Karcinom pankreatu je onemocnění s vysokou mortalitou, často zachycené v pozdním stadiu. Jedinou kurativní léčbou je chirurgická resekce, výsledky zlepšuje adjuvantní chemoterapie. U pacientů s pokročilým nálezem je v 1. linii paliativní léčby doporučen režim FOLFIRINOX nebo doublet gemcitabin + nab-paklitaxel, ve 2. linii pak po terapii založené na gemcitabinu nal-IRI (pegylovaný lipozomálnî irinotekan) + 5-fluorouracil (5-FU) + leukovorin (LV), nebo režim s 5-FU, a po terapii s 5-FU gemcitabin v monoterapii či v kombinaci. Nové výsledky prezentované na kongresu ESMO 2023 ukázaly, že v 1. linii paliativní léčby je větším přínosem doublet gemcitabin + nab-paklitaxel než FOLFIRINOX. Prezentujeme kazuistiku 50letého muže s progresí karcinomu pankreatu po chirurgické a adjuvantní léčbě, u něhož bylo po selhání gemcitabinu + nab-paklitaxelu v 1. linii paliativní léčby dosaženo již ročního přežití s dobrou kvalitou života při kombinaci nal-IRI + 5-FU/LV.
Pancreatic cancer is a disease with high mortality rates, often detected at late stage. Surgical resection is the only curative treatment, and the outcomes are improved by adjuvant chemotherapy. In patients with an advanced finding, the FOLFIRINOX regimen or the doublet gemcitabine + nab-paclitaxel are recommended in the first-line of palliative treatment; in the second-line, following the gemcitabine-based treatment, nal-IRI (pegylated liposomal irinotecan) + 5-fluorouracil (5-FU) + leucovorin (LV), or the regimen with 5-FU are recommended; and following the therapy with 5-FU, gemcitabine in monotherapy or in combination. New results presented at the 2023 ESMO Congress showed that, in the first-line of palliative treatment, the doublet gemcitabine + nab-paclitaxel had a greater benefit than the FOLFIRINOX regimen. We report a case of a 50-year-old man with progression of pancreatic cancer after surgical and adjuvant treatment in whom, after failure of gemcitabine + nab-paclitaxel in the first-line of palliative treatment, one-year survival with a good quality of life was achieved with the combination of nal-IRI + 5-FU/LV.

INTRODUCTION: Recurrent or primary advanced metastatic cervical cancer (R/M CC) has a poor prognosis with a 5-year-survival rate of 16.5%, demanding novel and improved therapies for the treatment of these patients. The first-line standard of care for R/M CC now benefits from the addition of the immune checkpoint inhibitor, pembrolizumab, to platinum-based chemotherapy with paclitaxel and bevacizumab. Additionally, new options for second-line treatment have become available in recent years. AREAS COVERED: Here, we review current investigational drugs and discuss their relative targets, efficacies, and potential within the R/M CC treatment landscape. This review will focus on recently published data and key ongoing clinical trials in patients with R/M CC, covering multiple modes of action, including immunotherapies, antibody-drug conjugates, and tyrosine kinase inhibitors. We searched clinicaltrials.gov for ongoing trials and pubmed.ncbi.nih.gov for recently published trial data, as well as recent years' proceedings from the annual conferences of the American Society of Clinical Oncology (ASCO), European Society for Medical Oncology (ESMO), European Society of Gynaecological Oncology (ESGO), and the International Gynecologic Cancer Society (IGCS). EXPERT OPINION: Therapeutics currently attracting attention include novel immune checkpoint inhibitors, therapeutic vaccinations, antibody-drug conjugates, such as tisotumab vedotin, tyrosine kinase inhibitors targeting HER2, and multitarget synergistic combinations.
Paclitaxel, a compound naturally occurring in yew, is a commonly used drug for the treatment of different types of cancer. Unfortunately, frequent cancer cell resistance significantly decreases its anticancer effectivity. The main reason for the resistance development is the paclitaxel-induced phenomenon of cytoprotective autophagy occurring by different mechanisms of action in dependence on a cell type and possibly even leading to metastases. Paclitaxel also induces autophagy in cancer stem cells, which greatly contributes to tumor resistance development. Paclitaxel anticancer effectivity can be predicted by the presence of several autophagy-related molecular markers, such as tumor necrosis factor superfamily member 13 in triple-negative breast cancer or cystine/glutamate transporter encoded by the SLC7A11 gene in ovarian cancer. Nevertheless, the undesired effects of paclitaxel-induced autophagy can be eliminated by paclitaxel co-administration with autophagy inhibitors, such as chloroquine. Interestingly, in certain cases, it is worthy of potentiating autophagy by paclitaxel combination with autophagy inducers, for instance, apatinib. A modern strategy in anticancer research is also to encapsulate chemotherapeutics into nanoparticle carriers or develop their novel derivatives with improved anticancer properties. Hence, in this review article, we summarize not only the current knowledge of paclitaxel-induced autophagy and its role in cancer resistance but mainly the possible drug combinations based on paclitaxel and their administration in nanoparticle-based formulations as well as paclitaxel analogs with autophagy-modulating properties.

