Nowadays, median nerve entrapment is a frequent issue. Many physicians are familiar with the most common median entrapment, which is the carpal tunnel syndrome (CTS). By contrast, less frequent entrapments, historically called “pronator syndrome” are still misdiagnosed as overuse syndrome, flexor tendinitis or other conditions. This article is meant to introduce proximal median nerve entrapments, followed by a case report of the rarest example – anterior interosseous nerve syndrome (AIN syndrome).

Perception of verticality can be assessed in two different ways: measuring of subjective visual vertical (SVV) and subjective haptic vertical (SHV). The evidence on aging of SVV and SVH is not conclusive and there is just little knowledge focused on this issue. The aim of this study was to compare accuracy of perception of SVV and SHV between groups of young and elderly healthy subjects. SVV examination was performed using the bucket test. An experimental tactile device was used to assess perceived SHV. Measurements of SVV and SHV were made in 27 young healthy and 30 elderly healthy subjects, both groups were right-handed due to self-report. SHV estimated position was significantly different (p< 0.01) in young and elderly (counterclockwise shift of 0.72 ± 3.70° and 3.51 ± 3.99°, respectively) and the SHV range (4.17 ± 5.40° and 9.64 ± 7.42°, respectively) was also different (p< 0.01). The differences were caused by significant difference in the supination maneuver (clockwise rod rotation, p< 0.001) which resulted in counterclockwise shift of 2.80 ± 4.90° and 8.33 ± 4.62° in young and elderly respectively. Pronation part of the SHV task (counterclockwise rod rotation) did not significantly differ between groups. SVV estimated position and range were not statistically different between young and elderly. These results provide evidence that the ability to detect SVV is not impaired, SHV seems also not to be affected by aging but there may be methodologic issues in SHV testing in elderly which should be controlled for in future studies. Results of both pronation and supination maneuvers should be provided along with SHV position.

Lidská chůze a běh se oprávněně považují za optimální pohybové aktivity téměř pro každého. Rekapitulujeme základní kineziologické a neurofyziologické poznatky o lidské lokomoci. Spolu s tím chápání bolestivých pohybových syndromů zejména jako důsledků funkční pohybové dyskoordinace při déletrvající a rychlé chůzi a rekreačním běhu. Nabízíme vybraná diagnostická a terapeutická východiska, užitečná pro klinického neurologa.
Human walking and running are rightly considered to represent an optimal physical activity for almost everyone. We recapitulate the principal kinesiology and neurophysiological findings about human locomotion. Futhermore this is present with respect to underestanding of painful movement disorders particularly as the result of functional physical discoordination during both prolonged fast walking and recreational running. We suggest selected diagnostic and therapeutic bases useful for the clinical neurologist.

PURPOSE OF THE STUDY: Abraham Colles classified and described fractures of the distal epiphyseal radius. He recommended the arm should be immobilized in a cast that extends from the base of the fingers to above the elbow, while holding this joint at ninety degrees of flexion the forearm in pronation and the wrist in slight flexion and ulnar deviation. We identified the brachioradialis muscle as the main culprit in the frequently observed loss of reduction of the fracture. Since the brachioradialis is attached to the distal region of the radius and functions as a flexor of the elbow when the forearm is in pronation, its stimulation easily displaces a reduced fracture, particularly if its geometry suggests axial instability. We concluded that post-reduction stabilization in supination was more desirable than in pronation. MATERIAL AND METHODS: Prospective study of one hundred and fifty-six patients suffering from Colles' fractures who were treated with the functional method. Approximately one-half of the fractures were immobilized in pronation and the other half in supination. The median age of the patients was 49 years. After approximately eleven days of immobilization in an above-the-elbow cast that held the forearm in a relaxed attitude of supination and the wrist in slight flexion and ulnar deviation, a new cast or brace was applied. The appliance permitted flexion of the elbow and slightly limited extension. We utilized modified Lindstom criteria to assess radiological results, according to types of fractures and by groups treated in supination and pronation. RESULTS: In the type I and III (non-displaced) fracture series there appeared to be no significant difference in the functional results between the pronation and supination treated groups. In the type II category, in the supinated fractures, there were 9 excellent, 4 good and no fair or poor results. In the pronated group 9 excellent, 8 good and one fair result. The functional results in type IV fractures treated in supination were excellent in 11 instances, good in 7 and fair in 2. In fractures treated in pronation there were 5 excellent, 10 good and 5 fair results. There were no poor results in either group. 85% of type II fractures and 85% of type IV fractures treated in supination had excellent or good results. In the pronation group, 67% had excellent or good results in type II and 40% in type IV classification. In combining the results for all types of braced Colles' fractures, (I-IV) 93% of the supination group and 87% of the pronation group achieved excellent or good functional results. In analyzing overall results regardless of type of fracture or position of immobilization, 90% of the patients had excellent or good results. CONCLUSION: We treated Colles' fractures in supination and compared the results with those obtained when treated in pronation. The results indicated a lower incidence of re-displacement in the supination group. We developed a forearm brace that permits flexion of the elbow, but prevented pronation of the forearm, and limited extension of the elbow in approximately the last fifteen degrees. It permits minimally limited flexion of the wrist but prevents wrist dorsiflexion. It makes impossible any radial deviation. The place of surgery in the management of Colles' fractures should be limited to those fractures that when treated by non-surgical means are not likely to render satisfactory functional and cosmetic results. There is not at this time a consensus as to when to use the surgical approach. The complication rate from the surgery have not clearly identify superiority of one over the other. Nonetheless, the surgical treatment has a definite place in the armamentarium of the orthopaedic surgeon. In a number of situations, it is the treatment of choice.

