INTRODUCTION: Among children born small for gestational age, 10-15% fail to catch up and remain short (SGA-SS). The underlying mechanisms are mostly unknown. We aimed to decipher genetic aetiologies of SGA-SS within a large single-centre cohort. METHODS: Out of 820 patients treated with growth hormone (GH), 256 were classified as SGA-SS (birth length and/or birth weight <-2 SD for gestational age and life-minimum height <-2.5 SD). Those with the DNA triplet available (child and both parents) were included in the study (176/256). Targeted testing (karyotype/FISH/MLPA/specific Sanger sequencing) was performed if a specific genetic disorder was clinically suggestive. All remaining patients underwent MS-MLPA to identify Silver-Russell syndrome, and those with unknown genetic aetiology were subsequently examined using whole-exome sequencing or targeted panel of 398 growth-related genes. Genetic variants were classified using ACMG guidelines. RESULTS: The genetic aetiology was elucidated in 74/176 (42%) children. Of these, 12/74 (16%) had pathogenic or likely pathogenic (P/LP) gene variants affecting pituitary development (LHX4, OTX2, PROKR2, PTCH1, POU1F1), the GH-IGF-1 or IGF-2 axis (GHSR, IGFALS, IGF1R, STAT3, HMGA2), 2/74 (3%) the thyroid axis (TRHR, THRA), 17/74 (23%) the cartilaginous matrix (ACAN, various collagens, FLNB, MATN3), and 7/74 (9%) the paracrine chondrocyte regulation (FGFR3, FGFR2, NPR2). In 12/74 (16%), we revealed P/LP affecting fundamental intracellular/intranuclear processes (CDC42, KMT2D, LMNA, NSD1, PTPN11, SRCAP, SON, SOS1, SOX9, TLK2). SHOX deficiency was found in 7/74 (9%), Silver-Russell syndrome in 12/74 (16%) (11p15, UPD7), and miscellaneous chromosomal aberrations in 5/74 (7%) children. CONCLUSIONS: The high diagnostic yield sheds a new light on the genetic landscape of SGA-SS, with a central role for the growth plate with substantial contributions from the GH-IGF-1 and thyroid axes and intracellular regulation and signalling.

Familial short stature (FSS) describes vertically transmitted growth disorders. Traditionally, polygenic inheritance is presumed, but monogenic inheritance seems to occur more frequently than expected. Clinical predictors of monogenic FSS have not been elucidated. The aim of the study was to identify the monogenic etiology and its clinical predictors in FSS children. Of 747 patients treated with growth hormone (GH) in our center, 95 with FSS met the inclusion criteria (pretreatment height ≤-2 SD in child and his/her shorter parent); secondary short stature and Turner/Prader-Willi syndrome were excluded criteria. Genetic etiology was known in 11/95 children before the study, remaining 84 were examined by next-generation sequencing. The results were evaluated by American College of Medical Genetics and Genomics (ACMG) guidelines. Nonparametric tests evaluated differences between monogenic and non-monogenic FSS, an ROC curve estimated quantitative cutoffs for the predictors. Monogenic FSS was confirmed in 36/95 (38%) children. Of these, 29 (81%) carried a causative genetic variant affecting the growth plate, 4 (11%) a variant affecting GH-insulin-like growth factor 1 (IGF1) axis and 3 (8%) a variant in miscellaneous genes. Lower shorter parent's height (P = 0.015) and less delayed bone age (BA) before GH treatment (P = 0.026) predicted monogenic FSS. In children with BA delayed less than 0.4 years and with shorter parent's heights ≤-2.4 SD, monogenic FSS was revealed in 13/16 (81%) cases. To conclude, in FSS children treated with GH, a monogenic etiology is frequent, and gene variants affecting the growth plate are the most common. Shorter parent's height and BA are clinical predictors of monogenic FSS.

