Poruchy autonomní funkce srdce asociované s komorovými extrasystolami (KES) jsou často spojeny se snížením ejekční frakce a se zvýšenou kardiovaskulární morbiditou a mortalitou. Standardní zobrazovací techniky jako magnetická rezonance srdce (CMR) a koronární angiografie zajišťují vyloučení jiných příčin komorových extrasystol jako ischemie a neischemické fibrózy myokardu, ale nejsou schopny zobrazit narušení autonomního nervového systému srdce. Toto lze studovat pomocí scintigrafie myokardu (SPECT) s použitím 123 I-metajodbenzylguanidinu (MIBG). Vyšetřili jsme pilotní soubor šesti pacientů, z nichž u tří pacientů byla zjištěna porucha v adrenergní inervaci myokardu levé komory, a ty prezentujeme jako kazuistiky.
Disturbances of autonomic function in the heart associated with premature ventricular complexes (PVC) are associated with a decrease in ejection fraction as well as increased cardiac morbidity and mortality. Standard imaging techniques such as cMRI and coronary angiography secure the exclusion of other causes of PVC as ischemia and progressive fibrosis. These methods are not suitable for cardiac autonomic nervous system imaging and can't describe the cause of related PVCs. This may be described by 123I-metaiodobenzylguanidine (MIBG) scintigraphy. We examined a pilot group of 6 patients, in which 3 individuals presented a disorder in the adrenergic innervation of the left ventricular myocardium and we present these as case reports.

BACKGROUND: There is a continuing interest in finding effective methods for scar treatment. Dry needling is gaining popularity in physiotherapy and is defined by Western medicine as a type of acupuncture. The terms acupuncture and dry needling have been used interchangeably so we have focused on the efficacy of dry needling or acupuncture in scar treatment. OBJECTIVE: The aim of this systematic review was to determine the usefulness of dry needling or local acupuncture for scar treatment. In our search process, we used the terms 'acupuncture,' 'needling,' or 'dry needling' to identify all relevant scientific papers. We have focused on the practical aspects of local management of different scar types with dry needling or acupuncture. SEARCH STRATEGY: The search strategy included different combinations of the following keywords: 'scar', 'keloid', 'dry needling', 'needling', 'acupuncture', 'treatment', 'physical therapy'. This systematic review was conducted in accordance with PRISMA guidelines. MEDLINE (PubMed, EBSCOHost and Ovid), EMBASE (Elsevier), and Web of Science databases were searched for relevant publications from inception through October 2023. INCLUSION CRITERIA: The studies that investigated the effectiveness of dry needling or acupuncture for scar treatment were included. DATA EXTRACTION AND ANALYSIS: The main extraction data items were: the needling technique; needle: diameter, length; needling locations; manual needling manipulation; number of sessions; settings; outcomes and results. RESULTS: As a result of a comprehensive search, 11 manuscripts were included in the systematic review, of which eight were case reports, two were randomized trials and one study concerned case series. Two case reports scored 2-4 out of 8 points on the JBI checklist, five studies scored 5-7, and one study scored 8 points. The methodological quality of the two clinical trials was rated as good or fair on the PEDro scale. The case series study scored 7 of 10 points on the JBI checklist. A meta-analysis was not possible as only two randomized trials, eight case reports, and one case series were eligible for review; also, scar assessment scales and pain severity scales were highly heterogeneous. CONCLUSIONS: The studies differed regarding the delivery of dry needling or local acupuncture for scar treatment. Differences included treatment frequency, duration, number of treatments, selection of needle insertion sites, number of needles used, angle of needle placement, and use of manual needling manipulation. Článek pojednává o riziku přenosu infekce SARS-CoV-2 (Severe Acute Respiratory Syndrome-related coronavirus) pro plod v děloze. Transplacentární přenos viru je možný, ale je vzácný díky speciálnímu ochrannému vybavení placenty. Ve vlastním souboru rodiček, který autoři sestavili, bylo riziko nákazy plodu a novorozence nízké. U nemocných rodiček vyšetřovali histologické změny placenty a popsali obraz placentitidy způsobený infekcí SARS-CoV-2. Autoři přidali ilustrační kazuistiku nedonošeného dítěte s vrozenou infekcí SARS-CoV-2. V diskuzi pak autoři popisují možnosti a podrobnosti přenosu SARS-CoV-2 na plod a novorozence, onemocnění novorozenců a prognózu na základě literárních referencí.
This article discusses the risk of transmission of SARS-CoV-2 (Severe acute respiratory syndrome-related coronavirus) infection to the fetus in utero. Transplacental transmission of the virus is possible but is rare due to the special protective equipment of the placenta. In the authors' own cohort of parturients, the risk of fetal and neonatal infection was low. They examined histological changes in the placentas of the affected mothers and described a picture of placentitis caused by SARS-CoV-2 infection. The authors added an illustrative case report of a premature infant with congenital SARS-CoV-2 infection.In the discussion, the authors then describe the possibilities and details of SARS-CoV-2 transmission to the fetus and newborn, neonatal disease, and prognosis based on literature references.

