Kazuistika popisuje léčbu diabetika 2. typu, u kterého po stanovení diagnózy došlo poměrně časně k selhání účinku kombinace perorálních antidiabetik, a kde bylo již plně indikováno zahájení léčby inzulínem. Dle přání pacienta byla léčba zahájena premixovaným inzulínem a tato léčba byla intenziᜀkována dle vývoje nemoci. Pacient je dlouhodobě dobře kompenzován a je se svou léčbou spokojen. Léčba není provázená přírůstkem tělesné hmotnosti ani výskytem hypoglykemií. V tomto článku je diskutován výběr léčby premixovaným inzulínem, jeho přednosti a limitace a také výsledky klinických studií.

The case report describes the treatment of a patient with the type 2 diabetes mellitus, in whom the effect of combined oral antidiabetic agents failed quite early after the diagnosis and the insulin therapy was fully justified. Based on patient's wish the treatment with premixed insulin was initiated and this therapy was intensified according to a progression of the disease. The patient is well controlled for a long time and he is satisfied with this treatment. The treatment is accompanied neither by weight gain nor hypoglycemic episodes. The choice of the treatment with premixed insulin, its benefits and limitations and results of clinical trials are discussed in this paper.
