Fractures of the proximal tibia are high energy fractures. According to available literature, 3-4% of all fractures heal problematically with non-union or osteomyelitis. Usage of locking compression plating system as well as usage of hybrid fixator is associated with the risk of compartment syndrome, local infection and subsequent malunion, osteomyelitis or non-union. We present a case of 30-year-old male suffering from posttraumatic non-union. The medial femoral condyle flap was selected due to its favourable properties providing a highly vascularised osteogenic tissue in combination with anterolateral thigh flap and Corlett loop. Our case report describes a reconstruction of a long weight-bearing bone. Despite the amount of anastomoses and usage of vein graft, the selected method of reconstruction seems to be safe and with a long-lasting benefit for the patient.

Toxická epidermální nekrolýza je autoimunitní onemocnění, které se dominantně projevuje v oblasti kůže a sliznic. Jedná se o závažné bulózní onemocnění, při kterém dochází k indukci apoptózy v oblasti dermo-epidermální junkce. U většiny případů byla popsána souvislost s předchozím užitím nějakého farmaka. Základním přístupem k zastavení progrese onemocnění je imunosuprese. Bohužel pacienti s rozsáhlou ztrátou kožního krytu a mnohdy defektní sliznicí jsou konfrontováni s celou řadou často i oportunních potenciálně patogenních mikroorganismů. Nepřekvapí proto, že v dnešní době jsou infekční komplikace dominantní příčinou úmrtí u takto postižených pacientů. I přes důslednou literární revizi jsme nenašli ucelenou kazuistiku týkající se rozvoje multifokální aspergilové infekce u pacienta s tímto onemocněním.
Toxic epidermal necrolysis is an autoimmune disease expressed predominantly on the skin and mucous membranes. It is a serious bullous disease manifesting itself by induction of apoptosis in the dermo-epidermal junction. In most cases,it is attributable to the use of some drug. The basic approach to stopping progression of the disease is immunosuppression. Unfortunately, patients with such extensive loss of epidermis and defective mucosa are confronted by a variety of opportunistic, potentially pathogenic microorganisms. Unsurprisingly, infectious complications are today a predominant cause of death in patients thusly affected. Despite thorough review of the literature, we found no comprehensive case report concerning the development of multifocal Aspergillus infection in patients with this disease.

Cíl: Stanovit základní epidemiologické ukazatele u popálených pacientů s mikromycetární infekcí. Identifikovat nejdůležitější mikromycety u popálených pacientů. Materiál a metodika: Monocentrická, retrospektivní studie, do které byli zařazeni všichni dospělí popálení pacienti, kteří byli hospitalizováni v letech 2007–2015 a u kterých byla v průběhu hospitalizace izolována mikromyceta. Pro hodnocení závažnosti termického traumatu byl použit ABSI (Abbreviated burn severity index). Výsledky byly statisticky zpracovány. Výsledky: Celkem bylo ve sledovaném období identifikováno 61 pacientů s termickým traumatem, u kterých byla izolována kvasinka či vláknitá houba. Z tohoto počtu bylo 37 mužů a 24 žen (M:F ratio 1,5:1). Průměrný věk pacientů v souboru byl 57,3 let (29 pacientů bylo ve věku do 60 let, 32 pacientů ve věku nad 60 let včetně). Šest pacientů zemřelo (letalita byla 9,8 %). Průměrný rozsah popálené plochy byl 21,6 % TBSA (medián 14,0 %). U těchto pacientů bylo vykultivováno celkem 90 kmenů mikromycet (79 kvasinek, 11 vláknitých hub). U 30 pacientů byla mikromyceta izolována z popálené plochy, u 19 pacientů z dolních dýchacích cest, u 15 pacientů z urogenitální oblasti a u 7 pacientů z hemokultury. Mezi kvasinkami dominovaly kmeny non-albicans kandid (60 kmenů), Candida albicans byla izolována celkem 16x. U vláknitých hub to byly kmeny Aspergillus fumigatus (4x izolace) a Fusarium sp. (2x izolace). Závěr: Podařilo se nám identifikovat základní epidemiologické ukazatele u popálených pacientů s mikromycetární infekcí, podobně jako nejvýznamnější kvasinky a vláknité houby, které způsobují infekci u těchto pacientů.
Goal: Determination of basic epidemiological parameters of burn patients with micromycetes infection. Identification of the most important micromycetes in burn patients. Material and methods: Monocentre retrospective study enrolling all adult burn patients who were hospitalized between 2007 and 2015 and in whom micromycetes were isolated during hospitalization. ABSI index (Abbreviated Burn Severity Index) was used to evaluate severity of thermal trauma. Results were statistically analysed. Results: There were 61 patients with thermal trauma identified in total during the period of observation, and there were yeast or fibrous fungi isolated. There were 37 males and 24 females (M:F ratio – 1.5:1) in this group. The average age of patients was 57.3 years (29 patients were aged up to 60 years, 32 patients were over the age of 60 years, inclusive). 6 patients died (lethality was 9.8%). The average extent of the burn area was 21.6% TBSA (median 14.0%). There were 90 strains of micromycetes cultured in total in these patients (79 yeasts, 11 fibrous fungi). Micromycetes were isolated from burn area in 30 patients, from the lower airways in 19 patients, from the urogenital area in 15 patients and from blood culture in 7 patients. Non-albicans Candida species were predominant among yeasts (60 strains); Candida albicans was isolated 16 times in total. Aspergillus fumigatus (4 isolations) and Fusarium species (2 isolations) were predominant species among fibrous fungi. Conclusion: We successfully identified the basic epidemiological parameters in burn patients with micromycetes infection, similarly to the most important yeasts and fibrous fungi causing infection in these patients.

