OBJECTIVES: To determine the treatment effect of corticosteroids in periradicular therapy (PRT) for radicular pain and to compare different types of corticosteroids and ozone. We also examined the effect in different indication groups for periradicular therapy for each type of treatment agent. BACKGROUND: Various studies have examined the therapeutic value of periradicular infiltration using treatment agents consisting of local anesthetic and corticosteroids or ozone application for radicular pain. This is the first study to compare different types of corticosteroids and ozone. METHODS: Eligible patients with radicular pain who failed conservative management were divided into five indication groups and prospectively followed to assess the PRT effect of corticosteroids or ozone application. PRT was performed under computer tomography (CT) monitoring. A set of three PRT applications in three weeks was applied and the outcome was evaluated using a visual analogue score for back and leg pain. The in-group and between-group treatment effect was tested using the Wilcoxon signed-rank test and the Kruskal-Wallis H-test with Dunn's post-hoc tests, respectively. The dependency between treatment effectiveness and indication for each group was tested using the Kruskal-Wallis H-test and Dunn's post-hoc tests. RESULTS: We prospectively followed 150 patients, randomized into three groups of 50 patients each. The follow-up rate was 100%. All three treatment agents showed a statistically significant treatment effect (P<0.001). The statistically significant effect was higher in betamethasone (Diprophos) versus methylprednisolone (Depomedrone) (P=0.019) and Diprophos versus ozone (P<0.001). Diprophos also showed the highest decrease of VAS after therapy versus VAS prior to therapy (median decrease = 4) compared to Depomedrone and ozone (median decrease = 3 and 2, respectively). The statistically significant outcome was better with the indication of spondylolisthesis and disc herniation (P=0.019) indication for the Diprophos group and between spinal stenosis and spondylolisthesis (P=0.022) and spondylolisthesis and disc herniation (P=0.016) for the ozone group. CONCLUSION: Clinical improvement occurred in all three groups but Diprophos showed the statistically best treatment effect compared to Depomedrone and ozone. Disc herniation resulting in radicular pain had a statistically significant better effect in comparison with spondylolisthesis in the Diprophos and ozone groups, but the ozone group showed heterogeneity depending on treatment effect and indication.

BACKGROUND: The fixed dose combination of calcipotriene (CAL) and betamethasone dipropionate (BDP) is a well-established topical treatment option for psoriasis based on strong scientific rationale for the single agents having complementary efficacy and safety. CAL/BDP PAD-cream is an easily spreadable cream based on PAD TechnologyTM, an innovative formulation and drug delivery system. OBJECTIVES AND METHODS: A Phase 3, multicentre, randomized, investigator-blind, active and vehicle-controlled trial enrolling 490 patients with mild to moderate psoriasis according to the Physician Global Assessment (PGA) scale was conducted in three European countries. Products were applied once daily for 8 weeks. The aim of the trial was to evaluate the efficacy and safety of CAL/BDP PAD-cream as well as treatment acceptability compared to CAL/BDP gel and PAD-cream vehicle. Primary endpoint was percentage change in modified Psoriasis Area and Severity Index (mPASI) from baseline to Week 8. RESULTS: The percentage mean change from baseline to Week 8 in mPASI for CAL/BDP PAD-cream (67.5%) was superior compared to PAD-cream vehicle (11.7%; p < 0.0001) and non-inferior to CAL/BDP gel (63.5%). The proportion of patients achieving PGA treatment success (at least two-step improvement to clear or almost clear) after 8 weeks was superior for CAL/BDP PAD-cream (50.7%) compared to PAD-cream vehicle (6.1%, p < 0.0001) and statistically significantly greater than CAL/BDP gel (42.7%, p = 0.0442). Patient-reported psoriasis treatment convenience score (PTCS) for CAL/BDP PAD-cream was rated superior to CAL/BDP gel at Week 8 (p < 0.0001) and the mean change in DLQI from baseline to Week 8 improved statistically significantly more in the CAL/BDP PAD-cream group compared to both PAD-cream vehicle (p < 0.0001) and CAL/BDP gel (p = 0.0110). Safety assessments during the trial demonstrated that CAL/BDP PAD-cream was well-tolerated. CONCLUSION: CAL/BDP PAD-cream is a novel topical treatment of psoriasis that has a high efficacy and a favourable safety profile combined with a superior patient-reported treatment convenience.

