The IHA test was evaluated in the diagnosis of amoebiasis. Axenically-grown E. histolytica was used as an antigen. A total of 427 sera from symptomatic (intestinal and extra-intestinal) amoebiasis patients. asymptomatic carriers, patients with parasitic intestinal infections other than E. histolytica, and healthy controls were tested. From 288 symptomatic cases of amoebiasis, 232 (80.6 per cent) gave positive reactions. In 93 asymptomatic cases of amoebiasis, 55 (59.1 percent) were seropositive. In testing of sera from 16 subjects with parasitic intestinal infections other than E. histolytica, a low-level positive IHA titres occurred in 2 (12.5 per cent). The test was also positive with a low titre in 3 (10.0 per cent) of the 30 subjects of the healthy control group. The results indicate that the IHA test is of value in the confirmation of intestinal and extra-intestinal amoebic infections especially in cases where the parasite is difficult to demonstrate.

A total of 2,883 foreign students at the age of 18-30 years were examined for amoebiasis after their arrival to Czechoslovakia. Stool examinations revealed the presence of Entamoeba histolytica in 112 of them (3.9%). Students from 38 countries were found to be infected with this parasite. In a set of 2,064 students from these countries E. histolytica prevalence in stool was 5.4%. There were greater differences in the prevalence between individual countries inside a geographical region than between individual geographical regions. The highest E. histolytica prevalence in stool was found in students from tropical and southern Africa (6.7% of 745 examined) and the lowest in students from South-eastern Asia (3.1% of 321 examined). In a simple cross-section study, antibodies against E. histolytica were detected by enzyme-linked immunosorbent assay (ELISA) in the sera of 1,001 persons. Antibodies were detected in 7.9% of students at the following titres: 1:200 in 4.5%, 1:600 in 1.5%, 1:1,800 in 1.9%. Antibodies occurred more frequently in students carrying E. histolytica cysts (X2 = 14.9). Titre of ELISA antibodies in patients with confirmed liver abscess was higher than 1:1,800. counterimmunoelectrophoresis (CIEP) test was used for serum examinations of patients who had been demonstrated by ELISA to be seropositive and of those carrying E. histolytica cysts. In a set of 170 patients CIEP antibodies were also more frequent in those carrying E. histolytica cysts (X2 = 26.95). A comparison of the results of ELISA and CIEP tests in the same patients revealed that CIEP antibodies were more dependent on the actual parasitization with E. histolytica than ELISA antibodies.(ABSTRACT TRUNCATED AT 250 WORDS)

Very few studies have been carried out on the role of liver macrophages (Kupffer cells) during the course of hepatic amoebiasis. The kinetics of phagocytic activity of Kupffer cells and blood monocytes was studied in guinea pigs intra-mesenterically infested with Entamoeba histolytica. The phagocytic capacity of blood monocytes of normal animals was comparatively lower than Kupffer cells for both latex and haemolysin coated sheep red blood cells. Significant decline in phagocytic response of Kupffer cells and blood monocytes of infected animals was observed right from 2nd post infection day and it kept on decreasing with the progress of infection. Depression in phagocytic response of Kupffer cells and blood monocytes was more marked in those animals who had higher grades of pathological lesions. Hence, an inverse correlation was obtained between the phagocytic capacity and severity of amoebic lesions (P less than 0.01). The significance of depression in phagocytic response of Kupffer cells and blood monocytes may be responsible for the development of hepatic lesions.

