Autori popisujú klinický obraz a rádiologické nálezy u 4 detí s ložiskovou ischémiou mozgu. Vo všetkých prípadoch po banálnom úraze hlavy alebo počas fyzickej aktivity vznikla náhle alebo postupne čistá motorická hybná porucha s expresívnou afáziou alebo bez nej. CT a angiografícký nález bol u všetkých detí rovnaký. V CT mozgu urobenom 1-8 dní po vzniku hybnej poruchy boli hypodenzity v oblasti bazálnych ganglií. V angiografickom vyšetrení boli prítomné nepravidelnosti lumenu vnútorhej krkavice supraklinoidálne, iniciálneho úseku strednej a prednej mozgovej tepny a v dvoch prípadoch kompletná oklúzia strednej mozgovej tepny. Vyšetrenia sa opakovali 9 mesiacov až 3 roky po vzniku ochorenia. V CT pretrvával obraz postmalatických cýst v oblasti bazálnych ganglií, angiografický nález sa u 3 detí zlepšil, u jedného dieťaťa bol v norme. Fatické poruchy sa upravili u všetkých detí, u dvoch detí pretrváva hybná porucha. Uvedený klinický obraz i rádiologické nálezy sú t5npické pre disekujúcu aneuryzmu vnútornej krkavice a mozgových tepien. Disekujúca aneuryzma môže byť príčinou ložiskovej ischemic mozgu u detí a v etiologicky nejasných prípadoch je potrebné myslieť na túto možnosť.
The authors describe the clinical picture and radiological findings in four children with focal ischaemia of the brain. In all instances the children developed after a trivial injury or during physical activity a sudden or gradual purely motor disorder with or without expressive aphasia. CT and the angiographic finding were in all children identical. In the CT of the brain made 1-8 days after development of the motor disorder there were hypodensities in the area of the basal ganglia. In the angiographic finding there were irregularities of the lumen of the internal carotid in a supraclinoidal position, of the initial portion of the middle and anterior cerebral artery and i i in two instances complete occlusion of the middle cerebral artery. The examinations were repeated after 9-month to 3-year intervals after onset of the disease. In the CT the picture of postmalatic cysts in the area of basal ganglia persisted, the angiographic finding improved in three children, in one chUd it was normal. Phatic disorders improved in all children. in two children the motor disorder persists. The mentioned clinical picture and radiological findings are typical for a dissecting aneurysm of the internal carotid and cerebral arteries. A dissecting aneurysm can be the cause of focal ischaemia of the brain in children and in etiologically obscure cases this possibility must be taken into account.

Migréna je častým ochorením aj u detí, mozgový infarkt ako komplikácia migrény je však veľmi zriedkavý. Autori popisujú kazuistiku 12ročného dievčaťa, u ktorého sa po trojmesačnej anamnéze migrény bez aury pri záchvate bolestí hlavy objavili pravostranné hemikonvulzie s pozáchvatovou hemiplégiou s CT korelátom malácie v favom putamene. Hybná porucha sa upravila v priebehu 24 hodín. V EEG boli prítomné atypické ostré vlny okcipitálne vfavo, angiografícký nález bol v norme. Podobný stav s ľahším priebehom sa opakoval o 18 dní a o 8 mesiacov neskôr znovu bolesť hlavy. hemikonvulzie vľavo, reverzibilná hemiparéza bez CT a angiografického korelátu. Liečená najskôr antiepileptikami a flunarizínom, po normalizácii EEG po 15 mesiacoch bola liečba ukončená. Nasledujúcich 6 rokov pri liečbe lisuridom a flunarizínom len sporadické bolesti hlavy bez aury a vegetatívnej symp^omatológie. Posledné tri roky bez ťažkostí a bez liečby. Migrenózny infarkt predstavuje zriedkavú príčinu ložiskovej ischémie mozgu u deti.
Migraine is a frequent disease even in children, migrainous stroke as a complication of migraine is however very rare. The authors describe the case-history of a 12-year-old girl who after a three-month history of migraine without aura developed during an attack of headache dextrolateral hemiconvulsions with post-attack hemiplegia with a CT correlate of malacia in the left putamen. The motor disorder receded within 24 hours. In the EEG there were atypical sharp waves in an occipital position on the left, the angiographic finding was normal. A similar condition with a milder course occurred again after 18 days, and 8 months later again headache, hemiconvulsions on the left side, reversible hemiparesis without a CT and angiographic correlate. The patient was treated at first with anti-epileptics and flunarizine after normalization of the EEG after 15 months treatment was terminated. During the subsequent six years with lisuride and flunarizine treatment there was only sporadic headache without aura and a vegetative symptomatology. During the last three years there were no complaints. treatment. Migrainous stroke is a rare cause of focal cerebral ischaemia in children.
