Úvod: V poslední dobì se vede diskuze o snížení kardiorespiraèní zdatnosti u naší populace. Normy pro kardiorespiraèní zdatnost byly vytvoøeny v 70. letech minulého století v rámci rozsáhlé populaèní studie, kterou od té doby nikdo nezopakoval, a to z organizaèních i finanèních dùvodù. Cíle: Cílem této práce bylo zhodnotit kardiorespiraèní zdatnost u sportovcù a nesportovcù vyšetøovaných bìhem dvou let na jednom pracovišti na základì monitorování parametru maximální spotøeby kyslíku (VO2max). Metodika: Retrospektivní analýza zdravotní dokumentace u sportovcù a pacientù, kteøí v letech 2016 a 2017 byli vyšetøeni na Klinice tìlovýchovného lékaøství. Ze studie byly vylouèeny osoby s onemocnìním, které by mohlo ovlivòovat kardiorespiraèní zdatnost, a osoby s jiným dùvodem ukonèení zátìže, než je dosažení subjektivního a objektivního maxima. Ve vìkovém rozmezí 11–59 let byla spiroergometrie provedena u 690 osob (430 mužù). V této skupinì bylo zahrnuto 346 sportovcù (259 mužù) prùmìrného vìku 22,8 ± 11,8 a 344 nesportovcù (171 mužù) prùmìrného vìku 35,0 ± 15,3. Maximální spotøebu O2 jsme srovnali s platnými tabulkami pro daný vìk a pohlaví. Ke statistickému porovnání bylo použito z skóre a 95% interval spolehlivosti. Výsledky: Ve skupinì nesportovcù mìlo hodnoty VO2max v normì celkem 79,1 % osob (95%CI: 74,8%– 83,4%), pod normoumìlo hodnoty 18,9%a nad normou 2,0%nesportovcù. Ve skupinì sportovcù mìlo hodnotyVO2max v normì celkem81,5%(95%CI: 77,4%– 85,6%), pod normou mìlo hodnoty 2,3 % a nad normou 16,2 % sportovcù. Pøi porovnání sportovcù a nesportovcù v distribuci z-skóre pro parametr VO2max byl prokázán signifikantní rozdíl (p < 0,0001). Ve skupinì sportovcù bylo signifikantnì více osob s hodnotami VO2max nad normou a signifikantnì ménì osob s hodnotami pod normou. Celkovì normy splòuje signifikantnì ménì probandù než 95%, a to ve skupinì nesportovcù, ale i ve skupinì sportovcù. Ani v jednom ze souborù neleží hodnota 95 % v 95% intervalu spolehlivosti pro podíl probandù s hodnotami v normì. Závìr: Kardiorespiraèní zdatnost jak u sportovcù, tak u nesportovcù celkovì neodpovídá populaèní normì ze 70. let minulého století. Odborná veøejnost by mìla zahájit diskuzi, zda je èas pro multicentrickou studii v ÈR, která by stanovila nové normy.
Introduction: We are now discussing the decline of cardiorespiratory fitness of our population. Czech population norms were published in the 1970s. These data are based on large population study and they were not updated because of organizational and financial reasons. Aim: The aim of this study is to assess cardiorespiratory fitness of athletes and non-athletes in which maximal oxygen consumption was evaluated. Material and Methods: Retrospective analysis of athletes and other subjects which were evaluated in the Department of Exercise Medicine and Cardiovascular Rehabilitation, University Hospital Olomouc, Czech Republic. Into this study were enrolled only subjects without a disease which could influence cardiovascular fitness. Subjects with the different reason of exercise test termination than is subjective or objective maximum exercise tolerance were excluded. Spiroergometry was performed in 690 subjects (430 men, age 11–59). 346 athletes (259 men) of average age 22.8 ± 11.8 and 344 non-athletes (171 men) of average age 35.0 ± 15.3 were included. Maximal oxygen consumption was compared with population norms. The statistical analysis was performed using z score and 95% confidence interval. Results: In the non-athletes group 79.1% of subjects reached population norm (95% CI: 74.8% – 83.4%), 18.9% was below norm and 2.0% above population norm. In the athletes group 81.5% (95% CI: 77.4% – 85.6%), reached population norm, 2.3% was below norm and 16.2% above population norm. In the comparison of athletes and non-athletes we have found a significant difference in the VO2max max z-score distribution (p < 0.0001). In the athlete's group, significantly more subjects above population norm were present. In both groups less than 95% of probands reached norms and in both groups, 95% value reaches a 95% confidence interval. Conclusions: Cardiorespiratory fitness in both groups (athletes/non-athletes) is not the same as presented in population norms in the 1970s. Experts should initiate discussion whether there is a need for a new multicentric study which would provide new population norms.
