Lutecium vipivotid tetraxetan (177Lu-PSMA-617, přípravek Pluvicto) je nové terapeutické radiofarmakum, které prokázalo v klinických studiích slibné léčebné výsledky u pacientů s metastazujícím kastračně rezistentním karcinomem prostaty předléčených inhibitorem dráhy androgenního receptoru (adrogen receptor pathway inhibitors, ARPI) +/- chemoterapií. Kazuistika prezentuje 75letého pacienta s metastazujícím karcinomem prostaty, předléčeného inhibitorem dráhy androgenního receptoru i chemoterapií, který absolvoval terapii 177Lu-PSMA-617 s ukončením po dvou cyklech pro radiografickou progresi onemocnění. Iniciální vzestup prostatického specifického antigenu (PSA) u pacienta po první aplikaci byl následován léčebnou odpovědí s výrazným poklesem PSA a výborným klinickým účinkem trvajícím devět měsíců. Radionuklidová terapie představuje vysoce inovativní přístup v léčbě karcinomu prostaty, navíc s účinností i v pokročilých fázích onemocnění.

Lutetium vipivotide tetraxetan (177Lu-PSMA-617, Pluvicto) is a new therapeutic radiopharmaceutical that has shown promising therapeutic results in clinical trials in patients with metastatic castration-resistant prostate cancer pretreated with ARPI +/- chemotherapy. This case report presents a 75-year-old patient with metastatic prostate cancer, pretreated with androgen receptor pathway inhibitor and chemotherapy, who underwent therapy with Pluvicto (177Lu-PSMA-617) with discontinuation after two cycles due to radiographic disease progression. Initially, there was an increase in prostate specific antigen (PSA) after the first administration. However, it was followed by a treatment response with a significant decrease in PSA and an excellent clinical effect lasting nine months. Radionuclide therapy represents a highly innovative approach in the treatment of prostate cancer with efficacy even in advanced stages of the disease.