Přibližně u 10 % dětí, které se narodily malé na svůj gestační věk (SGA; small for gestational age) s porodní hmotností a/nebo délkou nižší než −2 SD, nedojde po narození k růstovému výšvihu. Tyto děti zůstávají malé po celé dětství (s výškovým skóre směrodatné odchylky [ht-SDS] pod −2,5 SDS) a skončí jako malí dospělí. Označujeme je jako SGA-SS (small for gestational age with short stature). Evropská léková agentura schválila již v roce 2003 SGA-SS jako indikaci pro léčbu růstovým hormonem. Z 1502 dětí SGA-SS evidovaných v české národní databázi příjemců růstového hormonu REPAR již 397 dětí (221 chlapců) ukončilo růst a dosáhlo téměř finální dospělé tělesné výšky. Léčba růstovým hormonem v dávce 0,034 mg/kg/den (medián) byla zahájena u chlapců ve věku 8,5 roku (medián; P5-95: 3,2–15,0), u dívek ve věku 7,3 roku (3,9–13,5). Průměr ht-SDS při zahájení léčby byl −3,21 (SD: 0,91). Léčba byla ukončena u chlapců ve věku 16,2 roku (medián; P5-95: 14,0–18,7 roku), u dívek ve věku 14,3 roku (12,4–17,3). Léčba vedla do okamžiku ukončení léčby k průměrnému přírůstku ht-SDS o 1,59 a k dosažení téměř dospělé výšky (near-final height) ht-SDS −1,62 (SD: 0,97; p < 0,0001 proti výšce před léčbou). Při ukončení léčby tedy většina pacientů dosáhla výšky nad −2 SDS, v rozmezí hodnot běžné populace. Po adjustaci na ht-SDS v 18 letech věku tato výška odpovídá ht-SDS −2,30 (SD: 1,09; p < 0,0001 proti výšce před léčbou). I po této adjustaci, která efekt léčby mírně podhodnocuje, byla léčba růstovým hormonem úspěšná a vedla ke zlepšení v průměru o 0,91 ht-SDS. Výsledky analýzy z databáze REPAR prokazují u dětí SGA-SS účinnost léčby růstovým hormonem a její potenciál významně zlepšit dospělou tělesnou výšku.
Of children born small for gestational age (SGA) with birth weight and/or birth length below −2 SD, about 10% fail to catch-up after birth. These children remain short over childhood (with height standard deviation score [ht-SDS] below −2.5 SDS) and end up as short adults. They are assigned as SGA-SS (small for gestational age with short stature). European Medicine Agency approved growth hormone therapy in SGA-SS already in 2003. Out of 1502 SGA-SS children that are registered in the Czech national database of growth hormone recipients REPAR, 397 children (221 boys) already completed growth and achieved near-final height. Growth hormone therapy (median dose 0.034 mg/kg/day) was initiated at age 8.5 years in boys (median; P5-95: 3.2-15.0) and 7.3 years in girls (3.9-13.5). The mean ht-SDS at treatment initiation was −3.21 (SD: 0.91). Treatment was terminated at age 16.2 years in boys (median; P5-95: 14.0-18.7) and 14.3 years in girls (12.4-17.3). By treatment termination, the patients have had gained 1.59 ht-SDS (mean) and have achieved near-final height −1.62 ht-SDS (SD: 0.97; p<0.0001 if compared to pre-treatment). Thus, majority of patients exceeded −2 SDS at treatment completion and achieved height within normal limits. After adjusting for ht-SDS at age 18 years, their height corresponded to ht-SDS −2.30 (SD: 1.09; p < 0.0001 if compared to pre-treatment). Growth hormone therapy was successful even after this adjustment that mildly underestimates the treatment effect, and led to mean height increase of 0.91 ht-SDS. Analysis results of the REPAR database confirm efficacy of growth hormone therapy in SGA-SS children and its potential to significantly improve the adult height.
O úspěchu léčby růstovým hormonem a dosažení optimální dospělé výšky rozhoduje jednak růst před zahájením puberty a jednak vydatnost pubertálního růstového výšvihu. U kohorty 397 dětí (221 chlapců) z české národní databáze příjemců růstového hormonu REPAR, které se narodily malé na svůj gestační věk, měly malou výšku v dětství (SGA-SS) a již dosáhly své téměř dospělé výšky, jsme vyhodnotili vliv prepubertální a pubertální složky růstu na celkový efekt léčby růstovým hormonem. Odděleně jsme posoudili děti také podle výšky rodičů (oba nad −2 SD výšky, jeden pod −2 SD výšky, oba pod −2 SD výšky) a podle porodních parametrů – nízké porodní hmotnosti, nízké porodní délky či kombinace obojího. Léčba růstovým hormonem vedla do okamžiku ukončení léčby k průměrnému přírůstku ht-SDS o 1,59, z toho za prepubertální období o 1,57. Během puberty se již výška při pokračující léčbě nezlepšila. Po adjustaci na ht-SDS v 18 letech věku stoupla výška ve srovnání se stavem při zahájení léčby o 0,91 ht-SDS (všechny tři parametry p < 0,0001 proti výšce před léčbou). Významně více vyrostly děti s oběma rodiči vyššími než −2 SD a také děti, které měly sníženou výlučně porodní hmotnost, nikoliv porodní délku. Výsledky analýzy z databáze REPAR prokazují účinnost léčby růstovým hormonem u dětí SGA-SS. O efektu léčby rozhoduje především prepubertální složka růstu, proto důsledné sledování růstu dětí narozených SGA a včasné zahájení léčby v časném dětství má pro ně klíčový význam.