Cílem výzkumu je posoudit vliv vybraných indexů a úhlových parametrů nohy na pohyb v hlezenním, kolenním, kyčelním kloubu a pánve během chůzového cyklu. Výzkumný soubor tvořilo 27 mužů ve věku 24,1 ? 2,2 let, výškou 181 ? 6 cm a hmotností 78 ? 9 kg. Vybrané indexy a úhlové parametry nohy jsme získali zpracováním otisků bosého chodidla u každého z probandů. Kinematické parametry chůze jsme sledovali pomocí optoelektronického systému Vicon MX. Zaznamenali jsme vliv indexu nohy na pohyb ve všech kloubech dolní končetiny, statisticky významné byly rozdíly v hodnotách maxima flexe a extenze kyčelního kloubu v sagitální rovině během chůzového cyklu. Zjistili jsme, že úhel nohy a úhel paty nemají vliv u zdravých osob na pohyb kloubů dolní končetiny a pánve. Úhel palce ovlivňuje hodnoty maxima plantární flexe v hlezenním kloubu ve fázi postupného zatěžování.
The purpose of the study is to investigate the influence of selected indexes and angle parameters of the foot on kinematic changes of the ankle, knee, hip and pelvis during the gait cycle. The research group consisted of 27 male with an average age of 24.1. Selected indexes and foot angular parameters were obtained by processing four imprints of the bare foot in each of the subject. The Vicon MX system was used for kinematic analysis of the gait. We have noted the influence of foot indexes on kinematic parameters in all joints of the lower extremity. Statistically significant differences were found in maximum flexion and extension of the hip in the sagittal plane during the gait cycle. We found that the foot angle and heel angle do not affect the kinematic parameters of the lower extremity and pelvis. Big toe angle affects the peak of ankle plantar flexion during loading response.

Vykloubení lokte je druhé nejčastější luxační poranění v lidském těle. Vzhledem k jeho četnosti a riziku omezení funkčního rozsahu pohybu v lokti se autoři ve svém sdělení snaží podat stručný přehled o této problematice. V úvodu představují anatomii lokte, biomechaniku, etiopatogenezi a klasifikaci luxací lokte. Popisují klinické příznaky a vyšetřovací metody používané ke stanovení výše uvedené diagnózy a shrnují současné možnosti terapie jednotlivých forem luxací. V závěru sdělení je uveden přehled léčených pacientů s akutní luxací lokte na Klinice úrazové chirurgie FN Brno LF MU od ledna 2008 do srpna 2011.
Elbow dislocation is the second most common major joint dislocation in adults. With a view to dislocation frequency and the risk of posttraumatic functional limitations, the authors present a brief review of acute elbow dislocations. Initially, they introduce the elbow anatomy, biomechanics, etiopathogenesis and the classification of elbow dislocations. Clinical symptoms and examination methods used to determine the aforementioned diagnosis are described. Treatment options for different forms of elbow dislocations are further presented. Finally, the authors provide an overview of patients treated for acute elbow dislocation at the Trauma Surgery Department (University Hospital Brno) from January 2008 to August 2011.

We assessed the long-term results of 58 Souter-Strathclyde total elbow replacements in 49 patients with rheumatoid arthritis. The mean length of follow-up was 9.5 years (0.7 to 16.7). The mean pre-operative Mayo Elbow Performance Score was 30 (15 to 80) and at final follow-up was 82 (60 to 95). A total of 13 elbows (22.4%) were revised, ten (17.2%) for aseptic loosening, one (1.7%) for instability, one (1.7%) for secondary loosening after fracture, and one elbow (1.7%) was removed because of deep infection. The Kaplan-Meier survival rate was 70% and 53% at ten and 16 years, respectively. Failure of the ulnar component was found to be the main problem in relation to the loosening. Anterior transposition of the ulnar nerve had no influence on ulnar nerve paresthaesiae in these patients.
