OBJECTIVES: The aim of the study is to analyse drug-related infectious diseases (DRID) rates for people who inject drugs (PWID) in the Pilsen Region in order to identify the main determiners of infection risk and also to provide a foundation for comparison between this region and the others in the Czech Republic. METHODS: In a descriptive cross-sectional study, we analysed the Pilsen Region's data on PWID. The data was transcribed from the 2003 to 2018 internal database of the Ulice Outreach Programme. In addition to the data regarding the testing of DRID, we analysed commercial sex work (CSW) and the PWID's duration of drug use, age and current address. The statistical analysis was performed using SPSS, primarily employing logistic regression (i.e., backward elimination method) to explore predictors of seropositivity. Moreover, we calculated its prevalence from an epidemiological perspective. RESULTS: In total, 384 PWID were tested, from which 54.7% were males, and 84.1% were from Pilsen. The average age for initiation of using drug was 19.37 years. The most used drug was methamphetamine (64.8%), 77 women (20.1%) were reported to be CSW. The prevalence of DRID was as follows: hepatitis C virus (HCV) 37.24%, syphilis 1.82%, hepatitis B virus (HBV) 0.78%, and HIV infection 0.26%. The analysis showed that men had a lower risk of syphilis than women. Individuals who started their drug use via injection had a 1.365-times higher risk of DRID in comparison to those who initiated intravenous drug use later in their drug-using lives. We identified a significant association between the drug type and the risk of HCV infection: the main predictor of seropositivity was the use of fentanyl, which posed a 1.930-times higher risk than in the case of methamphetamine. CONCLUSIONS: This study is the first descriptive cross-sectional study implemented in the Pilsen Region in the Czech Republic with a focus on the subpopulation of PWID with individual data. A high prevalence of HCV infection still persists but the prevalence of HBV and HIV infections in this study (and generally in the Czech Republic) is relatively low compared to foreign studies. Syphilis is not closely associated with injecting-drug use, but rather with the sexual behaviour of the people who use drugs intravenously. The most important predictor of seropositivity for syphilis was CSW. We also found the duration of being a CSW to be significant influence. The women who had been CSWs for less than 5 years had a significantly lower risk of syphilis than those who had prostituted for more than 5 years.

Cíl: Studie se zaměřuje na identifikaci hlavních rozdílů ve výskytu vybraných sexuálně přenosných infekcí v třech krajích České republiky. Metody: Deskriptivní průřezová studie analyzuje data získaná z jednotlivých institucí disponujících přístupy do Registru pohlavních nemocí za jednotlivé kraje. Do studie vstupovaly celkem tři kraje, přičemž do studie byla integrována pouze ta data, která se vyskytují v časově stejném intervalu ve všech krajích (tj. v letech 2006–2013). Pro porovnání vývoje epidemiologické situace byla použita míra incidence (incidence rate – IR) a specifická míra incidence výskytu (specific incidence rate –SIR) pro věk a věkové skupiny. Byly implementovány metody deskriptivní statistiky. Pro analýzu statisticky významných rozdílů byla použita ANOVA při p = 0,01, případně nepárový T-test, kterému předcházel F-test pro testování rozptylů. Výsledky: Nejvyšší incidence rate pro všechny integrované STIs byla uváděna v kraji Hlavní město Praha (HMP) 398,3 případů na 100 tisíc obyvatel, přičemž nejčastěji se vyskytovala ve dvou krajích kapavka (HMP) a Středočeský kraj (STR), zatímco v Plzeňském kraji (PLZ) se nejčastěji vyskytovala syfilis. ANOVA identifikovala statisticky významné rozdíly mezi všemi integrovanými kraji v rámci výskytu všech STIs jako skupiny infekcí i jednotlivých STIs. Ovšem T-test identifikoval statisticky signifikantní rozdíly ve většině případů pouze mezi HMP a zbylými dvěma kraji, avšak mezi PLZ a STR tyto rozdíly identifikovány nebyly. Rovněž byly identifikovány statisticky významné rozdíly (ANOVA pro všechny tři kraje vzájemně) pro SIR pohlaví a věkovou skupinu 15–64 let, kde bylo agregováno nejvíce případů. Ve všech krajích bylo více mužů (HMP 75 %, PLZ 58,7 % a STR 66 %). Většinu případů tvořili Češi (69,7–79,8 %). Koinfekce HIV byla reportována u HMP v 5,5 % případů u PLZ 3,9 % a u STR 5,3 %. Závěr: Studie předkládá výsledky porovnání dat ze tří krajů Česka za období 2006–2013. Poukazuje na rozdíly ve vývoji sexuálně přenosných infekcí v jednotlivých krajích a v některých atributech, zejména u mužů majících sex s muži a dalšími rizikovými skupinami. Apeluje rovněž na nedostatečnou znalost problematiky sexuálně přenosných infekcí u subpopulace MSM.
