Chirurgická revaskularizace myokardu časně po akutním infarktu myokardu léčeném trombolýzou
[Early surgical myocardial revascularization after acute myocardial infarction treated with thrombolysis (case history)]

Je uveden případ pacienta s akutním infarktem myokardu (AIM) přední stěny při mnohočetném těžkém katetrizačně neřešitelném postižení věnčitých tepen, který po iniciální léčbě i. v. trombolýzou na spádovém interním oddělení podstoupil pro recidivu stenokardií s rozvojem těžké levostranné srdeční insuficience urgentní chirurgickou revaskularizaci myokardu a zemřel časně po operaci na krvácivé komplikace, které však nebyly důsledkem předchozí trombolytické léčby. Je podán stručný přehled literatury, týkající se časné chirurgické revaskularizace myokardu po trombolytické léčbě AIM.

The authors present the case of a patient with acute anterior myocardial infarction with multiple severe impairment of his coronary arteries, which was insolvable by percutaneous coronary intervention. He was treated initially by i. v. thrombolysis which was administered in the Department of Medicine of a Community hospital. After recurrence of his angina, which was accompanied by severe left heart failure, he underwent urgent surgical myocardial revascularization, but succumbed early following the operation because of haemorrhagic complications, which were not associated with the previous thrombolysis. A brief review of the literature concerning early coronary artery bypass graft surgery after thrombolytic therapy of acute MI is presented.
