Doplnění digitální chromoendoskopie o validované klasifikace umožňuje endoskopistům in vivo přesně předpovědět histologii kolorektálních polypů. Znalost biologické povahy polypů bez jejich histologického vyšetření nabízí nové strategie, jako je „resect and discard“ a „observe and leave“. Tyto strategie přenášejí zodpovědnost za správnou diagnózu z patologa na vyšetřujícího, ale na oplátku racionalizují praxi v prevenci kolorektálního karcinomu. Autoři shrnují argumenty pro přijmutí těchto nových endoskopických strategií a podmínky, za kterých je možné jejich rozšíření do běžné endoskopické praxe. Digital chromoendoscopy used with validated classification scales allows endoscopists to predict colorectal polyp histology in vivo with a high degree of precision. The knowledge of the polyp histology without the need for biopsy offers endoscopists new strategies such as “resect and discard” and “observe and leave”. Although these strategies transfer the responsibility for correct diagnosis from the pathologist to the endoscopist, they allow us to rationalise the prevention of colorectal cancer. The authors summarise the arguments for accepting these new strategies and specify the requirements for their implementation into daily endoscopic practice. The cited international studies are complemented with data from our institution.
