AIMS: Our aim was to describe the changes in therapy and diabetes control in Ukrainian war refugee children with diabetes (CwD) during the first year of their stay in Czechia. METHODS: A total of 124 CwD (62 male, 62 female) were enrolled into this observational study. Anthropometric, laboratory and diabetes management data were acquired at baseline and at 3 months intervals for 12 months. All CwD were offered a CGM device during their first visit. Generalized Estimating Equation models were fitted in order to estimate the dynamics of studied characteristics. RESULTS: Median baseline HbA1c was 58 mmol/mol (IQR [48; 73]mmol/mol) (7.5 %, IQR[6.5;8.8]%). The HbA1c decreased significantly throughout the course of the study at a pace of - 2.2 mmol/mol (-0.2 %pt.) per visit (P = 0.01, CI[-3.2;-1.1]). The pace of the decrease in the average HbA1c was significantly higher in the group of CwD who received CGM in Czechia than in those who already had it from Ukraine by 2.9 mmol/mol (0.27 %pt.) per visit (P < 0.001, CI [-4.4; -1.3]). CONCLUSIONS: The steepest decrease in HbA1c was observed in CwD with newly initiated CGM underlining its vital role in improving the glucose control of CwD regardless of their background.

OBJECTIVE: Russia's invasion of Ukraine in February 2022 triggered a huge wave of migration, with thousands of refugees arriving at Prague's Central Station. With no medical service available, medical students started to volunteer and were running an infirmary available around the clock. The research aimed to explore medical students' professional experiences, emotionally challenging situations, and coping strategies in this demanding setting. METHODS: The authors used a qualitative analysis of semi-structured in-depth interviews with 19 participants recruited through purposive sampling. Interviews took place between April and May 2022. Data were recorded, transcribed, and processed by using thematic qualitative analysis. RESULTS: The research identified five emotionally challenging situations: caring for traumatized refugees, refusal of treatment, disruption of everyday life, independent decision-making, and complexity of voluntary work. Students adopted both adaptive and maladaptive coping strategies for dealing with stress and trauma. Almost half the students reported signs of secondary traumatization and moral distress; however, no one asked for psychological help. CONCLUSIONS: Medical school curricula should cover trauma-informed approaches, healthy coping strategies, and destigmatization of psychological problems.

BACKGROUND: We aimed to screen Ukrainian war refugees (UWR) in Czechia for depression and anxiety, and to assess their recognition of personal mental health problems and related help-seeking. METHODS: We conducted a cross-sectional study on a sample of UWR in Czechia. We used PHQ-8 and GAD-7 to screen for depression and anxiety, SELF-I to assess the recognition of respondents' own mental health problems, and a set of questions regarding mental health-related help-seeking. FINDINGS: Our sample consisted of 1,347 UWR. More than 41 % of respondents screened positively for moderate or severe depression and more than 23 % for moderate or severe anxiety. Self-recognition of mental health as well as help-seeking was very low among those who screened positively for moderate or severe depression or anxiety. INTERPRETATION: Even those UWR who report severe symptoms do not identify themselves as potentially having mental health issues and are not seeking help.