Both pre-pubertal growth and magnitude of the pubertal growth spurt are the prerequisites of potential of growth hormone therapy to optimize final height. We analyzed impact of pre-pubertal and pubertal growth components on the total effect of growth hormone therapy in a cohort of 397 children (221 boys) registered in the Czech national database of growth hormone recipients REPAR. All were born small for gestational age, had persistent short stature in childhood (SGA-SS) and already achieved their near-final height. We separately evaluated children according to parental heights (with both parents over −2 ht-SDS, one below −2 ht-SDS, both below −2 ht-SDS) and according to their size at birth – low birth weight, low birth length, low both parameters. Up to treatment completion, treatment led to a height gain of 1,59 ht-SDS; of this, the pre-pubertal gain was 1,57 ht-SDS. Thus, ht-SDS remained unchanged within puberty, despite continuous growth hormone administration. The final ht-SDS gain adjusted for age 18 years was 0,91 (all three parameters p<0.0001 if compared to pre-treatment). Children with both parents taller than −2 SD and children with exclusively low birth weight had significantly better treatment outcomes. Analysis results of the REPAR database confirm efficacy of growth hormone therapy in SGA-SS children. The treatment success is based on pre-pubertal growth component. Therefore, a careful follow-up of children born SGA and a timely initiation of growth hormone therapy in early childhood are of crucial importance.
Během posledních let došlo k výraznému nárůstu nových případů dětí a adolescentů s nesouladem rodu a pohlaví. Většina z nich je ve věku 15–17 let. Mnozí jedinci vyhledávají na doporučení sexuologa endokrinologickou péči. Příčina stoupajícího trendu není zcela jasná, liší se i některé postoje k navrženým postupům. Tento přehledový článek shrnuje současné přístupy a názory na péči o děti a adolescenty s nesouladem rodu a pohlaví, uvádí též současný stav v České republice.
In recent years, there has been a significant increase in new cases of children and adolescents with gender incongruence. Most of them are aged 15-17. Many individuals seek endocrine care on the recommendation of a sexologist. The cause of the rising trend is not entirely clear, some attitudes to the proposed procedures also differ. This overview article summarizes current approaches and opinions on the care of children and adolescents with gender incongruence, and also presents the current situation in the Czech Republic.
BACKGROUND: In Turner syndrome (TS), fluorescent in situ hybridization (FISH) karyotyping offers an alternative to classical karyotyping. OBJECTIVE: We tested the added value of FISH karyotyping from lymphocytes (mesodermal origin), buccal cells (ectodermal origin), and a rear-tongue smear (endodermal origin) to determine the 45,X cell line fraction and its impact on patient phenotype. DESIGN AND PATIENTS: Classical karyotyping and three FISH assays were done in 153 girls and women previously diagnosed with TS in four university hospitals. The 45,X cell line fraction was determined for each method and correlated with the major phenotypic signs. RESULTS: Classical karyotyping identified 45,X/46,XX mosaicism in 77/153 subjects (50%), 45,X monosomy in 52/153 (34%), and other karyotypes in 24/153 (16%). FISH from lymphocytes verified 45,X in 47/52 original cases, whereas 4/52 had 45,X/46,XX and 1/52 45,X/47,XYY mosaicism. The 45,X cell line fraction was higher in FISH from lymphocytes compared to classical karyotyping (median 86.4% vs. 70.0%; p < 0.001), while there was no difference for FISH from buccal or rear-tongue smear cells. The mean 45,X cell line fraction was more abundant in patients with several of the characteristic phenotypic signs compared to patients without them (p < 0.01), but the predictive power was insufficient. CONCLUSION: FISH analysis confirmed the findings of classical karyotyping; only a few 45,X monosomy cases were reclassified as mosaics. The 45,X cell line fraction did not show clinically meaningful prediction of the phenotype. FISH analysis of buccal or rear-tongue epithelial cells may be a non-inferior, less invasive alternative to classical karyotyping.
Pohlavní/rodová neshoda v dětství znamená nesoulad mezi prožívaným rodem a zevními pohlavními znaky, který může být zjevný již u prepubertálních dětí. Nárůst počtu dospívajících, kteří zpochybňují svou rodovou identitu, zaznamenaný v posledních letech vyvolává oprávněný zájem s cílem porozumět důvodům a zvolit správný medicínský postup pro daného jedince. V souhrnném článku autoři poskytují přehled problematiky v České republice.