Aim: The research aimed to identify and describe the main differences in the incidence of sexually transmitted infections in three regions of the Czech Republic. Methods: Descriptive cross-section study analyses data received from each institution that allowed access to the Registry of sexually transmitted illnesses of individual regions. Mainly three regions are being studied, yet the study considers only the data from the same time-period in all three regions (i. e., 2006–2013). To compare the development of the epidemiological situation, we considered the incidence rate (IR) and specific incidence rate for both age and age groups. Methods of descriptive statistics were implemented. For the analysis of statistically significant differences, we used ANOVA when p = 0.01, alternatively in some cases F-test before an unpaired T-test to test variability. Results: The highest incidence rate for all integrated STIs was reported in the Capital City of Prague region (CCP) – 398.3 cases per 100 thousand – whereas gonorrhoea appeared the most in CCP and Central Bohemia regions (CBR) and syphilis occurred the most in the Pilsen region (PLS). ANOVA identified statistically significant differences between all integrated regions within the frame of incidence of all STIs as a group of infections, and also as individual STIs. Of course, the T-test identified statistical differences in most cases only between CCP and the other two regions, even though these differences weren’t identified between CBR and PLR. Likewise, there were statistically important differences identified (by ANOVA for all three regions mutually) for the specific incidence rate for gender and the 15–64 age group category where the most cases were aggregated. In all three regions, there were more male cases (CCP 75%, PLR 58,7% and CBR 66%) with most of them being composed of Czechs (69.7–79.8 %). HIV coinfection was reported in 5.5% of cases in CCP, 3.9% in PLR and 5.3% in CBR. Conclusion: This study submits the results of data comparison from three Czech regions in the 2006–2013 time-period. It points out differences in the development of sexually transmitted infections in individual regions, mainly in men having sex with men and other risk groups. It also invokes the issue of the insufficient level of knowledge concerning sexually transmitted infections in the MSM subpopulation.

Zdravotní gramotnost je důležitým předpokladem pro udržení zdravé populace. V České republice a mezi českými autory neexistuje mnoho studií zaměřených na zdravotní gramotnost týkající se infekce HIV ve skupině dospívajících osob. V kontrastu s tím stojí poměrně vysoký vědecký zájem o informovanost a vnímání této problematiky dospívajícími v nejvíce postižených oblastech světa, zejména v subsaharské Africe a Indii. Dospívající patří v oblasti infekce HIV do vysoce rizikové skupiny, přičemž vzdělání je jedním z hlavních faktorů ovlivňujících úroveň rizika infekce HIV. Zvyšování zdravotní gramotnosti je však realizováno pouze na některých typech středních škol. Cílem této studie je ověřit zdravotní gramotnost týkající se infekce HIV u adolescentů ve dvou typech středních škol (gymnáziích a středních odborných učilišť), a to ve dvou krajských městech: v Plzni a v Českých Budějovicích. Pro zjištění úrovně zdravotní gramotnosti v oblasti infekce HIV byl sestaven dotazník s 13 otevřenými a 9 uzavřenými otázkami, který byl implementován mezi studenty (N = 1 942) v 16 vybraných školách v roce 2015. Dotazník byl předem testován a jeho vyplnění zabralo cca 15 minut. Studie prokázala, že ženy studující na gymnáziích disponují nejlepšími znalostmi v oblasti zdravotní gramotnosti infekce HIV. Oproti tomu nejnižší úroveň znalostí byla identifikována mezi muži, kteří studují střední odborná učiliště. Znalosti a orientace v oblasti problematiky HIV/AIDS však nestačí. Důležitý je i fakt, zda dospívající dokáží tyto znalosti přeměnit ve svůj prospěch, a přispět tak k redukci rizika infekce HIV a svého vlastního rizikového chování. Zavedení preventivních programů na všech typech škol a vzdělávání adolescentů v oblasti sexuálně přenosných nemocí je pro snížení expozice vůči HIV zcela zásadní. Existující rozdíly mezi pohlavími a školami mohou u některých těchto skupin zvýšit riziko infekce. Na základě zjištěných skutečností doporučujeme hledat cesty pro sjednocení a zefektivnění přístupu k výuce tohoto tématu na všech typech škol a více programy zaměřit na chlapce, neboť muži jsou největší skupinou HIV pozitivních osob v Česku.
Despite health literacy being an important assumption for maintaining a healthy population, there are not many studies focusing on health literacy concerning HIV infection among adolescents in the Czech Republic and among Czech authors. In contrast, there are plenty of scientific papers and researches focused on awareness and susceptibility of adolescents in most affected regions of the world, Sub-Saharan Africa and India particularly. Adolescents belong to a high-risk group of the population. Education is one of the most important factors that influence the level of risk of HIV infection. However, increasing health literacy is only implemented in some types of secondary schools in the Czech Republic. The goal of this study is to verify health literacy concerning HIV infection among adolescents in two types of Czech secondary schools (grammar school and vocational school) in two Czech regional cities (Plzeň and České Budějovice). To determine the level of health literacy of HIV infection, a questionnaire (13 open and 9 closed questions) was compiled. A descriptive cross-sectional study was implemented among students (N = 1 942) of 16 chosen schools in two cities of the Czech Republic in 2015. The questionnaire was pre-tested and it took about 15 minutes to complete. The association was tested by a chi-square test, the averages were then compared with an independent t-test, the p-value (0,05) was used in all tests. Most of the students (67,3%) had good health literacy about HIV/AIDS, statistically significant differences were found between students of grammar schools and vocational schools (p = 0,012). Differences were also found between men and women (p = 0,037). The respondents showed the best knowledge about transmission (94,3%), the poorest about epidemiological questions (18,3%). Gender and the type of school had a statistically significant influence on the level of knowledge (gender x2; p = 0,042, types of a school x2; p = 0,031). In this cross-sectional study, women studying at grammar schools had the best knowledge of HIV/AIDS. In contrast, the lowest level of knowledge was found in men studying in vocational schools. The school types and gender had a statistically significant influence on the level of knowledge. However – knowledge is not enough. To reduce infection risk potential will mean to help adolescents to transform knowledge into diminishing of their risky behaviour. The implementation of prevention in all types of schools is crucial for the reduction of exposure from HIV infection. We consider educating adolescents about sexually transmitted diseases important. Despite that our research showed differences among schools and gender. Those differences can make a particular group be more at risk of being infected. Based on the findings, it would be suitable to unify and improve the approach to the education of this topic in all types of schools and to target preventive and educational techniques to boys more, since men are the biggest group of the HIV positive people in the Czech Republic.