Background: The aim of the present study was to examine the efficacy of carboplatin in combination with paclitaxel in patients with metastatic castration-resistant prostate cancer pretreated with multiple regimens including docetaxel and androgen receptor-targeted agents. Methods: Clinical data from patients treated with carboplatin plus paclitaxel were collected retrospectively from a single institution. Results: 43 patients with metastatic castration-resistant prostate cancer were identified. Median number of cycles was ten (range: 1 to 23), prostate-specific antigen response was observed in 18 (42%) patients, median progression-free survival was 115 days and median overall survival was 8.1 months. Conclusion: Combination chemotherapy using taxane with carboplatin is an effective and well-tolerated therapy in heavily pretreated patients with metastatic castration-resistant prostate cancer.

Kazuistika popisuje případ pacientky s lokálně pokročilým karcinomem pankreatu, u které systémová chemoterapie (režim gemcitabin + nab-paclitaxel) a zařazení chemoradioterapie s cílem zvýšit lokální kontrolu vedla k dlouhodobé kontrole onemocnění a k neobvykle dlouhé době přežití v délce 26 měsíců. Postavení radioterapie v léčbě lokálně pokročilého karcinomu pankreatu je v současnosti kontroverzní. Probíhající studie mají za cíl identifikovat podskupinu pacientů, u kterých je zařazení lokoregionální terapie v léčbě lokálně pokročilého karcinomu pankreatu spojeno s prodloužením celkového přežití.
We describe a case-report of patient with locally advanced pancreatic cancer. Chemotherapy (gemcitabine + nab-paclitaxel) followed by chemoradiotherapy lead to long-term disease control and survival lasting 26 months. However radiotherapy in the treatment of locally advanced pancreatic cancer is controversial. There are studies which aims to identify subgroup of patients, in which adding locoregional therapy in the treatment of locally advanced pancreatic will lead in improvement in survival.