Among 72 patients clinically suspected of Entamoeba histolytica (E. histolytica) infections, 39 positive cases (54%) were detected serologically by the indirect hemagglutination (IHA) test. Parasitologically, microscopic examination of three consecutive stool specimens from all these patients indicated positivity for E. histolytica cysts and or trophozoites in 10 of the patients with IHA antibody titers greater than or equal to 1:128, which is of clinical significance. Another 2 patients were parasitologically positive but showed low IHA antibody titres (1:32-1:64); follow up indicated response to treatment with metronidazole. The highest serological positivity (100%) were detected in patients with liver abscess, all were clinically proven cases of extra-intestinal amoebiasis. IHA antibody levels of clinical significance were seen in all four patients with chronic dysentery with parasitological evidence of E. histolytica in their stools. In a group of patients with abdominal pain nine positives were detected serologically, four of which were positively diagnosed concurrently by parasitology; the remaining five patient's sera showed high IHA antibody titres with absence of cysts or trophozoites in stools, indicative possibly of persistence of antibodies from past infection. The serologic determination of E. histolytica IHA antibodies in a control group consisting of normal healthy school children and adults of both sexes without any clinical evidence of amoebiasis showed the absence of any positive titres of clinical significance; low titres (1:32-1:64) were detected in 5.2% of 232 sera tested. Parasitological examination of three consecutive stool specimens from all individuals in the control group showed the presence of cysts of E. histolytica in just two among 232 tested (0.9%).

Results of histopathological examination of a solitary chronical amoebic abscess in the liver of a patient from Cambodia are described. The abscess was at a proliferously reparative phase and Entamoeba histolytica was detected at the border between the necrosis and inner wall of abscess, as well as in the proper non-specific granulation tissue of the inner layers of abscess capsule. A differential diagnosis of the amoebic abscess from similar parasitary or pseudoparasitary liver lesions and differential diagnosis of amoebae in histological sections are given. For orientation examinations of the liver tissue for the presence of amoebae the authors recommend the impregnation after Grocott and staining with Goldner's trichrome for a more detailed evaluation of histological sections.

A study was conducted among 2 500 secondary school students in Calabar, in order to determine the incidence of amoebiasis caused by Entamoeba histolytica. Microscopic examination of faecal sample smears showed higher percentage infections in the males than in the females; 61 out of 1 250 males were infected (4.8%), while 52 of 1 250 females were infected (4.1%). Among infected persons, asymptomatic carriers (64.6%) and symptomatic cases (35.4%) were recorded. Infection was lower among students using water flush toilet facilities than those using pit and bucket latrines. The infection was attributed to poor sanitation and inadequate health facilities.

The results of intracaecal inoculation experiments on young albino rats with different strains of E. histolytica isolated in cultures from patients with different varieties of clinical amoebiasis, such as acute amoebic dysentery, amoebic liver abscess and chronic amoebic colitis, as well as from asymptomatic infections, have been presented and discussed. Caecal scoring was performed with regard to the condition of the wall of the caecum and the contents of its lumen. Successful infection with virulent amoebae was associated with the presence of considerable ulceration of the caecum, whereas, in infections with avirulent amoebae, the changes were apparently only slight or absent. The virulence and pathogenicity were considerably different for clinical case and symptomless carrier strains of E. histolytica. In the former group, high caecal scores and pathogenicity indices with ulceration of the caecum were noted, whilst in the latter, the amoebae were either virulent or avirulent. Results similar to those for the former group were obtained with two carrier strains only.

The authors present a case of a young male, who suffered with extraintestinal form of amoebiasis-amoebic liver abscesses. The patient traveled to India two month before symptoms onset. The diagnosis based on ultrasonography and computed tomography was definetely confirmed by serological examination. Metronidazol treatment was given initially, followed by percutaneous drainage. Open surgical tretment was indicated due to failure of percutaneous treatment. Patient was discharged home in a good condition one month after surgery. At this time patient is doing well eight months after surgery.

Among 10,418 patients of a Prague hospital, a plain infection with intestinal parasitic protozoans was identified in 1,319 persons (12.7%). Of these, 3.5% were infested with Giardia intestinalis, 0.3% with Entamoeba histolytica forma minuta, 5.7% with Endolimax nana. We evaluated the frequency of findings of protozoans in various clinical diagnoses. A statistically significant increase in frequency was recorded for E. nana in diagnoses of eosinophilia, giardiasis, amoebiasis and helminthiasis. A slight increase above the average was found for Entamoeba coli in diagnoses of giardiasis and helminthiasis. Most cases of infection with Entamoeba histolytica were associated with a stay abroad. No increase in the frequency of these protozoans was recorded for patients with diarrhea. An analysis of the results indicated that a nonpathogenic amoeba might participate in the origin of intestinal disorders in man.