Cílem studie bylo pohledem lékařů identifikovat a vysvětlit bariéry v komunikaci mezi českými lékaři a uprchlíky z Ukrajiny při poskytování zdravotní péče. Studie reaguje na příchod velkého množství nucených migrantů z Ukrajiny v roce 2022 a nároky, které jejich pobyt klade na český zdravotnický systém, a to zejména v oblasti komunikace lékaře s pacientem-migrantem. Práce je motivována zahraničními poznatky, kdy nedorozumění a nekvalitní komunikace může vést k nižší spokojenosti a adherenci pacientů, a v důsledku toho i k horším výsledkům zdravotní péče. Kvalitativní výzkum byl realizován 16 hloubkovými polostrukturovanými rozhovory s lékaři prvního kontaktu poskytujícími zdravotní péči ukrajinským uprchlíkům. Informanti byli vybíráni metodou záměrného výběru s cílem získat co nejpestřejší vzorek. Výsledná data byla analyzována s využitím tematického kódování. Z výsledků vyplynuly čtyři hlavní oblasti interkulturních bariér mezi lékaři a pacienty-ukrajinskými uprchlíky: (1) jazyk, (2) odlišnosti zdravotnických systémů Česka a Ukrajiny, (3) odlišný přístup ke zdraví a nemoci a (4) předsudky. Největším zdrojem neporozumění byla odlišnost českého a ukrajinského zdravotnického systému, ze které vyplývají odlišné role a postavení lékařů a pacientů ve zdravotnických systémech. Závěry výzkumu ukazují, že v poskytování zdravotní péče ukrajinským uprchlíkům hrají významnou roli interkulturní bariéry, které ale lze za využití vhodných nástrojů překlenout. Současná geopolitická situace v Evropě a narůstající diverzita ve společnosti vede k potřebě osvojovat si interkulturní kompetence v rámci pregraduálního a celoživotního vzdělávání lékařů. Stejně tak je třeba přijímat opatření, která podporují kulturně kompetentní zdravotní péči.
The aim of the study was to identify and explore barriers to communication between refugees from Ukraine and physicians providing health care, the perspective of physicians was embraced. The focus of the work reflects on the arrival of many forced migrants from Ukraine in 2022. Further, it focuses on the pressure that the presence of migrants’ places on the Czech healthcare system, especially in doctor-migrant patient communication. The objectives of this study are also motivated by existing international evidence that misunderstandings and poor-quality communication can lead to lower patient satisfaction and adherence, and consequently poorer healthcare outcomes. The research was carried out in the form of qualitative research, 16 in-depth semi-structured interviews were conducted with physicians providing health care to Ukrainian refugees. Informants were selected using a purposive sampling method to obtain as diverse a sample as possible. The resulting data were analysed by applying thematic coding. The results showed that four main areas of intercultural barriers play a key role in the relationship between physicians and Ukrainian patients. These are: (1) language, (2) differences in healthcare systems, (3) different attitudes towards health and illness and (4) prejudice. The major source of misunderstanding was the difference between the Czech and Ukrainian healthcare systems, which leads to a different position of the patient in the healthcare system. The conclusions prove that intercultural barriers play a significant role in the provision of health care to Ukrainian refugees but can be addressed to a large extent. The current situation in Czechia and the increasing diversity in society call for the need to acquire intercultural competencies in undergraduate and continuing medical education. Similarly, the adoption of measures that promote culturally competent health care is needed.

MyHealth European project (2017-2020) was committed to elaborate on models to engage vulnerable migrants and refugees (VMR) in their health through community involvement. Low healthcare access and poor quality of healthcare services for VMR is a common reality in many European countries. The purpose of the present study, as part of MyHealth project, was the development of an agenda for actions and consequent recommendations to tackle the issue. A qualitative research design was applied at four study sites in Barcelona, Spain; Berlin, Germany; Brno, Czech Republic and Athens, Greece. The Metaplan® group discussions allowed the collection, organization and process of ideas and opinions elaborated in the collaborative groups. In total, 14 sessions took place: 4 with health and social professionals (n = 41) and 10 with VMR (n = 77). A participatory thematic analysis was performed at every session and overall for all sessions a thematic analysis synthesized the findings. The suggested actions were divided into two levels of recommendations: (i) local authorities at destination country-related, such as the investment in health professionals' cultural competences, and (ii) VMR-related, such as adaptation of help-seeking behavior patterns. Special attention was proposed to women survivors of violence and homeless minors. The study concluded to an agenda for action in Europe. We advocate for a public health paradigm shift where, while holding a bottom-up approach, VMR as well as professionals working with them are actively and meaningfully engaged in the decision-making process of access-enhancing and health promotion strategies in a given socio-cultural context.

