47letý pacient s diabetem 2. typu má osobnost změněnu základním psychickým onemocněním – schizofrenií. Edukace v režimových opatřeních, dietě i nové léčbě je u tohoto pacienta obtížnější, pacient přijímá změny velmi pomalu. Jeho antidiabetická terapie, zahrnující po osmi letech trvání diabetu kombinaci metforminu, sitagliptinu a bazálního inzulínu, musela být intenzifikována pro narůstající postprandiální výkyvy glykemie po obědě a hyperglykemie přetrvávající do pozdních večerních hodin. Změna terapie na inzulín Ryzodeg přidaný k terapii metforminem XR zajistila kompenzaci pacienta pomocí jednoduchého režimu, aniž by bylo nutné navyšovat počet dávek inzulínu a počet perorálních antidiabetik.

A 47-year-old patient with type 2 diabetes mellitus has his personality altered by an underlying mental illness – schizophrenia. Education in regimen measures, diet and new treatment is more difficult for this patient as he accepts any changes very slowly. The patient’s antidiabetic therapy, including a combination metformin, sitagliptin and basal insulin after 8 years of diabetes duration, needed to be intensified due to increasing postprandial glycaemic fluctuations after lunch and hyperglycaemia persisting into the late evening hours. Changing therapy to insulin Ryzodeg supplemented with metformin XR ensured the patient’s diabetes control using a simple regimen without the need to increase the number of insulin doses and the number of oral antidiabetic drugs.
