Xultophy 100 jednotek/ml + 3,6 mg/ml je injekční antidiabetikum – kombinace inzulínu degludek a liraglutidu. Liraglutid je dlouhodobý analog GLP-1 (glucagon-like peptide-1, glukagonu podobný peptid 1) určený k subkutánní aplikaci jednou denně stejně jako dlouhodobý inzulínový analog degludek. Kombinace obou léčiv ve fixním dávkovacím poměru (jedna dávkovací jednotka obsahuje 1 jednotku inzulínu degludek a 0,036 mg liraglutidu) podávané jedním aplikátorem představuje jednoduchou a bezpečnou možnost intenzifikace inzulínové léčby v situaci, kdy selhává léčba bazálním inzulínem. Prezentovaná kazuistika dokumentuje s využitím profesionální kontinuální monitorace glukózy pomocí senzoru účinnou a bezpečnou intenzifikaci léčby převodem na Xultophy u pacientky s diabetem 2. typu dosud léčené kombinací perorálních antidiabetik a inzulínu.

Xultophy 100 U/mL+3.6 mg/mL is an injectable antidiabetic agent – a combination of insulin degludec and liraglutide. Liraglutide is a long-acting GLP-1 (glucagon-like peptide-1) analogue intended for subcutaneous once-daily administration, the same as the long-acting insulin analogue degludec. The combination of both preparations in a fixed dose ratio (one dosage unit contains 1 unit of insulin degludec and 0.036 mg of liraglutide) administered by a single applicator provides a simple and safe way to intensify insulin treatment in the case that basal insulin treatment fails. The presented case report documents the effective and safe intensification of treatment by conversion to Xultophy, using professional continuous glucose monitoring with a sensor, in a patient with type 2 diabetes previously treated with a combination of oral antidiabetics and insulin glargine 100 U/mL.
