Karcinom pankreatu je onemocnění s vysokou mortalitou, často zachycené v pozdním stadiu. Jedinou kurativní léčbou je chirurgická resekce, výsledky zlepšuje adjuvantní chemoterapie. U pacientů s pokročilým nálezem je v 1. linii paliativní léčby doporučen režim FOLFIRINOX nebo doublet gemcitabin + nab-paklitaxel, ve 2. linii pak po terapii založené na gemcitabinu nal-IRI (pegylovaný lipozomálnî irinotekan) + 5-fluorouracil (5-FU) + leukovorin (LV), nebo režim s 5-FU, a po terapii s 5-FU gemcitabin v monoterapii či v kombinaci. Nové výsledky prezentované na kongresu ESMO 2023 ukázaly, že v 1. linii paliativní léčby je větším přínosem doublet gemcitabin + nab-paklitaxel než FOLFIRINOX. Prezentujeme kazuistiku 50letého muže s progresí karcinomu pankreatu po chirurgické a adjuvantní léčbě, u něhož bylo po selhání gemcitabinu + nab-paklitaxelu v 1. linii paliativní léčby dosaženo již ročního přežití s dobrou kvalitou života při kombinaci nal-IRI + 5-FU/LV.
Pancreatic cancer is a disease with high mortality rates, often detected at late stage. Surgical resection is the only curative treatment, and the outcomes are improved by adjuvant chemotherapy. In patients with an advanced finding, the FOLFIRINOX regimen or the doublet gemcitabine + nab-paclitaxel are recommended in the first-line of palliative treatment; in the second-line, following the gemcitabine-based treatment, nal-IRI (pegylated liposomal irinotecan) + 5-fluorouracil (5-FU) + leucovorin (LV), or the regimen with 5-FU are recommended; and following the therapy with 5-FU, gemcitabine in monotherapy or in combination. New results presented at the 2023 ESMO Congress showed that, in the first-line of palliative treatment, the doublet gemcitabine + nab-paclitaxel had a greater benefit than the FOLFIRINOX regimen. We report a case of a 50-year-old man with progression of pancreatic cancer after surgical and adjuvant treatment in whom, after failure of gemcitabine + nab-paclitaxel in the first-line of palliative treatment, one-year survival with a good quality of life was achieved with the combination of nal-IRI + 5-FU/LV.

BACKGROUND: Preoperative FOLFIRINOX chemotherapy is increasingly administered to patients with borderline resectable (BRPC) and locally advanced pancreatic cancer (LAPC) to improve overall survival (OS). Multicenter studies reporting on the impact from the number of preoperative cycles and the use of adjuvant chemotherapy in relation to outcomes in this setting are lacking. This study aimed to assess the outcome of pancreatectomy after preoperative FOLFIRINOX, including predictors of OS. METHODS: This international multicenter retrospective cohort study included patients from 31 centers in 19 European countries and the United States undergoing pancreatectomy after preoperative FOLFIRINOX chemotherapy (2012-2016). The primary end point was OS from diagnosis. Survival was assessed using Kaplan-Meier analysis and Cox regression. RESULTS: The study included 423 patients who underwent pancreatectomy after a median of six (IQR 5-8) preoperative cycles of FOLFIRINOX. Postoperative major morbidity occurred for 88 (20.8%) patients and 90-day mortality for 12 (2.8%) patients. An R0 resection was achieved for 243 (57.4%) patients, and 259 (61.2%) patients received adjuvant chemotherapy. The median OS was 38 months (95% confidence interval [CI] 34-42 months) for BRPC and 33 months (95% CI 27-45 months) for LAPC. Overall survival was significantly associated with R0 resection (hazard ratio [HR] 1.63; 95% CI 1.20-2.20) and tumor differentiation (HR 1.43; 95% CI 1.08-1.91). Neither the number of preoperative chemotherapy cycles nor the use adjuvant chemotherapy was associated with OS. CONCLUSIONS: This international multicenter study found that pancreatectomy after FOLFIRINOX chemotherapy is associated with favorable outcomes for patients with BRPC and those with LAPC. Future studies should confirm that the number of neoadjuvant cycles and the use adjuvant chemotherapy have no relation to OS after resection.