of recommendationsCorticosteroids should be administered to women at a gestational age between 24+0 and 33+6 weeks, when preterm birth is anticipated in the next seven days, as these have been consistently shown to reduce neonatal mortality and morbidity. (Strong-quality evidence; strong recommendation). In selected cases, extension of this period up to 34+6 weeks may be considered (Expert opinion). Optimal benefits are found in infants delivered within 7 days of corticosteroid administration. Even a single-dose administration should be given to women with imminent preterm birth, as this is likely to improve neurodevelopmental outcome (Moderate-quality evidence; conditional recommendation).Either betamethasone (12 mg administered intramuscularly twice, 24-hours apart) or dexamethasone (6 mg administered intramuscularly in four doses, 12-hours apart, or 12 mg administered intramuscularly twice, 24-hours apart), may be used (Moderate-quality evidence; Strong recommendation). Administration of two "all" doses is named a "course of corticosteroids".Administration between 22+0 and 23+6 weeks should be considered when preterm birth is anticipated in the next seven days and active newborn life-support is indicated, taking into account parental wishes. Clear survival benefit has been observed in these cases, but the impact on short-term neurological and respiratory function, as well as long-term neurodevelopmental outcome is still unclear (Low/moderate-quality evidence; Weak recommendation).Administration between 34 + 0 and 34 + 6 weeks should only be offered to a few selected cases (Expert opinion). Administration between 35+0 and 36+6 weeks should be restricted to prospective randomized trials. Current evidence suggests that although corticosteroids reduce the incidence of transient tachypnea of the newborn, they do not affect the incidence of respiratory distress syndrome, and they increase neonatal hypoglycemia. Long-term safety data are lacking (Moderate quality evidence; Conditional recommendation).Administration in pregnancies beyond 37+0 weeks is not indicated, even for scheduled cesarean delivery, as current evidence does not suggest benefit and the long-term effects remain unknown (Low-quality evidence; Conditional recommendation).Administration should be given in twin pregnancies, with the same indication and doses as for singletons. However, existing evidence suggests that it should be reserved for pregnancies at high-risk of delivering within a 7-day interval (Low-quality evidence; Conditional recommendation). Maternal diabetes mellitus is not a contraindication to the use of antenatal corticosteroids (Moderate quality evidence; Strong recommendation).A single repeat course of corticosteroids can be considered in pregnancies at less than 34+0 weeks gestation, if the previous course was completed more than seven days earlier, and there is a renewed risk of imminent delivery (Low-quality evidence; Conditional recommendation).

V současné době je pro udržovací léčbu astmatu k dispozici celá řada inhalačních léků a jejich kombinací v různých inhalačních systémech. V květnu 2020 byl Evropskou lékovou agenturou (EMA) schválen pro léčbu astmatu léčivý přípravek Atectura® Breezhaler®, který obsahuje fixní dvojkombinaci indakaterol/mometason furoát a je podáván jednou denně prostřednictvím práškového inhalátoru.
Currently, a variety of inhaled drugs and its combinations in different types of inhaler devices exist on the market that are available for the maintenance treatment of asthma. In May 2020, the European Medicines Agency (EMA) approved Atectura® Breezhaler® containing fixed combination of indacaterol and mometasone furoate administered once daily via a dry powder inhaler for the treatment of asthma.