Gender incongruence in childhood refers to the mismatch between a child‘s individual conception of their own gender and their assigned sex at birth based on their physical sex characteristics. This condition may present as early as in the prepubertal age. The upsurge in the number of adolescents questioning their gender identity in recent years has generated pronounced interest to understand the basis of this incongruity and subsequently provide appropriate medical support. In the following review article, the authors provide a background about this subject in the Czech Republic.
INTRODUCTION: The growth hormone deficiency (GHD) diagnosis is controversial especially due to low specificity of growth hormone (GH) stimulation tests. It is therefore believed that children diagnosed with GHD form a heterogeneous group with growth disorder frequently independent on GH function. No study evaluating the complex etiology of growth failure in children with diagnosed GHD has been performed thus far. AIMS: To discover genetic etiology of short stature in children with diagnosed GHD from families with short stature. METHODS: Fifty-two children diagnosed with primary GHD and vertically transmitted short stature (height SDS in the child and his/her shorter parent <-2 SD) were included to our study. The GHD diagnosis was based on growth data suggestive of GHD, absence of substantial disproportionality (sitting height to total height ratio <-2 SD or >+2 SD), IGF-1 levels <0 for age and sex specific SD and peak GH concentration <10 ug/L in two stimulation tests. All children were examined using next-generation sequencing methods, and the genetic variants were subsequently evaluated by American College of Medical Genetics standards and guidelines. RESULTS: The age of children at enrollment into the study was 11 years (median, IQR 9-14 years), their height prior to GH treatment was -3.0 SD (-3.6 to -2.8 SD), IGF-1 concentration -1.4 SD (-2.0 to -1.1 SD), and maximal stimulated GH 6.3 ug/L (4.8-7.6 ug/L). No child had multiple pituitary hormone deficiency or a midbrain region pathology. Causative variant in a gene that affects growth was discovered in 15/52 (29%) children. Of them, only 2 (13%) had a genetic variant affecting GH secretion or function (GHSR and OTX2). Interestingly, in 10 (67%) children we discovered a primary growth plate disorder (ACAN, COL1A2, COL11A1, COL2A1, EXT2, FGFR3, NF1, NPR2, PTPN11 [2x]), in one (7%) a genetic variant impairing IGF-1 action (IGFALS) and in two (12%) a variant in miscellaneous genes (SALL4, MBTPS2). CONCLUSIONS: In children with vertically transmitted short stature, genetic results frequently did not correspond with the clinical diagnosis of GH deficiency. These results underline the doubtful reliability of methods standardly used to diagnose GH deficiency.

Cíl studie: Použití léčebného ozáření a vysokodávkované chemoterapie je spojeno s rizikem časných i pozdních komplikací. Postižení štítné žlázy je známým problémem doprovázejícím alogenní transplantaci kmenových buněk krvetvorby (HSCT – hematopoietic stem cell transplantation). Cílem této práce je zhodnotit výskyt a projevy postižení štítné žlázy u pacientů, kteří v našem centru podstoupili alogenní transplantaci v dětském věku a adolescenci (Transplantační jednotka kostní dřeně, Klinika dětské hematologie a onkologie, Fakultní nemocnice Motol, Praha). Pacienti a metody: Jedná se o retrospektivní analýzu dat pacientů, kteří podstoupili HSCT v dětském věku a v adolescenci a následně u nich bylo diagnostikováno postižení štítné žlázy. Funkci štítné žlázy jsme hodnotili klinicky a laboratorně v době před transplantací, šest měsíců a každý rok po transplantaci. Sonografické vyšetření bylo prováděno u pacientů s prokázanou tyreopatií a preventivně pak v delším časovém odstupu po transplantaci (více než pět let, zejména u skupiny ozářených pacientů). Zhodnotili jsme data 463 pacientů (295 mužů, 168 žen), transplantovaných v mediánu věku 7,8 roku (rozmezí 0,1–20,5) v letech 1989 až 2018. V době hodnocení přežíval každý z nich nejméně jeden rok po transplantaci. Pro maligní onemocnění bylo léčeno 301 pacientů (65 %). Jedna třetina pacientů měla jako součást předtransplantační přípravy celotělové ozáření (CTO), přípravný režim zahrnující použití Busulfanu byl uplatněn u téměř 50 % pacientů. Nepříbuzného dárce mělo 327 pacientů, u 33,4 % byl dárcem shodný sourozenec nebo jiný rodinný dárce. Výsledky: Postižení štítné