BACKGROUND: Pancreatic ductal adenocarcinoma remains one of the most lethal malignancies, with few treatment options. NAPOLI 3 aimed to compare the efficacy and safety of NALIRIFOX versus nab-paclitaxel and gemcitabine as first-line therapy for metastatic pancreatic ductal adenocarcinoma (mPDAC). METHODS: NAPOLI 3 was a randomised, open-label, phase 3 study conducted at 187 community and academic sites in 18 countries worldwide across Europe, North America, South America, Asia, and Australia. Patients with mPDAC and Eastern Cooperative Oncology Group performance status score 0 or 1 were randomly assigned (1:1) to receive NALIRIFOX (liposomal irinotecan 50 mg/m2, oxaliplatin 60 mg/m2, leucovorin 400 mg/m2, and fluorouracil 2400 mg/m2, administered sequentially as a continuous intravenous infusion over 46 h) on days 1 and 15 of a 28-day cycle or nab-paclitaxel 125 mg/m2 and gemcitabine 1000 mg/m2, administered intravenously, on days 1, 8, and 15 of a 28-day cycle. Balanced block randomisation was stratified by geographical region, performance status, and liver metastases, managed through an interactive web response system. The primary endpoint was overall survival in the intention-to-treat population, evaluated when at least 543 events were observed across the two treatment groups. Safety was evaluated in all patients who received at least one dose of study treatment. This completed trial is registered with ClinicalTrials.gov, NCT04083235. FINDINGS: Between Feb 19, 2020 and Aug 17, 2021, 770 patients were randomly assigned (NALIRIFOX, 383; nab-paclitaxel-gemcitabine, 387; median follow-up 16·1 months [IQR 13·4-19·1]). Median overall survival was 11·1 months (95% CI 10·0-12·1) with NALIRIFOX versus 9·2 months (8·3-10·6) with nab-paclitaxel-gemcitabine (hazard ratio 0·83; 95% CI 0·70-0·99; p=0·036). Grade 3 or higher treatment-emergent adverse events occurred in 322 (87%) of 370 patients receiving NALIRIFOX and 326 (86%) of 379 patients receiving nab-paclitaxel-gemcitabine; treatment-related deaths occurred in six (2%) patients in the NALIRIFOX group and eight (2%) patients in the nab-paclitaxel-gemcitabine group. INTERPRETATION: Our findings support use of the NALIRIFOX regimen as a possible reference regimen for first-line treatment of mPDAC. FUNDING: Ipsen. Chirurgie zůstává nejdůležitější modalitou v léčbě resekabilního karcinomu pankreatu, avšak pouze 10-15 % pacientů je vhodnými kandidáty. I přes stále vysoký podíl lokálních recidiv po radikální operaci byla adjuvantní radioterapie v současnosti nahrazena chemoterapií, která má dle randomizovaných studií lepší výsledky. V neoadjuvantní indikaci se také preferuje spíše léčba systémová, ale další studie zkoumající postavení radioterapie u této skupiny pacientů jsou nutné. Radioterapie má místo u neresekabilního onemocnění, kde v kombinaci se systémovou léčbou ovlivňuje lokální kontrolu onemocnění i celkové přežití. Preferovány jsou nové techniky radioterapie včetně hypofrakcionované ablativní a stereotaktické radioterapie. U metastatického onemocnění lze také zvažovat ozáření metastáz s paliativním záměrem nebo s cílem oddálit progresi onemocnění a zlepšit přežití.
Surgery remains the most important modality in the treatment of resectable pancreatic cancer, but only 10-15% of patients are suitable candidates. Despite the still high rate of local recurrence after radical surgery, adjuvant radiotherapy has now been replaced by chemotherapy, which has been shown to have better outcomes in randomized trials. Systemic therapy is also preferred in the neoadjuvant indication, but further studies investigating the position of radiotherapy in this group of patients are needed. Radiotherapy has a place in unresectable disease, where in combination with systemic therapy it affects local disease control and overall survival. New radiotherapy techniques including hypofractionated ablative and stereotactic radiotherapy are preferred. In metastatic disease, radiation to metastases may also be considered with palliative intent or to delay disease progression and improve survival. The use of new technologies for imaging and radiotherapy planning continues to evolve, but the combination of radiotherapy with more effective systemic therapy is also being used more.

Kazuistika popisuje případ pacienta s karcinomem pankreatu, který po operaci a adjuvantní terapii časně progredoval do metastatického stadia. Paliativní systémová léčba v rámci druhé linie léčby metastatického onemocnění vedla k dlouhodobé kontrole onemocnění s následnou metastasektomií reziduálních plicních ložisek. Při časném relapsu po resekci metastáz onemocnění byl pacient léčen s ohledem na výsledky NGS vyšetření platinovým derivátem. Personalizovaná terapie vedla k časné odpovědi a opětovné kontrole onemocnění. V budoucnu je u pacienta v plánu maintenance terapie PARP inhibitory.
We present a case report of a patient with pancreatic cancer who developed metastatic disease early after surgery and adjuvant therapy. Palliative systemic treatment resulted in long-term control of the disease, followed by pulmonary metastasectomy of residual lesions. Early relapse of the disease after metastasectomy was treated with platinum derivatives, based on the results of NGS testing. Personalized treatment has led to a rapid response and stabilization of the disease. Maintenance therapy with PARP inhibitors will follow in the future.