Primární kožní lymfomy (PCL, primary cutaneous lymphomas) jsou nehodgkinské lymfomy, pro které je charakteristická přítomnost maligních T nebo B buněk v kůži. Nejčastějším typem PCL je mycosis fungoides (MF). Typicky má onemocnění mnohaletý vývoj, ve kterém rozlišujeme stadium skvrn, plaků a tumorů. V pokročilých stadiích choroby může dojít k šíření maligních buněk z kůže do lymfatických uzlin, vnitřních orgánů nebo periferní krve. Zásadní pro stanovení diagnózy MF je klinický obraz a histologické vyšetření. Léčebné možnosti u pacientů s mycosis fungoides lze rozdělit na terapii cílenou na kůži (SDT, skin directed therapy) a systémovou léčbu. V kazuistice pacientky sledované pro mycosis fungoides je popsán průběh onemocnění a využití léčebných metod a jejich kombinací.
Primary cutaneous lymphomas (PCL) are non-hodgkin lymphomas which are characterised by presence of malignant T-cells or B-cells in the skin. The disease has typically long-term development, where we differentiate three clinical stages – patches, plaques and tumours. In advanced stages of the disease malignant cells can spread from skin to lymphatic nodes, visceral organs or peripheral blood. MF diagnosis is established through clinical picture and crucially through histology. Treatment modalities of patients with MF can be divided into skin directed therapy (SDT) and systemic treatment. The case report of female patient with mycosis fungoides descri- bes the course of disease and use of treatment modalities and their combinations.

Geriatričtí pacienti trpící psoriázou bývají s terapií často nespokojeni a vyzkoušeli již různé způsoby terapie. Tato případová studie popisuje 72letého pacienta trpícího psoriázou od rané dospělosti. Postupně si prošel lokální léčbou, fototerapií i léčbou systémovou. Terapie kožní pěnou s fixní kombinací betamethasonu a kalcipotriolu (Enstilar) výrazně zlepšila stav pacienta. Navíc se ukázala klíčová role lékové formy, neboť stejná fixní kombinace účinných látek v jiné formě (gel) nemusí být podle poznatků z této kazuistiky stejně účinná jako kožní pěna.

Lichen sclerosus je chronické kožní zánětlivé onemocnění postihující děti i dospělé, převážně ženy. Nemoc vede k atrofii epidermis, fibróze a jizvení a je často provázená pruritem. Nejčastěji postihuje oblast anogenitální, kde často působí poruchy funkce a je spojeno se zvýšeným rizikem vzniku karcinomu kůže vulvy, může se však vyskytovat i v jiných lokalizacích. Patogeneze nemoci je nejasná. Uplatňují se různé faktory včetně genetické predispozice a lokální iritace. Nejčastěji se zvažuje autoimunitní charakter nemoci zprostředkované cytokiny Th1 buněk, provázené zvýšenou expresí microRNA-155 a tvorbou protilátek proti glykoproteinu 1 extracellulární matrix. Diagnóza je většinou klinická, biopsie zpravidla není nutná. Lékem volby jsou velmi silné lokální kortikosteroidy (clobetasol), u mužů je často kurativní cirkumcize. U rozsáhlejšího postižení je možná aplikace fototerapie (zejména UVA1, méně UVB311), výjimečně se uplatňuje systémová terapie metotrexátem a kortikoidy. Vzhledem k chronickému charakteru onemocnění a možnému vzniku malignity je vhodné dlouhodobé sledování nemocných.
Lichen sclerosus is a chronic inflammatory skin disease affecting children and adults, mostly women, leading to atrophy of the epidermis, fibrosis and scarring, often accompanied by pruritus. It most often affects the anogenital area, where it causes functional impairment and is associated with an increased risk of vulvar skin cancer, however it can also occur in other localizations. The pathogenesis of the disease is unclear. Various factors are considered, including genetic predisposition and local irritation. The autoimmune nature of the disease, mediated by Th1 cell cytokines, accompanied by the increased expression of microRNA-155 and the production of antibodies to extracellular matrix glycoprotein 1, is most commonly proposed. The diagnosis is usually clinical, biopsy is mostly not necessary. The treatment of choice are strong topical corticosteroids (clobetasol), circumcision is often curative in men. For more extensive skin involvement, application of phototherapy is possible (especially UVA1, less UVB311), rarely systemic therapy with methotrexate and corticosteroids is used. Due to the chronic nature of the disease and the possible development of malignancy, long-term follow-up of patients is recommended.