BACKGROUND: Most patients with epithelial ovarian cancer (EOC) relapse despite primary debulking surgery and chemotherapy (CT). Autologous dendritic cell immunotherapy (DCVAC) can present tumor antigens to elicit a durable immune response. We hypothesized that adding parallel or sequential DCVAC to CT stimulates antitumor immunity and improves clinical outcomes in patients with EOC. Based on the interim results of sequential DCVAC/OvCa administration and to accommodate the increased interest in maintenance treatment in EOC, the trial was amended by adding Part 2. METHODS: Patients with International Federation of Gynecology and Obstetrics stage III EOC (serous, endometrioid, or mucinous), who underwent cytoreductive surgery up to 3 weeks prior to randomization and were scheduled for first-line platinum-based CT were eligible. Patients, stratified by tumor residuum (0 or <1 cm), were randomized (1:1:1) to DCVAC/OvCa parallel to CT (Group A), DCVAC/OvCa sequential to CT (Group B), or CT alone (Group C) in Part 1, and to Groups B and C in Part 2. Autologous dendritic cells for DCVAC were differentiated from patients' CD14+ monocytes, pulsed with two allogenic OvCa cell lines (SK-OV-3, OV-90), and matured in the presence of polyinosinic:polycytidylic acid. We report the safety outcomes (safety analysis set, Parts 1 and 2 combined) along with the primary (progression-free survival (PFS)) and secondary (overall survival (OS)) efficacy endpoints. Efficacy endpoints were assessed in the modified intention-to-treat (mITT) analysis set in Part 1. RESULTS: Between November 2013 and March 2016, 99 patients were randomized. The mITT (Part 1) comprised 31, 29, and 30 patients in Groups A, B, and C, respectively. Baseline characteristics and DCVAC/OvCa exposure were comparable across the treatment arms. DCVAC/OvCa showed a good safety profile with treatment-emergent adverse events related to DCVAC/OvCa in 2 of 34 patients (5.9%) in Group A and 2 of 53 patients (3.8%) in Group B. Median PFS was 20.3, not reached, and 21.4 months in Groups A, B, and C, respectively. The HR (95% CI) for Group A versus Group C was 0.98 (0.48 to 2.00; p=0.9483) and the HR for Group B versus Group C was 0.39 (0.16 to 0.96; p=0.0336). This was accompanied by a non-significant trend of improved OS in Groups A and B. Median OS was not reached in any group after a median follow-up of 66 months (34% of events). CONCLUSIONS: DCVAC/OvCa and leukapheresis was not associated with significant safety concerns in this trial. DCVAC/OvCa sequential to CT was associated with a statistically significant improvement in PFS in patients undergoing first-line treatment of EOC. The main problem precluding successful therapy with conventional taxanes is de novo or acquired resistance to taxanes. Therefore, novel experimental taxane derivatives (Stony Brook taxanes; SB-Ts) are synthesized and tested as potential drugs against resistant solid tumors. Recently, we reported alterations in ABCC3, CPS1, and TRIP6 gene expression in a breast cancer cell line resistant to paclitaxel. The present study aimed to investigate gene expression changes of these three candidate molecules in the highly resistant ovarian carcinoma cells in vitro and corresponding in vivo models treated with paclitaxel and new experimental Stony Brook taxanes of the third generation (SB-T-121605 and SB-T-121606). We also addressed their prognostic meaning in ovarian carcinoma patients treated with taxanes. We estimated and observed changes in mRNA and protein profiles of ABCC3, CPS1, and TRIP6 in resistant and sensitive ovarian cancer cells and after the treatment of resistant ovarian cancer models with paclitaxel and Stony Brook taxanes in vitro and in vivo. Combining Stony Brook taxanes with paclitaxel caused downregulation of CPS1 in the paclitaxel-resistant mouse xenograft tumor model in vivo. Moreover, CPS1 overexpression seems to play a role of a prognostic biomarker of epithelial ovarian carcinoma patients' poor survival. ABCC3 was overexpressed in EOC tumors, but after the treatment with taxanes, its up-regulation disappeared. Based on our results, we can suggest ABCC3 and CPS1 for further investigations as potential therapeutic targets in human cancers.

Lékové, drug-eluting balonkové katétry umožňují lokální aplikaci účinné antiproliferativní látky, která omezuje neointimální hyperplazii v cévní stěně. Vzhledem ke snadné vazbě na povrch balonkového katétru a rychlému průniku do tkání bývá jako účinná látka využíván především paclitaxel. Rozhodující roli hraje způsob vazby paclitaxelu na povrch balonkového katétru. Nejčastěji je používán iopromid, který zvyšuje rozpustnost paclitaxelu a jeho permeabilitu. Iopromid-paclitaxelové balonkové katétry prokázaly svoji efektivitu v léčbě in-stent restenóz především v holých kovových stentech. Další oblastí využití by mohla být léčba stenóz malých koronárních tepen.
Drug-eluting balloon catheters allow local delivery into the vessel wall of an effective antiproliferative agent which reduces neointimal hyperplasia. Given its capacity to easily bind to the surface of the balloon catheter and rapidly penetrate into tissues, paclitaxel is mainly used as the active substance. The method of paclitaxel binding to the surface of the balloon catheter plays a crucial role. Iopromide is most commonly used as a carrier which increases the solubility of paclitaxel and its penetration into tissues. Iopromide-paclitaxel balloon catheters have been shown to be effective in the treatment of in-stent restenosis, particularly in bare metal stents. Another area of application could be the treatment of small coronary artery stenoses.

V naší kazuistice prezentujeme pacientku s BRCA pozitivním karcinomem ovaria, která byla diagnostikována ve IV. stadiu onemocnění a podstoupila léčebné modality, které byly přístupné. Kazuistikou jsme chtěli zdůraznit význam multidisciplinárního týmu, nutnost genetického vyšetření při rozhodování o léčbě, a také vliv antiangiogenní a cílené léčby na přežití pacientky pří dobré kvalitě života.
In our case report we present a patient with BRCA-positive ovarian cancer, who was diagnosed with stage IV and underwent treatment modalities which were accessible. In the case report, we wanted to emphasize the importance of a multidisciplinary team, the need for genetic testing in treatment decisions, as well as the impact of antiangiogenic and targeted treatment on the patienťs survival in a good quality of life.
