INTRODUCTION: One of the most debilitating problems encountered by people with multiple sclerosis (MS) is the loss of balance and coordination. Our study aimed to comprehensively evaluate the effectiveness of one year of Tai-chi exercise in patients with MS using both subjective and objective methods, including posturography. METHODS: This was a single-group longitudinal one-year study performed from the 1st of January 2019 to the 1st of January 2020. The primary outcomes of interest were the Mini-Balance Evaluation Systems Test (Mini-BESTest) and static posturography measures as objective methods to detect subtle changes associated with postural control/balance impairment. Secondary outcomes were measures of depression, anxiety, cognitive performance, and quality of life. All objective and subjective parameters were assessed four times: at baseline, and after three, six and 12 months of regular Tai-chi training. The difference was calculated as a subtraction of baseline values from every timepoint value for each measurement. If the normality test was passed, parametric one-sample t-test was used, if failed, Wilcoxon signed ranks test was used to test the difference between the baseline and each timepoint. Alpha was set to 0.017 using Bonferroni correction for multiple comparisons. RESULTS: Out of 25 patients with MS enrolled, 15 women with MS (mean age 44.27 years) were included for statistical analyses after completing the 12-month program. After 12 months, significant improvements were found in all objective balance and gait tests: Mini-BESTest (p<0.001), static posturography measures (total area of the centre of foot pressure - TA; p = 0.015), 25 Feet Walk Test (25FWT; p = 0.001), anxiety (Beck Anxiety Inventory - BAI; p = 0.005) and cognition tests (Paced Auditory Serial Addition Test - PASAT; p = 0.003). Measures of depression (Beck Depression Inventory - BDI; p = 0.071), cognition (Symbol Digit Modalities Test - SDMT; p = 0.079), and health-related quality of life (European Quality of Life 5-Dimensions Questionnaire - EQ-5D-5L; p = 0.095) showed a trend of improvement but were not significant, which could be the result of a small sample and increased bias due the type II error. CONCLUSION: According to these preliminary results, this study indicates the possible beneficial effects of long-term Tai-chi training on patients with MS. Although these findings need to be confirmed by further studies with a larger sample of participants of both genders and require more rigorous randomized controlled trials (RCT) design, our findings support the recommendation of regular and long-term Tai-chi exercise in patients with MS. Rasmussenova encefalitída je zriedkavé chronické neurologické ochorenie, ktorého typický klinický obraz predstavujú: farmakorezistentná epilepsia, progredujúca atrofia jednej mozgovej hemisféry a progredujúci neurologický deficit. Postihuje predovšetkým deti, ale nevyhýba sa ani adolescentom a mladým dospelym. Neuropatologické a imunologické štúdie svedčia pre účasť T lymfocytmi sprostredkovaného cytotoxického zápalu s potenciálnou účasťou autoprotilátok a aktivovanej mikroglie v patologickom procese. Primárna príčina ochorenia však zostáva neobjasnená. Magnetická rezonancia mozgu je vhodný biomarker ochorenia, v tejto dobe nemáme spoľahlivé sérologické markery. Súčasné spôsoby imunomodulačnej liečby majú schopnosť zlepšiť a spomaliť priebeh ochorenia, ale dlhodobé východiská imunoterapie sú nejasné. Hemisferektómia predstavuje najúčinnejší spôsob liečby farmakorezistentnej epilepsie, ale nevyhnutné sú nežiaduce funkčné deficity. Rozhodnutie o chirurgickej liečbe môže byť problematické, ak chýba významnejší funkčný deficit a ide o staršieho pacienta. Pre zúčastnených – pacienta, jeho rodinu, včítane ošetrujúcich lekárov – je určenie správneho času, kedy sa posunúť od medikamentóznej liečby k chirurgickej liečbe, náročný rozhodovací proces. Vzhľadom na závažnú prognózu je nevyhnutné pacienta s podozrením na Rasmussenovu encefalitídu neodkladne odoslať do epileptologického centra.
Rasmussen's encephalitis is a rare chronic neurologic disease characterized by three main features: refractory epilepsy, progressive unihemispheric atrophy of the brain and progressive neurological deficit. This chronic focal encephalitis typically affects children but adolescents and adults are also involved. Neuropathological and immunological studies indicate that Rasmussen's encephalitis is probably driven by a T-cell cytotoxic response with potential additional contribution by autoantibodies and activated microglia. Primary cause of the disease remains unsolved. MRI may be a good biomarker in Rasmussen's encephalitis but we do not have relevant serological markers yet. Immunomodulatory treatments seem to slow disease progressiaon in Rasmussen's encephalitis but long-term outcomes remain unclear. Functional hemispherectomy remains the only effective treatment for seizures but there are inevitable functional compromises associated with it. The decision and timing of surgery may be problematic in the absence of a dense neurological deficit, and if the patient is older. For patients, their families and treating physicians, choosing the right time to move from medical management to surgery is a real therapeutic dilemma. Due to poor prognosis, patients with susspected Rasmussen's encephalitis should immediately be referred to an epilepsy centre.

Infekcie môžu u pacientov so sclerosis multiplex zhoršovať klinický stav. Vakcinácia môže mať preventívny účinok, ale súčasne sa môže stať spúšťačom nového ataku ochorenia. Doteraz neboli zjednotené názory na použitie vakcinácie pri tomto ochorení. Predkladaný prehľad podáva aktuálny pohľad na túto problematiku.
Infections can in patients with multiplex sclerosis cause impairment of their clinical status. Vaccination can have preventive effect, but at the same time it can become a trigger of a new attack of the disease. Up till now there has not been unified opinion about vaccination in this disease. The article presents a current view on this issue.