Nowadays, median nerve entrapment is a frequent issue. Many physicians are familiar with the most common median entrapment, which is the carpal tunnel syndrome (CTS). By contrast, less frequent entrapments, historically called “pronator syndrome” are still misdiagnosed as overuse syndrome, flexor tendinitis or other conditions. This article is meant to introduce proximal median nerve entrapments, followed by a case report of the rarest example – anterior interosseous nerve syndrome (AIN syndrome).

Stafylokokový syndrom toxického šoku (STŠ) je závažné onemocnění, které může být život ohrožující. Původcem jsou kmeny Staphylococcus aureuss produkcí toxinu syndromu toxického šokuTSST-1 a/nebo enterotoxinu. Existují dvě formy této infekce, menstruální a nemenstruální. Za 40 let v období 1983–2022 jsme v NRL pro stafylokoky CEM SZÚ zaregistrovali 156 případů STŠ bez souvislosti s menstruací. V řadě propouštěcích zpráv byl uváděn těžký průběh včetně hospitalizace na oddělení ARO. Z celého souboru skončilo 29 případů úmrtím. STŠ nejčastěji komplikoval pyodermie (26,9 %) a zhnisané poúrazové rány (18,6 %). Na dalších místech byly zhnisané rány pooperační (14,7 %) a zhnisané popáleniny (10,2 %). V článku charakterizujeme toxinogenní kmeny S. aureus, které jsme mohli potvrdit jako původce těchto onemocnění. Nejčastěji se jednalo o producenty TSST-1 v kombinaci s enterotoxinem – obvykle typu A: 74 (47,3 %). Toxin TSST-1 produkovalo výlučně 21 kmenů (13,5 %). Celkem 61 kmenů (39,1 %) produkovalo pouze nějaký typ enterotoxinu (A, B, C, D a H), samostatně nebo i ve dvojkombinaci. Většina kmenů byla citlivá k methicilinu (MSSA), pouze 8 kmenů bylo MRSA. Zjištění klinické diagnózy a tedy i adekvátní terapie, může být u STŠ obtížné, protože kromě základních příznaků existuje řada další klinických projevů, které se u tohoto onemocnění mohou vyskytovat. Je tedy velice potřebné zjištění stafylokokové toxinové etiologie.
Staphylococcal toxic shock syndrome (TSS) is a serious condition that can be life-threatening. It is caused by Staphylococcus aureus strains with the production of toxic shock syndrome toxin-1 (TSST-1) and/or enterotoxin. There are two forms of this infection, menstrual and non-menstrual. In the 40 years between 1983 and 2022, we registered 156 cases of TSS unrelated to menstruation in the NRL for Staphylococci of the Centre for Epidemiology and Microbiology of the National Institute of Public Health. A number of discharge reports indicated a severe course of the disease including hospitalization at the Department of Anaesthesiology and Resuscitation. Out of the entire set, 29 cases ended in death. TSS most often complicated pyoderma gangrenosum (26.9%) and traumatic wounds (18.6%). Other places were festering surgical wounds (14.7%) and festering burns (10.2%). In the article, we characterize the toxigenic S. aureus strains that we were able to confirm as the causative agents of these diseases. The most common were TSST-1 producers in combination with enterotoxin – usually type A: 74 (47.3%). TSST-1 toxin was exclusively produced by 21 strains (13.5%). A total of 61 strains (39.1%) produced only some type of enterotoxin (A, B, C, D and H), alone or in a double combination. Most strains were susceptible to methicillin (MSSA), only 8 strains were MRSA. Establishing a clinical diagnosis and therefore an adequate therapy can be difficult in TSS, because in addition to the basic symptoms, there are a number of other clinical manifestations that can occur in this disease. It is therefore necessary to identify the staphylococcal toxin aetiology.
