use is not limited by the type of glaucoma disease. In the event of insufficient effect, it may be repeated.

BACKGROUND: The aim of the study was to investigate the effect of the 532 nm (green) diode subthreshold micropulse laser (SML) in the treatment of non-centre involved clinically significant macular edema (CSME) in comparison to the conventional laser photocoagulation (CLP). METHODS: A total of 60 eyes of patients diagnosed with non-centre involved CSME were randomly divided into two groups. SML photocoagulation was performed in the first group (G1), while CLP in the second one (G2). Central macular thickness (CMT) and best corrected visual acuity (BCVA) were measured prior to treatment and at 3 and 6 months after intervention. RESULTS: G1 participants had significantly better CMT at 6 months after laser application (p = 0.04) compared to G2. Additionally, CMT in both groups was significantly lower 6 months after laser application in comparison to baseline values (G1: p < 0.001, G2: p = 0.002). Moreover, significant improvement was detected 6 months after SML in G1 regarding BCVA compared to values before laser treatment (p = 0.001). CONCLUSION: SML was more effective than CLP in reducing CMT and improving BCVA in patients with non-centre involved CSME. Therefore, it seems that SML can be a good substitute for CLP in DME treatment if confirmed in future studies.

Cyclodestructive procedures are an alternative of surgical treatment of medically refractory glaucoma. Aim: To assess efficiency and safety of diode cyclophotocoagulation (CPC). Methods: Retrospective study included 81 eyes with advanced glaucoma operated with CPC for elevated intraocular pressure (IOP) from January 2017 to January 2019. CPC was performed in retrobulbar anesthesia with contact diode laser FOX (A.R.C. Laser, Germany) of wavelength 810 nm, transsclerally to the ciliary body. Energy settings: intensity 2 W, exposition time 2 s, 18 applications in arc of 360°. Decrease of intraocular pressure was investigated during month 1, 6 and 12 after CPC. Safety was evaluated according to the best corrected visual acuity (BCVA) on Snellen optotypes and number of postoperative complications. Results: Study on 31 eyes of 24 patients, 21 (68 %) women and 10 (32 %) men, with follow-up during more than 12 months. Mean follow-up time was 19.5 ± 6.1 (from 12 to 29) months. Average age was 75.9 ± 9.2 (56 - 93) years. Indication for CPC was primary open angle glaucoma in 15 eyes (49 %), primary angle closure glaucoma in 6 eyes (19 %) and secondary glaucomas in 10 eyes (32 %). All patients were on therapy of 4 antiglaucomatic drops and 10 of them (32 %) on acetazolamide pills. IOP before CPC was 25.4 ± 11.0 (13–56) mm Hg. After 1 year IOP decreased to 16.9 ± 6.1 (8–40) mm Hg. Best corrected visual acuity (BCVA) before CPC was 0.39 ± 0.34 (0-1), 1 year after CPC 0.36 ± 0.33 (0-1). 1 year after CPC, 11 eyes (35 %) lost 0.23 ± 0.14 rows. 6 from these (19 %) due to other acquired ocular pathologies. Hypotony occurred in 6 % and uveitis in 10 % eyes. Conclusion: CPC is a safe and effective method of lowering IOP.
Úvod: Cyklodeštrukčné techniky sú alternatívou chirurgickej liečby glaukómu nedostatočne kompenzovaného medikamentóznou liečbou. Cieľ: Hodnotenie účinnosti a bezpečnosti diódovej cyklofotokoagulácie (CPC). Metódy: Retrospektívna štúdia 81 očí s pokročilými glaukómovými zmenami, ošetrených CPC od januára 2017 do januára 2019, v retrobulbárnej anestézii kontaktným diódovým laserom FOX (A.R.C. Laser, Germany) o vlnovej dĺžke 810 nm, transsklerálne, na oblasť ciliárnych výbežkov 1,5 mm od limbu v rozsahu 360° vynechaním č. 3 a 9 v počte 18 zásahov s intenzitou 2 W a expozíciou 2 s. Účinnosť CPC hodnotená podľa zníženia vnútroočného tlaku (VOT) 1, 6 a 12 mesiacov po CPC a bezpečnosť podľa výskytu komplikácií a výslednej najlepšie korigovanej centrálnej zrakovej ostrosti (NKCZO). Výsledky: Do súboru bolo zaradených 21 (68 %) ženských a 10 (32 %) mužských očí so sledovacou dobou minimálne 12 mesiacov, priemerne 19,5 ± 6,1 (rozsah 12–29) mesiacov. Vek pacientov v čase operácie bol 75,9 ± 9,2 (56–93) rokov. CPC bola vykonaná u 15 očí (49 %) s primárnym glaukómom s otvoreným uhlom (PGOU), 6 očí (19 %) s primárnym glaukómom so zatvoreným uhlom (PGZU) a u 10 očí (32 %) pri sekundárnom glaukóme. Všetci pacienti užívali kombináciu 4 antiglaukomatík a 10 (32 %) užívalo acetazolamid v tabletkách. VOT pred CPC bol 25,4 ± 11,0 (13–56) mm Hg. Po roku bol VOT 16,9 ± 6,1 (8–40) mm Hg. Priemerná NKCZO pred operáciou bola 0,39 ± 0,34 (0–1), 1 rok po CPC 0,36 ± 0,33 (0–1). Ku poklesu NKCZO rok po CPC došlo u 11 očí (35 %), pokles o 0,23 ± 0,14, z toho u 6 očí (19 %) v dôsledku iných pridružených očných ochorení. Z komplikácií CPC sme zaznamenali prechodnú hypotóniu (6 %) a indukovanú uveitídu (10 %). Záver: CPC je účinná a bezpečná metóda zníženia VOT.
