OBJECTIVES: The analysis of organic acids in urine is an important part of the diagnosis of inherited metabolic disorders (IMDs), for which gas chromatography coupled with mass spectrometry is still predominantly used. METHODS: Ultra-performance liquid chromatography-tandem mass spectrometry (LC-MS/MS) assay for urinary organic acids, acylcarnitines and acylglycines was developed and validated. Sample preparation consists only of dilution and the addition of internal standards. Raw data processing is quick and easy using selective scheduled multiple reaction monitoring mode. A robust standardised value calculation as a data transformation together with advanced automatic visualisation tools are applied for easy evaluation of complex data. RESULTS: The developed method covers 146 biomarkers consisting of organic acids (n=99), acylglycines (n=15) and acylcarnitines (n=32) including all clinically important isomeric compounds present. Linearity with r2>0.98 for 118 analytes, inter-day accuracy between 80 and 120 % and imprecision under 15 % for 120 analytes were achieved. Over 2 years, more than 800 urine samples from children tested for IMDs were analysed. The workflow was evaluated on 93 patient samples and ERNDIM External Quality Assurance samples involving a total of 34 different IMDs. CONCLUSIONS: The established LC-MS/MS workflow offers a comprehensive analysis of a wide range of organic acids, acylcarnitines and acylglycines in urine to perform effective, rapid and sensitive semi-automated diagnosis of more than 80 IMDs.

OBJECTIVES: Presymptomatic detection of patients with rare diseases (RD), defined by a population frequency less than 1 : 2,000, is the task of newborn screening (NBS). In the Czech Republic (CZ), currently eighteen RD are screened: phenylketonuria/hyperphenylalaninemia (PKU/HPA), congenital hypothyroidism (CH), congenital adrenal hyperplasia (CAH), cystic fibrosis (CF), medium chain acyl-CoA dehydrogenase deficiency (MCADD), long chain 3-hydroxyacyl-CoA dehydrogenase deficiency (LCHADD), very long chain acyl-CoA dehydrogenase deficiency (VLCADD), carnitine palmitoyl transferase I and II deficiency (CPTID, CPTIID), carnitine-acylcarnitine translocase deficiency (CACTD), maple syrup urine disease (MSUD), glutaric aciduria type I (GA I), isovaleryl-CoA dehydrogenase deficiency (IVA), argininemia (ARG), citrullinemia (CIT), biotinidase deficiency (BTD), cystathionine beta-synthase-deficient homocystinuria (CBSD HCU), and methylenetetrahydrofolate reductase deficiency homocystinuria (MTHFRD HCU). The aim was to analyze the prevalence of RD screened by NBS in CZ. METHODS: We examined the NBS programme in CZ from 1 January 2010 to 31 December 2017, which covered 888,891 neonates. Dried blood spots were primarily analyzed using fluorescence immuno-assay, tandem mass spectrometry and fluorimetry. RESULTS: The overall prevalence of RD among the neonate cohort was 1 : 1,043. Individually, 1 : 2,877 for CH, 1 : 5,521 for PKU/HPA, 1 : 6,536 for CF (1 : 5,887 including false negative patients), 1 : 12,520 for CAH, 1 : 22,222 for MCADD, 1 : 80,808 for LCHADD, 1 : 177,778 for GA I, 1 : 177,778 for IVA, 1 : 222,223 for VLCADD, 1 : 296,297 for MSUD, 1 : 8,638 for BTD, and 1 : 181,396 for CBSD HCU. CONCLUSIONS: The observed prevalence of RD, based on NBS, corresponds to that expected, more precisely it was higher for BTD and lower for MSUD, IVA, CBSD HCU, MCADD and VLCADD. Early detection of rare diseases by means of NBS is an effective secondary prevention tool.

