Pokročilý, tedy metastazující nebo inoperabilní hormonálně pozitivní karcinom prsu s negativitou receptoru 2 pro lidský epidermální růstový faktor (human epidermal growth factor receptor 2, HER2), je nevyléčitelné onemocnění, u kterého máme k dispozici řadu terapeutických přístupů. V tomto článku se zaměříme na subpopulaci pacientek s primární nebo sekundární hormonální rezistencí a volbu první linie paliativní terapie inhibitory cyklin dependentní kinázy 4/6 (cyclin-dependent kinase 4/6, CDK4/6), což bude prezentováno i na konkrétní kazuistice. Z uspořádání, výsledků a analýz podskupin z klinických studií MONARCH-2, MONALEESA-3 a PALOMA 3 aktuálně vyplývá, že nejrobustnější data pro hormonálně rezistentní pacientky má abemaciclib v kombinaci s hormonální terapií. Nicméně situace není tak jednoduchá a srovnání výsledků těchto studií komplikuje rozdílné uspořádání a stratifikace pacientek.

Advanced hormone-positive breast cancer is an incurable disease, but fortunately nowadays we have several therapeutic approaches. In this article we will focus on the subpopulation with primary or secondary endocrine resistance and the choice of the first line of palliative therapy with cyclin-dependent kinase 4/6 (CDK4/6) inhibitors, which we will demonstrate it on case report. The design, results, and subgroup analysis of the MONARCH 2, MONALEESA-3, and PALOMA 3 clinical trials currently indicate that abemaciclib has the most robust data for endocrine resistant patients. However, the situation is not so simple and comparing the results of these studies complicates the different design and stratification of patients.
