The main intention of the project is to search for the differences in expression of genes of metabolism and transport, DNA repair and cell cycle genes between tumour tissues of patients and tissues of patients with bad response to the standard CLM chemotherapy. The partial aim will be to indentify prospectively, in patients with CLM, the pharmacogenomic profile of good response to the chemotherapy. Another aim of the project is to validate retrospectively the pharmacogenomic profile as it was identifiedin the prospective part, which is in connection with good response to the administered chemotherapy in group of patients with unresectable CLM. In case, that significant differences between patients are found, i.e. genes responsible for the different sensitivity to the therapy are identified, mechanism of sensitivity will be studied.

Celkové přežití nemocných s neoperabilními nebo paliativně ošetřenými metastázami kolorektálního karcinomu (CLM) je i přes následnou chemoterapii stále neuspokojivé. Hlavním záměrem předkládaného projektu je hledat rozdíly v expresi genů metabolismu, transportu, opravy DNA a buněčného cyklu mezi nádorovými tkáněmi pacientů dobře odpovídajících a tkáněmi pacientů špatně odpovídajících na standardní chemoterapii. Naším cílem je také prospektivně identifikovat, u nemocných s CLM, farmakogenomický profil dobré odpovědi na chemoterapii. Tyto předpokládané výsledky mají vést k racionalizaci a úpravě onkologické léčby s cílem maximální možné odpovědi, která by umožnila zvýšení počtu nemocných s úspěšným onkologickým downstagingem onemocnění.