Cíl: Cílem této studie bylo zhodnocení účinnosti nového topicky aplikovaného gelu pro zrychlení reepitelizace donorských míst. Materiál a metody: Oleogel-S10 je mast obsahující suchý extrakt z březové kůry obsahující triterpen bohatý na betulin, který byl testován v otevřené, zaslepené, prospektivní, kontrolované, randomizované, multicentrické studii hodnotící zlepšení hojení rány v donorských místech. Primární koncový parametr byl čas do uzavření rány a sekundární koncový parametr byla měření související s jizvou v době uzávěru rány a 3 a 12 měsíců po uzavření rány (POSAS, laserová kontrastní analýza LASCA, viskoelastická analýza). Výsledky: Uvádíme výsledky z jednoho centra (Klinika popálenin a rekonstrukční chirurgie, Fakultní nemocnice Brno), kde byla provedena tato klinická studie fáze III. Celkem bylo zařazeno 32 pacientů (25 mužů a 7 žen) s průměrným věkem 41,8 let (SD, ?11,66). Průměrný rozsah donorských míst pacienta ve studii byl 56,77 cm2 (SD, ?20,39). Střední čas hojení u skupiny s lékem (Oleogel-S10) byl 7 dnů (95% interval spolehlivosti 7–8 dnů) a u kontrol 8 dnů (95% IS 7–10 dnů). Srovnání údajů POSAS ze skupiny s lékem ukázal signifikantně menší hodnoty ve všech třech časových bodech ve srovnání s kontrolami. Perfuze jizev ve skupině s lékem dosáhla v průměru 115 perfuzních jednotek na konci léčby. Průměr byl 69,8 perfuzních jednotek po 3 měsících a 50,2 perfuzních jednotek po 12 měsících. Kontrolní místa vykazovala signifikantně vyšší hodnoty ve všech časových bodech (122,2 perfuzních jednotek, 73,9 perfuzních jednotek, 52,2 perfuzních jednotek). Signifikantní rozdíly byly detekovány ve viskoelastických vlastnostech kůže s tím, že místa léčená Oleogelem-S10 vykazovala příznivější hodnoty. Závěr: V našich výsledcích prokazujeme významnou účinnost Oleogelu-S10 na hojení donorských míst.
The aim of this study was to demonstrate the efficacy of a new topically applied gel in accelerating re-epithelialization of donor sites. Material and methods: Oleogel-S10, an ointment containing betulin-rich triterpene dry extract from birch bark was tested in an open, blindly evaluated, prospective, controlled, randomized multicentre study to improve wound healing in donor sites. The primary endpoint was time to wound closure, and secondary endpoints were scar related measurements at the time of wound closure, and 3 and 12 months after wound closure (POSAS, laser speckle contrast analysis, viscoelastic analysis). Results: We report the results from a single centre (Department of Burns and Reconstructive Surgery, University Hospital Brno) of this phase III clinical trial. A total of 32 patients (25 men and 7 women) were included with the mean patient age of 41.8 years (SD, ?11.66). The mean extent of patient’s donor sites in the study was 56.77cm2 (SD, ?20.39). Median healing time of the verum group (Oleogel-S10) was 7 days (95% Confidence Interval 7–8 days) and for controls 8 days (95% CI 7–10 days). Comparison of POSAS data from the verum group revealed significantly lower values at all three time points as compared to the controls. Perfusion of scars of the verum group reached on average of 115 perfusion units at the end of treatment; the average was 69.8 perfusion units at the 3-month follow-up and 50.2 perfusion units at the 12-month follow-up. Control sites displayed significantly higher values at all time points (122.2 perfusion units, 73.9 perfusion units, 52.2 perfusion units). Significant differences were detected in the skin’s viscoelastic properties, with sites treated with Oleogel-S10 displaying more favourable values. Conclusion: In our results, we demonstrate the significant effectiveness of Oleogel-S10 in donor sites healing

