Práce srovnává hodnocení anestezie a zotavení dvou typů anestezie pro malé gynekologické operace. 40 nepremedikovaných pacientek s ASA I-II bylo rozděleno do dvou skupin. V1. skupině byla podána anestezie alfentanil 15 μg/kg i.v. a po 60 sekundách propofol 2,5 mg/kg. Ve 2. skupině byl podán alfentanil 15 μg/kg spolu s midazolamem 0,04 mg/kg i.v. a po 60 sekundách byl podán propofol v redukované dávce 1,0 mg/kg. Pacientky byly ventilovány směsí N2O + O2, eventuálně byla anestezie prohloubena krátkou inhalací 4 % halotanu. Sledovali jsme parametry anestezie, dále po 2,5 minutách TK, pulz a SpO2 a kvalitu zotavení třemi testy ve 30. a 60. minutě. Obě skupiny se nelišily v parametrech anestezie a v 60. minutě stupněm zotavení. Midazolam v malé dávce 0,04 mg/kg i.v. aplikovaný 60 sekund před propofolem umožní redukovat dávky z 2,5 na 1,0 mg/kg. Umožní též redukci nákladů na anestezii při zachování důležitých skóre zotavení v 60. minutě.
To compare anesthesia and recovery after two different types of anesthesia for small gynecologic procedures, we randomized 40 unpremedicated patients ASA l-ll to two groups. Patients in the group 1 were anesthetized with the induction dose ofalfentanil 15 μg/kg followed with propofol 2,5 mg/kg after 60 seconds. Patients in the group 2 were anesthetized with alfentanil 15 μgAg with midazolam 0,04 mg/kg, iv, followed with propofol in reduced dose 1 mg/kg after 60 seconds. Patients were ventilated with the mixture of oxygen/nitrous oxide, with eventual deepening of anesthesia with halothane 4%. We followed parameters of anesthesia, blood pressure, heart rate and SpO2 every 150 sees. The recovery profile was assessed with three tests after 30 and 60 minutes. There were no differences in anesthesia parameters between groups and in an assessment of recovery after 60 minutes. Administration of midazolam in a small dose 0,04 mg/kg iv 60 seconds prior to propofol enables to reduce the propofol dose from 2,5 to 1,0 mg/kg. This leads to reduction in anesthesia costs while important recovery score after 60 minutes is maintained.
