Předkládáme kazuistiku mladé ženy s komplikovaným průběhem idiopatického střevního zánětu s těžkým luminálním a perianálním postižením.
We present a case report of a young female patient with complicated course of inflammatory bowel disease with severe luminal, as well as perianal symptoms. The patient was initially diagnosed with ulcerative colitis and underwent proctocolectomy with ileopouch-anal anastomosis. One year after oostomy closure she got pregnant and, at the same time, she developed perianal abscess and fistula. The diagnosis was thus changed to Crohn’s disease. During pregnancy, she underwent repeated perianal fistula drainages, surgical revision of ileus and treatment with ustekinumab was started. The patient delivered a healthy boy in November 2022 at 38th gestational week.

Diagnózu sme stanovili detekciou traktu fistuly fyzikálnym vyšetrením, anoskopiou, sondážou traktu fistuly a endorektálnou ultrasonografiou. Komplexný operačný výkon s konštrukciou posuvného endorektálneho laloku podľa nášho postupu zachováva funkciu sfinkterov a má relatívne nízke percento recidív. Počet predchádzajúcich operačných intervencií nemal žiadny vplyv na úroveň análnej kontinencie.
Introduction: Anal fistula is a common disease with incidence of 5.6 per 100,000 women and 12.3 men. It is most often of cryptoglandular origin. The aim of this study is to evaluate our experience with patients treated for complex anal fistula with our own complex surgical procedure with advancement endorectal flap. Methods: 524 patients with complex anal fistulas who were sent to our surgical clinic from January 2005 to the end of December 2022 were in- cluded in the study. We established the diagnosis by detecting the fistula tract by physical examination, anoscopy, probing the fistula tract and endorectal ultrasonography. We performed a complex operation together with the construction of the advancement endorectal flap in our own modification for all patients. Results: Primary surgical intervention in a group of 326 patients with complex anal fistulas (excluding patients with Crohn‘s disease) was successful in 283 (87%) patients. We identified advancement endorectal flap defect in the postoperative period in 17 (5.2%) patients, soiling in 16 (4.9%) and flatus incontinence in 9 (2.7%) patients. In a group of 120 patients after multiple surgeries (excluding patients with Crohn‘s disease), our surgical procedure was successful in 92 (76.6%) patients. In the postoperative period, we identified a advancement endorectal flap defect in 6 (5%) patients, soiling in 8 (6.6%) and flatus incontinence in 3 (2.5%) patients. Conclusions: The construction of the advacement endorectal flap was curative and without affecting the level of anal continence in 87% of patients after primary surgical intervention and in 76.6% after multiple surgical procedures. Complex surgery with the construction of the advancement endorectal flap according to our procedure preserves the function of the sphincters and has a relatively low percentage of recurrences. The number of previous surgical interventions had no affect on the level of anal continence.

Contrast fistulogram (FG) and distal pressure colostogram (DPCG) are standard diagnostic methods for the assessment of anorectal malformations. Pelvic magnetic resonance imaging (MRI) earned a place among essential diagnostic methods in preoperative investigations after the Currarino syndrome and a high incidence of associated spinal dysraphism were described. The aim of our study was to evaluate the possibility of substituting FG and DPCG by a modified pelvic MRI, e.g. MRI fistulogram (MRI-FG) and MRI colostogram (MRI-DPCG). The prospective study involved 29 patients with anorectal malformations who underwent a modified pelvic MRI. The length and course of fistulas and rectum, and the presence of sacral anomalies were studied on MRI images and compared with images obtained by radiologic examinations. Modified MRI brought identical results as contrast studies in 25 patients when related to the fistula and rectum length and course. MRI was more accurate for the detection of sacral anomalies. MRI-FG was the only imaging method used in the four most recent patients. The results support the assumption that conventional contrast examinations for the assessment of anorectal malformations can be replaced by MRI, thus reducing the radiation dose.

S využitím současných léků, především biologik, a správné chirurgické léčby lze dosáhnout dlouhodobého zhojení u více než 2/3 nemocných s perianální formou Crohnovy choroby.
Perianal fistulas affect more than one third of patients suffering from Crohn's disease. They represent severe phenotype of the disease and substantially decrease the quality of life in affected patients. Therapy requires a complex approach with combination and appropriate coordination of medical treatment and surgical techniques. Using the current medicines, especially biologics, as well as propper surgery, we can achieve the long-term healing in more than 2/3 of patients with perianal Crohn's disease.

Authors examined the results of surgical treatment for 123 breast age patients with Acute Paraproctitis (AP) being cured for the last 10 years. Was offered the less invasive, less traumatic, defensive way of fistulectomy at AP treatment in breast fed infants, reducing possible relapse up to minimum. The suggested approach provides full desquamation and removal of fistula channel epithelium lining rests, and therefore the rectum sphincter is not damaged and there is no much damage in mucus. Examining with scanning electron microscopy (SEM) of micro drainages used for micro sanitation in the form of nylon line with knots, has found that their use contributes to removal from the fistula cavity not only detritus, fibrin and desquamated cells, but pathogen content in the form of microorganisms such as sticks, and structures fungi. It was determined that it is necessary to use not only antibacterial and anti fungous therapy. The advantage of the new operational way over the traditional approach is prevention of wound reinfection. Of the 52 breast patients, 51 obtained results of treatment were quite good in the near and distant periods of illness. Application of minimally invasive fistulocriptoectomy at AP infants maximizes the improvement of the results of surgical treatment, and allows to minimize complications.