OBJECTIVE: Over 480 000 Ukrainian refugees have arrived in the Czech Republic since the Russian invasion of Ukraine in 2022, including over 500 people with HIV. This study describes the demographics, characteristics, and management of Ukrainian refugees with HIV in the Czech Republic. DESIGN: Retrospective, observational, noninterventional study. METHODS: Ukrainian nationals registering at HIV centers in the Czech Republic with war refugee status were included. Data were collected from medical records between 1 March and 31 July 2022. The study was registered with the Czech State Institute for Drug Control, ID number 2301200000. RESULTS: Four hundred and eighty-two patients were included in the study. Most patients were female (69.5%; n = 335/482) with well-controlled HIV. The median [interquartile range] CD4 + cell count was 597 [397] cells/μl of blood, and 79.3% ( n = 361/455) of patients had HIV RNA <40 copies/ml. Coinfections of hepatitis C virus, hepatitis B virus, and/or tuberculosis were reported for 17.4% ( n = 78/449), 9% ( n = 40/446) and 1.3% ( n = 6/446) of patients, respectively. In Ukraine, 85.7% ( n = 384/448) of patients had been receiving an integrase strand transfer inhibitor-based regimen and most (69.7%; n = 310/445) did not switch therapy upon arrival in the Czech Republic. CONCLUSION: Migration from Ukraine is changing the characteristics of HIV epidemiology in the Czech Republic. Ukrainian refugees with HIV have been provided with a high standard of medical care in the Czech Republic. Improved coordination between medical services within the Czech Republic and between countries in the European Union is necessary to optimize patient care.

Úvod a cíl: Cílem článku je odhadnout míru zneužívání alkoholu a některých dalších psychických problémů u Ukrajinců, kteří vstoupili na území České republiky po 24. únoru 2022. Předchozí studie zaměřené na užívání alkoholu a dalších návykových látek mezi Ukrajinci v České republice nelze na současnou situaci aplikovat, neboť se většinou týkaly mužské populace a ekonomické migrace, která se od situace v roce 2022 značně lišila. Metody: Online průzkum byl proveden na příležitostném (convenience) nereprezentativním vzorku 66 ukrajinských uprchlíků v České republice. Většina respondentů přišla do České republiky se svými dětmi. Ke zjištění důvodů pití alkoholu a případných depresivních symptomů byly použity standardní dotazníky týkající se užívání návykových látek (alkohol, tabák). Výsledky: Podle výsledků nástroje AUDIT lze většinu respondentů z hlediska pití alkoholu zařadit do kategorie nízkého rizika. Odpovědi některých respondentů indikovaly problémy spojené s alkoholem. Byla zjištěna vysoká pozitivní korelace mezi změnou frekvence pití alkoholu a změnou množství alkoholu užitého během typické příležitosti k pití, přičemž změny se zjišťovaly od začátku války a od příchodu do Česka. Výsledky také ukázaly vysokou míru symptomů depresivity a vysokou míru kouření. Závěr: Výsledky jsou v souladu s jinými (řídkými) výzkumy v této oblasti. Je zdůrazněn význam výzkumu konzumace alkoholu, přičemž by byl velmi vítaný dlouhodobější výzkum.
Introduction and objective: The article aims to estimate the extent of alcohol use and some other mental health issues among Ukrainians who entered the Czech Republic after 24th February 2022. Previous studies focused on the use of alcohol and other substances among Ukrainians in the Czech Republic can't be applied to the current situation as these studies mostly covered the male population and economic migration that differed considerably from the situation in 2022. Methods: A convenience non-representative online survey was conducted on a sample of 66 Ukrainian refugees in the Czech Republic. Most respondents came to the Czech Republic with their children. Standard questionnaires related to substance use (alcohol, tobacco) were used to determine the reasons for alcohol drinking and possible depression symptoms. Results: According to the results of the AUDIT tool, most respondents can be classified as low risk in terms of alcohol drinking. Some respondents referred to alcohol-related problems. Since the beginning of the war and since arriving in Czechia, there is a high positive correlation between the change in drinking frequency and the change in the amount of alcohol use during a typical drinking occasion. The results also showed a high level of depression symptoms and a high level of smoking. Conclusion: The results are in line with other (rare) research in the field. The importance of alcohol consumption research is stressed, while longer-term research is highly desirable.