Cíl studie: Stanovit prevalenci infekčních komplikací u popálených pacientů vyžadujících intenzivní péči v rámci jednodenní multicentrické studie za účasti řady popáleninových center z různých zemí Evropy. Materiál a metodika: Celkem bylo v databázi EBA vybráno 87 center jak v západní, tak i ve východní Evropě. Pro diagnostiku jednotlivých infekčních komplikací u pacientů s termickým traumatem, byla použita doporučení ABA (Americká popáleninová asociace). Získaná data byla statisticky zpracována. Výsledky: Celkem bylo do studie zařazeno 134 pacientů (44 en) z 27 popáleninových center v Evropě. Průměrný věk pacientů v souboru byl 40,39 roku (SD ? 22,17). Průměrné ABSI bylo 7,5 (SD ? 2,54), průměrný rozsah popálené plochy byl 30,49 % TBSA (SD ? 20,14). Průměrná délka hospitalizace v den provedení studie byla 24,32 dní (SD ? 30,64). Infekční komplikace byly zjištěny u 92 pacientů (68,7 %). Celkem 76 pacientů (56,7 %) splňovalo kritéria pro infekci popálené plochy, 26 pacientů (19,4 %) pro infekci krevního řečiště, 21 pacientů (15,7 %) pro pneumonii, 13 pacientů (9,7 %) pro infekci močového systému. U 29 pacientů (21,6 %) byla zjištěna multifokální infekce. U 67 pacientů (50,0 %) byl zjištěn jako potenciálně patogenní mikroorganismus grampozitivní kmen bakterie, u 73 pacientů (54,5 %) gramnegativní kmen bakterie a u 18 pacientů (13,4 %) byla zjištěna kvasinka. U žádného z pacientů v souboru nebyla izolována vláknitá houba. Závěr: Jedním ze základních pilířů budoucích standardů v jednotlivých popáleninových centrech je monitorování výskytu infekčních komplikací u popálených pacientů, prevence šíření těchto komplikací a hlášení rezistentních patogenů v Evropě. Předkládaná práce je důležitým projektem právě v této oblasti.
The objective of this study was to determine the prevalence of infectious complications in burn patients requiring intensive care in a one-day multicenter study encompassing burn centers in various European countries. Material and methods: The European Burns Association database identified 87 centers in Western and Eastern Europe, 27 of which agreed to cooperate. American Burn Association recommendations were used for diagnosis of various infectious complications in patients with thermal trauma. Results: From those centers, we randomly assigned 134 patients (44 women) to the analysis. Mean age of the group was 40.39 ? 22.17(SD) years. Mean abbreviated burn severity index was 7.5?2.54, mean size of burned area was 30.49 ? 20.14% of total body surface area. Mean length of hospitalization to date was 24.32 ? 30.64 days. Infectious complications were observed in 92 patients (68.7%), 76 (56.7%) of whom met the criteria for infection of the burned area, 26 patients (19.4%) for bloodstream infection, 21 (15.7%) for pneumonia, and 13 (9.7%) for urinary system infection. Multifocal infections were found in 29 patients (21.6%). Gram-positive bacterial strains as potentially pathogenic microorganisms were identified in 67 patients (50.0%), Gram-negative bacterial strains in 73 (54.5%), and yeasts in 18 (13.4%) patients. Filamentous fungi were not isolated from any patient in the group. Conclusion: Cornerstone future standards in individual burn centers should be to monitor the occurrence of infectious complications in burn patients, prevent the spread of these complications, and report resistant pathogens. This work constitutes an important project in this area.

OBJECTIVE: To identify the basic epidemiological characteristics of children hospitalized with diagnosis of Staphylococcal scalded skin syndrome in the Czech Republic in the years 1994-2009. INTRODUCTION: Staphylococcal scalded skin syndrome (SSSS) is a relatively rare disease in childhood. This syndrome was first defined in 1878 by Baron Gottfried Ritter von Rittershainem and belongs to the group of diseases called Burn-like syndromes. It is a bullous skin disease caused by exfoliative toxins which are produced by certain types of Staphyloccocus aureus. Typical structures affected by these toxins are desmosome proteins called Desmoglein-1 located in the stratum granulosum of epidermis. Unlike in Lyell's syndrome or Stevens-Johnson's syndrome, the exfoliation is caused by loss of adhesivity particularly in the stratum granulosum and not by induction of apoptosis in the dermo-epidermal junction. MATERIAL AND METHODS: This retrospective study was conducted on patients hospitalized in the Czech Republic in the period from 1.1.1994 to 31.12.2009. The basic condition for the inclusion in the retrospective study was age under 1 year and hospitalization due to SSSS. A total of 399 children (177 girls) fulfilled the criteria for inclusion into the study. Information was obtained from a central data depository, the Department of Health Information and Statistics, Czech Republic. RESULTS: A total of 399 children under 1 year were hospitalized for the diagnosis of SSSS in the study period. The group included 177 girls and 222 boys. M:F ratio was 1.25:1. The average incidence of SSSS in the Czech Republic was 25.11 cases per 100,000 children under 1 year of age. The highest recorded incidence in the followed period was in 1994, when a total of 57 cases of SSSS was reported, namely 53.47 per 100,000 children. By contrast, in 2003, there were reported only 12 cases and the incidence of 12.81 per 100,000 children. The average length of hospitalization was 6.39 days. In 1995, the highest average length of hospitalization was reported, which was 8.1 days, and then in 2007, the lowest average length of hospitalization, 4.4 days. There was no significant difference in the length of hospitalization in boys and girls. None of the 399 children in the population died. CONCLUSION: In our retrospective study, we established basic epidemiological characteristics of a group of children aged under 1 year with diagnosis of SSSS. As epidemiological data show, the occurrence of this syndrome is not sporadic, but steady.
