Úvod: Extravazace je přesun tekutiny mimo její konduit do extracelulární tkáně, což může vést k lokální zánětlivé reakci, kompartment syndromu, nekróze tkáně a ztrátě kůže v celé tloušťce postižené oblasti. Pro prevenci této komplikace je naprosto důležité včasné rozpoznání léčba extravazačního poranění. Popis případu: Prezentujeme případ ilustrující extravazační poranění prsu řešené renovovanou operační technikou – akutní nízkotlakou evakuací. Čtyřiapadesátiletá žena se dostavila na pohotovost, kdy do pravého prsu jí uniklo 1,5 litru celkové parenterální výživy podávané centrálním žilním katétrem. Pacientka byla zajištěna do 24 hodin od incidentu a měla difúzně oteklý, citlivý a erytematózní pravý prs, ale neměla žádnou nasedající kožní nekrózu. Pro odsátí extravazátu celkové parenterální výživy a pro minimalizaci aspirace zdravé tukové tkáně prsu byla provedena suchá akutní nízkotlaké evakuace dvěma malými incizemi. Bylo popsáno pouze několik případů extravazačních poranění do prsu a většina případů zahrnuje extravazaci chemoterapie z centrálního katétru. Tato komplikace je v literatuře pravděpodobně podceňována. Závěr: Naše kazuistika poukazuje na možnost využití aspirace při léčbě difúzně se šířících extravazátů. Akutní nízkotlaká evakuace je vhodnější termín než liposukce, protože zde není záměrem evakuovat zdravou tukovou tkáň. Tento nový termín je méně matoucí a může pomoci lékařskému personálu a pacientům lépe porozumět léčbě, protože jej nespojuje s estetickým zákrokem. Jedná se o účinnou a kosmeticky vyhovující techniku léčby časného extravazačního poranění prsu při totální parenterální výživě.
Background: Extravasation is the movement of fluid outside its conduit into the extracellular tissue, possibly leading to a local inflammatory reaction, compartment syndrome, tissue necrosis, and full thickness skin loss at the affected area. To prevent these complications, early recognition, referral and treatment of an extravasation injury is of utter importance. Case report: We present a case, illustrating an extravasation injury into the breast managed by a renovated surgical technique – emergency evacuation low-pressure suction (EELS). A 54-year-old woman attended the emergency department with 1.5 L of total parenteral nutrition leaked into her right breast via a central portal catheter. The patient was seen within 24 hours of the incident, and presented with a diffuse swollen, tender and erythematous right breast but no overlying skin necrosis. Dry EELS via two small stab incisions was performed to evacuate the extravasated total parenteral nutrition and to minimise the aspiration of healthy fat tissue of the breast. Only a few cases of extravasation injuries into the breast have been described and most cases involve chemotherapy extravasation from a central catheter. Presumably there is an underreporting of this complication in the literature. Conclusion: This case-report promotes the use of aspiration in the management of diffusely spread extravasated injuries. EELS is a more appropriate term than liposuction, as there is no intention to evacuate healthy fat tissue. This new term is less confusing and might help medical staff and patients to better understand the treatment as it will break the affiliation with an aesthetic procedure. It is an effective and cosmetically satisfactory technique to treat early total parenteral nutrition extravasation injury at the level of the breast.

Určení ideálního načasování revizní operace po infikované kranioplastice je náročné. Je třeba vzít v úvahu jak hojení infikované kosti, tak připravenost měkkých tkání. Neexistuje žádný zlatý standard týkající se načasování revizní operace a řada studií má protichůdné výsledky. Mnoho studií doporučuje vyčkat přibližně 6–12 měsíců, aby se snížilo riziko reinfekce. Tato kazuistika zdůrazňuje, že odklad revizní operace infikované kranioplastiky je užitečnou a přínosnou modalitou. Umožňuje delší časový rámec pozorování výskytu dalších infekčních epizod. Kromě toho „delay“ zvyšuje neovaskularizaci tkáně a může tak vést k méně invazivním rekonstrukčním technikám s minimalizací morbidity donorského místa.
It is a challenge to define the ideal timing for revision surgery following an infected cranioplasty. Both healing of infected bone and preparedness of soft tissue must be considered. There is no gold standard regarding the timing of revision surgery and a lot of studies have contradictory findings. Many studies recommend waiting for 6–12 months to reduce reinfection risks. This case report highlights that delay in revision surgery for an infected cranioplasty is a useful and rewarding modality. It allows a longer observational timeframe to monitor for infectious episodes. Furthermore, vascular delay enhances tissue neovascularization and may therefore lead to less invasive reconstructive techniques with minimized donor site morbidities.

Basal cell carcinoma (BCC) is the most prevalent malignancy, with rising incidence worldwide. Despite its naturally slow growth and initially low metastatic potential, it can cause significant morbidity and mortality when unrecognized, inadequately treated or poorly followed up. Authors present the case of a 61-year-old male with a 7-year history of multiple incomplete excisions of a "simple" BCC on the forehead. A CT scan of the head revealed an invasive mass (5.2 cm laterolateral x 4.0 cm craniocaudal) in the frontal area. There was no evidence of metastasis. Complete resection of the lesion and reconstruction was achieved in three stages. Final reconstruction was achieved using a left frontal fasciocutaneous flap. The secondary defect was closed with an advancement flap of the scalp and donor sites were covered using a split-thickness skin graft from the upper limb. This case demonstrates the necessity for vigilance in the approach to, diagnosis, treatment and follow-up of these skin neoplasms. The development of giant BCCs should be avoided at all costs. Increased size of BCCs corresponds with increased recurrence rate, metastatic rate, morbidity, mortality, treatment difficulties and overall costs.
