AIMS: To analyse the hallmarks of ocular manifestations of and therapeutic modalities for syphilis in the last two decades. The clinical features of syphilitic uveitis, and association with the human immunodeficiency virus (HIV) coinfection are described. METHODS: Retrospective study of 16 patients diagnosed with ocular syphilis confirmed by serological tests in the General University Hospital in Prague between the years 2004 and 2021. General characteristics of ocular and systemic manifestations and visual functions were analysed. RESULTS: An increasing incidence of syphilitic uveitis correlates with a general rise in syphilis cases. In our study, the ocular manifestation of syphilis was panuveitis (44%), posterior uveitis (31%) and anterior uveitis (25%). Posterior uveitis was found in 3 patients (19%) associated with preretinal infiltrates, that are often present in syphilitic uveitis. The worst visual outcomes were among patients with human immunodeficiency virus (HIV) coinfection and/or neurosyphilis, however the data were not significant. Optic disc edema was present in 56%, macular involvement in 37% of patients. Overall, 31% of patients in our cohort had persistent visual field defects due to impairment of their optic nerve or macula despite the final median Snellen visual acuity of 1.0. Two out of sixteen patients were treated with corticosteroids in addition to antibiotics. CONCLUSION: Posterior uveitis with preretinal infiltrates and optic disc edema should arouse suspicion of ocular syphilis. Recent data show the advantages of adjacent systemic corticosteroid treatment for severe forms of syphilitic uveitis and/or neuritis. Our observation supports this finding.

HIV infection in children is rare in developed countries. The relatively low proportion of women among HIV-positive individuals, the screening of pregnant women, and prophylactic measures to prevent vertical transmission hinder a more significant spread of the infection in children. Currently, there are around 20 HIV-infected children in health care in the Czech Republic, including refugees from Ukraine. Antiretroviral therapy (ART) is an effective tool in the care of HIV-infected people that allows for a virtually asymptomatic course of the disease, with a life expectancy comparable to that of the general population. The treatment of children with HIV infection requires an interdisciplinary approach: at least a general paediatrician and infectious disease specialist with expertise in HIV infection. The general paediatrician should even be prepared for rare cases of hitherto undetected or unacknowledged HIV infection in children. The clinical presentation of HIV infection and treatment management in children have its specific features described in this article.
Infekce HIV u dětí je v rozvinutých zemích vzácná. Relativně nízké zastoupení žen mezi HIV pozitivními, screening těhotných a profylaktická opatření k zábraně vertikálního přenosu nedovolují významnější rozšíření infekce u dětí. Aktuálně je v České republice včetně uprchlíků z Ukrajiny ve zdravotní péči okolo 20 HIV infikovaných dětí. Účinným nástrojem péče o HIV infikované je antiretrovirová terapie (ART), která dovoluje prakticky bezpříznakový průběh nemoci s předpokladem délky života srovnatelné s běžnou populací. Léčba dětí s infekcí HIV vyžaduje interdisciplinární přístup - přinejmenším praktického pediatra a infektologa se zkušeností v problematice infekce HIV. Praktický pediatr musí být přípraven i na vzácné případy dosud neodhalené nebo nepřiznané infekce HIV u dětí. Klinický obraz infekce HIV a vedení léčby má u dětí své zvláštnosti popsané v článku.

BACKGROUND: HIV-positive men who have sex with men (MSM) are more likely to experience human papillomavirus (HPV) infection. The persistent HPV infection is the major factor in the development of anal and oropharyngeal neoplasms. Data on the prevalence of anal and oral HPV in MSM are almost absent from the countries of Central and Eastern Europe. We conducted a cross-sectional study focused on the prevalence of oral and anal HPV infections and the relationship between current anal and oral HPV intrapersonal infection in a Czech population of predominantly HIV-positive MSM. METHODS: Oral gargle and anal swab samples from 205 predominantly HIV-positive MSM from the Czech Republic were analysed for HPV infection using PCR. Selected sociodemographic and clinical data were correlated with HPV detection using generalized linear models and multivariate analysis. RESULTS: HPV infection was detected in 183 (96.8%) anal and 48 (23.6%) oral samples. The most common type of HR-HPV was HPV16 in both anal (25.4%) and oral (2.5%) samples. Multiple anal HPV infections and the presence of vaccine-targeted HR-HPV types were significantly correlated with abnormal anal cytology and HIV status. CONCLUSION: The prevalence of anal HPV infection in Czech predominantly HIV-positive MSM ranks among the highest reported, while oral HPV prevalence is consistent with MSM populations. Minimal overlap of oral and anal HPV types within a patient was observed.

The aim of this paper is to introduce the digitalization process and its effects on better reach of the target population. Progress in the digitalization and e-health tools worldwide enables new opportunities in prevention, diagnostics and treatment for people living with HIV (PLHIV) and people in the risk of HIV infection, hepatitis C (HCV) and other sexually transmitted infections (STIs), especially in the context of the COVID-19 pandemic. The system already used for voluntary counselling and testing (VCT) at the CheckPoint Centre Zagreb run by the non-governmental organization (NGO) Croatian Association for HIV and Viral Hepatitis (CAHIV) was upgraded and adapted (due to the COVID-19 prevention epidemiological measures) and developed for implementation of the pilot project of feasibility and acceptability of home HIV self-testing (HIVST) among men who have sex with men (MSM) in Zagreb. A special feature of the HIVST mobile application enables an innovative approach in collecting clients' test result feedback. This paper presents the method of use digitalization of the VCT and HIVST activities to support and increase availability of screening testing. Described procedures of new technologies application in VCT services and preliminary results of the HIVST pilot project indicate that technology-delivered interventions can contribute and improve access and utilisation of HIV/STI prevention and care services.

Pandemie HIV/AIDS zůstává celosvětovým problémem. V České republice je infekce rozšířena zejména mezi muži, ale v posledních letech přibývá výskytu onemocnění i mezi ženami. Dermatologické problémy jsou u osob žijících s HIV časté, mnohokrát vedou i k diagnostice této infekce. Uvádíme postup při vyšetřování anti-HIV protilátek, výčet zdravotních i epidemiologických indikací, při kterých je vhodné vyšetření provádět a seznam HIV center. Uvádíme průběh HIV infekce s výskytem typických příznaků v závislosti na stadiu onemocnění. Závěrem se věnujeme profylaxi podáváním antiretrovirotik po rizikové situaci a informujeme i o možnosti preexpoziční profylaxe. Včasná diagnostika umožní kvalitní život nemocného a snižuje možnost přenosu infekce na další osoby.
Pandemic HIV/AIDS infection is still a global problem. The infection is mainly among men in the Czech Republic but in recent years the disease has increased among women as well. Dermatological problems are common in people living with HIV and often lead to the diagnosis of this infection. We present the procedure for the investigation of anti-HIV antibodies and a list of health and epidemiological situations in which it is appropriate to examine these antibodies and a list of HIV centers. The occurrence of typical symptoms is reported depending on the stage of the disease. Finally, we focus on prophylaxis by administering antiretrovirals after a risk situation and inform about the possibility of preexposure prophylaxis. Early diagnosis enables a quality of patient ́s live and reduces the possibility of infection transmission to others.
