Chronická neinfekční onemocnění tvoří ve vyspělých zemích značnou část nákladů na zdravotní péči. Porozumění adherenci a faktorům, které ji ovlivňují, může mít významně pozitivní dopady na rozpočet zdravotního systému. Hlavním cílem této studie bylo zjistit, jaké parametry mají vliv na adherenci pacientů k biologické anti-TNF léčbě ve třech diagnózách – revmatoidní artritida, Crohnova nemoc a psoriáza. Byla prokázána souvislost mezi postoji pacientů k léčbě a jejich jistotou, že své léky užívají správně. S klesající jistotou v užívání léků rostou obavy z nežádoucích účinků, a naproti tomu s rostoucí jistotou narůstá i pocit potřeby užívání léčiv. Pacienti, kteří jsou si více jistí tím, jak mají svá léčiva správně užívat, jsou více adherentní.

Chronic non-infectious diseases account for considerable proportion of the entire healthcare cost in developed countries. Understanding adherence and the factors that influence it might have significant positive effects on healthcare system budget. The main objective of the present study was to find out about the parameters influencing patients adherence to biological anti-TNF therapy administered for three diagnoses – rheumatoid arthritis, Crohn's disease, and psoriasis. The study succeeded in demonstrating association between patients attitude to therapy and their certainty that they used their drugs correctly. Declining certainty regarding the use of drugs is accompanied by growing concerns about undesirable effects, while we also found that growing certainty results in growing need to use the drugs. Patients who are more sure about the correct use of their drugs exhibit higher adherence.