Multiple sclerosis (MS) is an inflammatory autoimmune disease occurring in genetically sensitive individuals. As migration of immune cells into the CNS is facilitated by the Very Late Antigen 4 (VLA-4) integrin molecule, the VLA4 gene may be considered as a plausible candidate genetic risk factor for susceptibility to MS. Therefore, the objective of our study was to investigate the association between two genetic polymorphisms located in the VLA4 gene and the risk of multiple sclerosis. One hundred seventeen MS patients and 165 control subjects from Slovakia were genotyped for VLA4 gene SNP polymorphisms at positions 269 (C/A) and 3061 (A/G). The same study cohorts were also genotyped for the rs3135388 polymorphism tagging the HLA-DRB1*15:01 allele, which is a known genetic factor associated with susceptibility to develop MS in many populations. Our findings show for the first time that the rs3135388 polymorphism is a strong risk factor for MS in the Slovak population. Investigation of the VLA4 gene polymorphisms revealed a significantly higher frequency of the 3061AG genotype in MS patients compared to the controls (P ≤ 0.05). We suggest that the 3061AG polymorphic variant is an independent genetic risk factor for MS development in our population as it was significantly associated with this disease. The association was also confirmed after applying multivariate logistic-regression analysis adjusted for gender, age and HLA-DRB1*15:01 positivity as possible influencing factors.

[Progressive necrotizing myelopathy - case report]
Progresívna nekrotizujúca myelopatia je v rámci nekompresívnych myelopatií veľmi zriedkavé ochorenie. Charakteristickým klinickým nálezom je miechová symptomatológia s dominujúcim postihnutím periférneho motoneurónu. V likvore je typický nález so zvýšenými celkovými bielkovinami a neprítomnými oligoklonálnymi pásmi, niekedy i s miernou pleiocytózou. Špecifickým MR kritériom je rozšírenie aspoň troch a viacerých susedných vertebrálnych segmentov miechy s hypersignálom v TW2 obraze a s enhancementom po kontrastnej látke v priebehu mesiacov vystriedané atrofiou, čo zodpovedá nekróze miechy potvrdenej pri bioptickom vyšetrení. Diferenciálna diagnostika je široká. Treba vylúčiť okrem častejších ochorení (SM, zápalové, neoplastické, systémové, cievne a metabolické príčiny myelopatie ) aj zriedkavé paraneoplastické postihnutie miechy. V najnovších prácach sa zvažuje autoimunitný patomechanizmus so vzťahom ku neuromyelitis optica na základe výskytu špecifických NMO IgG protilátok proti aquaporínu 4. Ochorenie má progresívny charakter s možným terapeutickým efektom imunomodulačnej liečby. Autori prezentujú kazuistiku 67-ročného pacienta s výrazne atopickým terénom pri imunologickom vyšetrení, spĺňajúceho klinické aj MR kritériá progresívnej nekrotizujúcej myelopatie. Liečba kortikoidmi bola bez efektu, preto nasadená liečba intravenóznymi imunoglobulínmi, ktorá viedla ku prechodnému zlepšeniu klinického stavu so zlepšením ťažkého motorického deficitu. Dva roky po začiatku manifestácie neurologickej symptomatológie bol u pacienta diagnostikovaný konvenčný typ karcinómu z buniek obličky gr II WHO, prečo i napriek opakovane negatívnym nálezom antineuronálnych protilátok musíme zvažovať možný paraneoplastický pôvod progresívnej nekrotizujúcej myelopatie.
Progressive necrotizing myelopathy is an extremely rare disease, even considered among non-compressive myelopathies. Spinal cord symptoms are characteristic of it, with the peripheral motor neuron severely affected. A typical cerebrospinal fluid finding is increased total protein without oligoclonal bands, sometimes with mild pleocytosis. A specific MRI criterion is the involvement of at least three or more neighbouring vertebral spinal cord segments with hypersignal in the TW2 picture and gadolinium enhancement, evolving over months into atrophy, which corresponds to spinal cord necrosis confirmed on bioptic examination. The range of differential diagnosis is wide. Apart from other more common diseases (multiple sclerosis, inflammatory, neoplastic, systemic, vascular and metabolic causes of myelopathy), rare paraneoplastic spinal cord involvement must also be excluded. Recently, autoimmune pathogenetic mechanisms related to neuromyelitis optica have been considered, based on the occurrence of specific NMO IgG antibodies against aquaporin 4. The disease has a progressive course, although immunomodulatory treatment may possibly prove therapeutic. The authors present the case report of a 67-year-old man with noticeably atopic terrain revealed on immunological examination, fulfilling clinical and MRI criteria for progressive necrotizing myelopathy. Treatment with corticoids had no effect and was therefore followed by intravenous immunoglobulins, which resulted in a temporary improvement in the clinical condition and an improvement in the serious motor deficiency. Two years after the first manifestation of neurological symptoms, a conventional type of carcinoma was diagnosed in the patient from gr. II WHO kidney cells. As a consequence, in spite of repeated negative findings for antineuronal antibodies, we have to consider a possible paraneoplastic origin for the progressive necrotizing myelopathy.