Introduction. The aim of prospective study was to evaluate the ability of echocardiography and cardiac biomarkers to predict in-hospital mortality and the risk of brain infarction during a 12-month follow-up period (FUP) with anticoagulation in pulmonary embolism (PE) patients. Methods. Eighty-eight consecutive acute PE patients (39 men, mean age 63 years) were enrolled; 78 underwent baseline echocardiography and brain magnetic resonance imaging (MRI). After a 12-month FUP, 58 underwent brain MRI. In-hospital mortality and the rates of new ischemic brain lesions (IBL) on MRI with clinical ischemic stroke (IS) events were predicted based on echocardiography (patent foramen ovale presence with right-to-left shunt – PFO/RLS; right/left ventricle diameter ratio – RV/LD; tricuspid annulus plane systolic excursion – TAPSE; tricuspid annulus systolic velocity – ST; pulmonary artery systolic pressure – PASP) and biomarkers results (amino-terminal fragment of brain natriuretic peptide – NT-proBNP and cardiac troponin T – cTnT). Results. Our series involved 88 patients, of whom 11 (12.5%) presented high-risk PE, 24 (27.3%) intermediate-high risk PE, 19 (21.6%) intermediate-low risk PE and 34 (38.6%) patients had low risk PE. Nine patients (10.2%) died during hospitalization including high-risk PE [6/9 (66.6%)] and intermediate-high-risk PE [3/24 (12.5%)]. cTnT [odds ratio (OR) 4.3; 95% confidence interval 0.59–31.3, P = 0.014], NT-proBNP (OR 14.2 [1.5–133.4], P = 0.02), RV/LD ≥0.79 (OR 36.6 [4.2–316.4], P = 0.001), TAPSE (OR 0.55 [0.34–0.92, P = 0.022) and PASP ≥51.5 mmHg (OR 33.3 [3.8–292.6], P = 0.022) were predictors of in-hospital mortality. Seventeen patients (19.3%) experienced IS (n = 8) or new IBL (n = 9). On multivariate analysis, PFO/RLS (OR 27.1 [3.0–245.3], P = 0.003) and ST ≤14.5 cm/s (OR 34.1 [CI 3.4–344.0], P = 0.003) were independent predictors of IS and IBL risk. Conclusions. High blood troponin T, NT-proBNP, RV dilatation/systolic dysfunction and pulmonary hypertension predicted in-hospital mortality. PFO/RLS presence and ST were predictors of clinically apparent/silent brain infarction.

Úvod: Cystatin C je považován za prognostický marker kardiovaskulárních onemocnění. Cílem této prospektivní práce bylo zhodnotit přínos hodnocení cystatinu C v prognostické stratifikaci k predikci nemocniční mortality a závažných nežádoucích kardiálních příhod (major cardiac adverse events, MACE) u pacientů s infarktem myokardu s elevacemi úseku ST (STEMI) a porovnat význam cystatinu C s dalšími ukazateli renálních funkcí a skóre GRACE (Global Registry of Acute Coronary Events). Metody: Do prospektivní studie bylo zařazeno 659 konsekutivních pacientů (479 mužů, průměrný věk 65 let) s akutním infarktem myokardu s elevacemi úseku ST léčených primární perkutánní koronární intervencí (PCI). Při přijetí byl proveden standardní laboratorní odběr zahrnující cystatin C, troponin T, N-terminální fragment natriuretického propeptidu typu B (NT-proBNP), další biomarkery renálních funkcí. Schopnost cystatinu C, dalších biomarkerů a skóre GRACE predikovat nemocniční mortalitu a MACE (rekurence akutního koronárního syndromu, iktus, in-stent trombóza, mortalita) byla hodnocena pomocí c-statistiky. Výsledky: Nemocniční mortalita dosáhla 4 % (n = 26), výskyt MACE pak 6,8 % (n = 45). Vstupní hodnota cystatinu C, kreatininu, urey, glomerulární filtrace, troponinu T, NT-proBNP a GRACE byla shledána jako nezávislý prognostický faktor. Sérová koncentrace cystatinu C a hodnota skóre GRACE byla signifikantně vyšší u zemřelých (1,65 ± 0,91 vs. 0,97 ± 0,41 mg/ml; p < 0,001, resp. 138 ± 43 vs. 99 ± 31; p < 0,001). Hodnota plochy pod křivkou (area under the curve, AUC) pro predikci mortality a MACE pro cystatin C a skóre GRACE byla 0,83 a 0,88, respektive 0,66 a 0,72 (vše p < 0,001) s optimální hraniční hodnotou 1,3 mg/ml pro cystatin C a 136 pro skóre GRACE. Hodnota cystatinu C nad mez 1,30 mg/l byla asociována s nejvyšším adjustovaným poměrem šancí (odds ratio, OR) 3,85 (95% interval spolehlivosti 2,36-6,38; p < 0,001) pro nemocniční mortalitu (senzitivita 77 %, specificita 86 %). Kombinace cystatinu C a skóre GRACE nevedla k dalšímu signifikantnímu zlepšení rizikové stratifikace (OR 1,05; 95% interval spolehlivosti 1,04-1,07; p < 0,001). Závěr: Cystatin C byl potvrzen jako prediktor časného výsledku srovnatelný se skóre GRACE u pacientů se STEMI.