žlázy bylo diagnostikováno u 169 pacientů (36,5 % z celkového počtu) v mediánu dva roky (rozmezí: 0,4–21,3 roku) po HSCT. Primární hypotyreóza byla nejčastějším postižením (110 pacientů, 65,1 %), klinické příznaky mělo pouze devět pacientů. Zvýšené hladiny autoprotilátek (aTPO, aTG) byly detekovány u 95 pacientů. Uzly štítné žlázy byly v rámci ultrazvukového vyšetření nalezeny u 28 pacientů, v mediánu 11,5 roku (5,1–23,4) po transplantaci, 14 se již léčilo pro poruchu funkce štítné žlázy (hypotyreóza = 8, autoimunitní onemocnění štítné žlázy – AITD = 6). U osmi pacientů (1,7 % z celkového počtu, 28,5 % pacientů s uzlem) byl diagnostikován karcinom štítné žlázy v mediánu 10,8 roku (rozmezí 5,4–21,5 let) po transplantaci, sedm pacientů z osmi mělo přípravný režim s CTO. Závěry: Alogenní transplantace kmenových buněk krvetvorby je léčebnou modalitou pro řadu maligních i nemaligních onemocnění. Se stále se zlepšujícími výsledky léčby je nyní pozornost věnována také oblasti pozdních následků léčby u dlouhodobě přežívajících pacientů. Postižení štítné žlázy je jednou z nejčastějších komplikací. Je proto nezbytné pravidelně (po 6–12 měsících) kontrolovat a vyšetřovat funkci štítné žlázy klinicky a zejména laboratorními parametry. Riziko sekundárních malignit těchto pacientů roste v čase. Pečlivé sledování zahrnující ultrazvukové vyšetření (za 1–3 roky) je velmi důležité pro časnou detekci nádorů, zejména u pacientů po celotělovém záření.
Radiotherapy and high-dose chemotherapy can cause a lot of early and late effects. Thyroid dysfunction is a known late complication following HSCT (hematopoietic stem cell transplantation). The objective of this study was to analyze the occurrence and characteristics of thyroid dysfunction in patients who have undergone HSCT in childhood and adolescence at our center (HSCT Unit, Department of Pediatric Hematology and Oncology, University Hospital Motol, Prague). Patients and methods: A retrospective review of clinical records was conducted for patients who had undergone allogeneic HSCT in childhood and adolescence, and subsequently were diagnosed with thyroid involvement. Thyroid gland function was evaluated in regular terms (pre-HSCT, six months, one year and then annually after HSCT). Ultrasound examination was performed in patients with thyroid gland involvement and as a screening. We have evaluated data in 463 patients (295 male, 168 female) who underwent allogeneic transplantation at a median age of 7.8 years (range: 0.1–20.5) from 1989 till 2018 and were alive more than one year after transplantation at the time of evaluation. Three hundred one patient (65%) were treated for malignant disease. In one‑third of patients who underwent HSCT, total body irradiation (TBI) conditioning regimen was used, almost 50% of patients underwent Busulphan based condition regimen. Three hundred twenty-seven transplantations (66.6%) were performed from an unrelated donor, 33.4% from a matched sibling or other family donor. Results: One hundred sixty-nine patients (36.5%) were diagnosed with thyroid gland involvement at a median of 2.0 (range: 0.4–21.3) years after HSCT. Primary hypothyroidism was the most common type of thyroid dysfunction (110 patients – 65.1%), only nine of them were with clinical signs. Thyroid anti-bodies (aTPO, aTG) were detected in 95 patients. Thyroid nodules were found during ultrasound examination in 29 patients at a median of 11.5 (range 5.1–23.4) years after HSCT, fifteen patients were treated due to thyroid dysfunction (hypo = 8, AITD = 7). Thyroid carcinoma was diagnosed in 8 patients (in 1.7% of all patients, 27.6% of patients with nodules) at a median of 10.8 (range 5.4–21.5) years after HSCT. All but one had received TBI based conditioning regimen. Conclusions: Allogeneic hematopoietic stem cell transplantation is a potentially curative therapy for a variety of malignant and non-malignant disorders. With the improved outcome, increasing attention has been drawn to late complications in long-term survivors. Thyroid dysfunction is one of the most frequent complications. Regular evaluation of thyroid gland function (every 6–12 months) including laboratory parameters is highly recommended. The risk of secondary malignancies after HSCT is increasing within time. Careful follow-up of thyroid status including annual ultrasound examination every 1–3 years, is very important for early detection of tumor, namely in all patients exposed to TBI.