V době primární diagnózy je výskyt metastatického stadia duktálního adenokarcinomu pankreatu (mPDAC) až v 60 %. U těchto pacientů jsou přítomny přibližně v 1/2 synchronní vzdálené metastázy, z toho asi ve 37-42 % jaterní. Dosavadní doporučení ve IV. stadiu onemocnění neindikovala primární chirurgický výkon. V poslední době, zejména s rozvojem systémové terapie, jsou publikovány prvotní výsledky, které dokládají přínos primárního chirurgického přístupu. Dosud nemáme výsledky probíhajících randomizovaných studií. Proto nemáme dostatečně silnou evidenci k doporučení změny strategie u mPDAC. Primární chirurgický výkon má být dosud umožněn jen v rámci klinických studií v hepatopankreatobiliárních centrech.
At the time of primary diagnosis, metastatic pancreatic ductal adenocarcinoma (mPDAC) occurs in up to 60%. In approximately half of these patients, synchronous distant metastases are present, with around 37-42% of them being liver metastases. The existing recommendations do not indicate a primary surgical procedure in stage IV of the disease. Recently, particularly with the advancement of systemic treatment, initial results have been published demonstrating the benefit of the primary surgical approach. The results of ongoing randomized trials have been unavailable so far; therefore, there is not strong enough evidence to recommend a change in the strategy for mPDAC. Primary surgical procedure is currently allowed only as part of clinical trials at hepatopancreatobiliary centres.

Karcinom pankreatu je onemocnění s rostoucí incidencí a mortalitou celosvětově. Současným standardem terapie je systémová chemoterapie bez ohledu na stadium onemocnění. I když kombinované režimy v adjuvanci, neoadjuvanci a paliaci vedly k prodloužení přežití, je karcinom pankreatu stále spojený s nejkratší dobou přežití v rámci onkologických diagnóz. Synonymem pro současnou onkologii je personalizovaná/precizní onkologie, kdy zásluhou lepší dostupnosti NGS jsme schopni identifikovat specifické podskupiny pacientů s karcinomem pankreatu, které mají jednak odlišnou prognózu, jednak vyžadují specifický přístup k léčbě. Cílem tohoto přehledu je podat informace o karcinomu pankreatu jako o heterogenním onemocnění, které vyžaduje personalizovaný přístup. Největší část je věnovaná podskupině s defekty v systému DDR/HRD vzhledem k poměrně vysoké frekvenci výskytu a možnosti víceméně personalizované terapie platinovými deriváty s ev. následnou maintenance terapií PARP inhibitory. Menší molekulárně definované podskupiny jsou diskutovány stručně i s ohledem na menší množství dat.
Pancreatic cancer is a disease with increasing incidence and mortality worldwide. Current standard of treatment is chemotherapy regardless of clinical stage. Despite improvements in survival thanks to multi-agent regimens in adjuvant, neoadjuvant and paliative setting, pancreatic cancer still has the worst prognosis across all malignancies. Oncology nowadays is more or less personalized/precise. With help of NGS we are able to identify specific subtypes of patients with pancreatic cancer, which have both, different prognosis and require specific approach to treatment. The aim of this review is to provide information about pancreatic cancer as a molecularly heterogeneous disease, which require personalized approach. Main part of review is about pancreatic cancer with defects in DDR/HRD system with an option for more or less personalized therapy with platinum derivatives, followed in some cases with maintenance with PARP inhibitors. Smaller molecularly defined subgroups are described briefly, because of smaller amount of data from the literature.
Despite constant advances in cancer research, the treatment of pancreatic adenocarcinoma remains extremely challenging. The intratumoral immunotherapy approach that was developed by our research group and was based on a combination of mannan-BAM, TLR ligands, and anti-CD40 antibody (MBTA) showed promising therapeutic effects in various murine tumor models, including a pancreatic adenocarcinoma model (Panc02). However, the efficacy of MBTA therapy in the Panc02 model was negatively correlated with tumor size at the time of therapy initiation. Here, we aimed to further improve the outcome of MBTA therapy in the Panc02 model using the glutamine antagonist 6-diazo-5-oxo-L-norleucine (DON). The combination of intratumoral MBTA therapy and intraperitoneal administration of DON resulted in the complete elimination of advanced Panc02 subcutaneous tumors (140.8 ± 46.8 mm3) in 50% of treated animals and was followed by development of long-term immune memory. In the bilateral Panc02 subcutaneous tumor model, we observed a significant reduction in tumor growth in both tumors as well as prolonged survival of treated animals. The appropriate timing and method of administration of DON were also addressed to maximize its therapeutic effects and minimize its side effects. In summary, our findings demonstrate that the intraperitoneal application of DON significantly improves the efficacy of intratumoral MBTA therapy in both advanced and bilateral Panc02 subcutaneous tumor murine models.