Introduction: Cystatin C has been implicated as a prognostic marker in cardiovascular diseases. The aim of prospective study was to evaluate the benefits of measuring cystatin C for prognostic stratification to predict hospital mortality and the rates of major cardiac adverse events (MACE) in ST elevation myocardial infarction (STEMI) patients and to compare cystatin C to other markers of renal function and Global Registry of Acute Coronary Events (GRACE) score. Methods: A total of 659 consecutive patients (479 men, mean age 65 years) from a prospective study on acute STEMI treated by primary percutaneous coronary intervention (PCI) were evaluated. Standard laboratory tests including cystatin C, troponin T, NT-terminal fragment of brain natriuretic peptide (NT-proBNP), markers of renal function were assessed on admission in all patients. Using c-statistic, the ability of cystatin C, other biomarkers and GRACE score to predict hospital mortality and MACE (acute coronary syndrome recurrence, stroke event, definite in-stent thrombosis and mortality) rate was evaluated. Results: All-cause hospital mortality and MACE occurrence were 4% (n = 26), resp. 6.8% (n = 45). Cystatin C, creatinine, urea, glomerular filtration rate, troponin T, NT-proBNP and GRACE on admission were identified as significant prognostic risk markers. Serum cystatin C level and GRACE score were significantly higher in non-survivors (1.65 ± 0.91 vs. 0.97 ± 0.41 mg/mL; p < 0.001, resp. 138 ± 43 vs. 99 ± 31; p < 0.001). The area under the curve (AUC) values for mortality and MACE rate prediction for cystatin C and GRACE score were 0.83 and 0.88, respectively 0.66 and 0.72 (all p < 0.001) with optimal cut-off values of 1.3 mg/mL for cystatin C and 136 for GRACE score. Cystatin C above cut-off > 1.30 mg/L was associated with the highest adjusted odds ratio (OR) 3.85 (95% confidence interval 2.36-6.38; p < 0.001), and predicted in-hospital mortality with 77% sensitivity and 86% specificity. The addition of cystatin C to the GRACE score (OR 1.05, 95% confidence interval 1.04-1.07; p < 0.001) was not significantly associated with improved risk stratification. Conclusions: Cystatin C is a predictor of early outcome comparable with the GRACE score in patients with STEMI.

Novorozenecký screening dědičných metabolických poruch je preventivní celopopulační program, který umožňuje včasnou diagnostiku a účinnou léčbu pacientů s tímto typem vzácných nemocí a který podstatně zlepšuje kvalitu jejich života. Novorozenecké screeningové programy se díky pokrokům v technologiích a léčbě celosvětově rozšiřují. V České republice byl novorozenecký screening zahájen v roce 1975 vyhledáváním fenylketonurie, pro další dědičné metabolické poruchy se rozšiřoval v letech 2009 a 2016. Nyní se vyhledává 15 cílových dědičných metabolických poruch, screeningový program může zachytit i další onemocnění, která nejsou primárně vyhledávána. Základní metodou pro novorozenecký screening dědičných metabolických poruch je stanovení vybraných metabolitů pomocí tandemové hmotnostní spektrometrie nebo stanovení enzymových aktivit v suché krevní kapce. Správná technika odběru krve na speciální filtrační papírky je základním předpokladem pro získání relevantních výsledků. Jako všechny screeningové programy i novorozenecký screening poskytuje falešně pozitivní nálezy, pravděpodobnost falešné pozitivity při screeningu dědičných metabolických poruch je v současnosti 1:1000. Mnohem vzácnější jsou nálezy falešně negativní, ale i na ně je třeba v klinické praxi myslet. Od roku 2010 bylo novorozeneckým screeningem zachyceno celkem 268 pacientů s některou z cílových nemocí nebo klinicky relevantním stavem jako je deficit vitaminu B12 či parciální deficit biotinidázy. Celková incidence 15 dědičných metabolických poruch zjišťovaných novorozeneckým screeningem v populaci České republiky je přibližně 1:2500 a je srovnatelná s dobře fungujícími zahraničními screeningovými programy.
Newborn screening of inherited metabolic diseases is a preventative program; it enables an early diagnosis and effective treatment of patients with these rare diseases and thus improves their quality of life. Newborn screening programs are expanding worldwide due to the progress in technology and treatment. In the Czech Republic, the newborn screening program started by detecting phenylketonuria in 1975 and expanded gradually by addition of other inherited metabolic disorders in 2009 and 2016. At present 15 inherited metabolic disorders are searched for as primary targets, however, the screening program may detect other diseases that are not primarily searched for. Determination of metabolites by tandem mass spectrometry or of enzyme activities by fluorimetry in dried blood spots are the primary approaches for newborn screening of inherited metabolic disorders. The quality of dried blood spots is the prerequisite for achieving relevant results. Newborn screening may generate false positive results; the likelihood of false positivity in screening of inherited metabolic disorders is currently 1:1000. False negative results are rare, but it is necessary to think about such possibility. Since 2010, a total of 268 patients with targeted diseases or relevant condition such as vitamin B12 deficiency or partial biotinidase deficiency were detected. The overall incidence of 15 inherited metabolic disorders in newborn screening in the population of the Czech Republic is approximately 1:2500 and it is comparable to well-functioning screening programs abroad.
