Prediction methods have become an integral part of biomedical and biotechnological research. However, their clinical interpretations are largely based on biochemical or molecular data, but not clinical data. Here, we focus on improving the reliability and clinical applicability of prediction algorithms. We assembled and curated two large non-overlapping large databases of clinical phenotypes. These phenotypes were caused by missense variations in 44 and 63 genes associated with Mendelian diseases. We used these databases to establish and validate the model, allowing us to improve the predictions obtained from EVmutation, SNAP2 and PoPMuSiC 2.1. The predictions of clinical effects suffered from a lack of specificity, which appears to be the common constraint of all recently used prediction methods, although predictions mediated by these methods are associated with nearly absolute sensitivity. We introduced evidence-based tailoring of the default settings of the prediction methods; this tailoring substantially improved the prediction outcomes. Additionally, the comparisons of the clinically observed and theoretical variations led to the identification of large previously unreported pools of variations that were under negative selection during molecular evolution. The evolutionary variation analysis approach described here is the first to enable the highly specific identification of likely disease-causing missense variations that have not yet been associated with any clinical phenotype.

The Eda pathway ( Eda, Edar, Edaradd) plays an important role in tooth development, determining tooth number, crown shape, and enamel formation. Here we show that the Eda pathway also plays a key role in root development. Edar (the receptor) is expressed in Hertwig's epithelial root sheath (HERS) during root development, with mutant mice showing a high incidence of taurodontism: large pulp chambers lacking or showing delayed bifurcation or trifurcation of the roots. The mouse upper second molars in the Eda pathway mutants show the highest incidence of taurodontism, this enhanced susceptibility being matched in human patients with mutations in EDA-A1. These taurodont teeth form due to defects in the direction of extension of the HERS from the crown, associated with a more extensive area of proliferation of the neighboring root mesenchyme. In those teeth where the angle at which the HERS extends from the crown is very wide and therefore more vertical, the mutant HERSs fail to reach toward the center of the tooth in the normal furcation region, and taurodont teeth are created. The phenotype is variable, however, with milder changes in angle and proliferation leading to normal or delayed furcation. This is the first analysis of the role of Eda in the root, showing a direct role for this pathway during postnatal mouse development, and it suggests that changes in proliferation and angle of HERS may underlie taurodontism in a range of syndromes.

Předmět sdělení: Ageneze zubu je nejčastější vývojovou poruchou dentice, kterou podle databáze Online Mendelian Inheritance in Man (OMIM) nacházíme přibližně u 20 % populace. Přestože se jedná o tak běžnou anomálii, její etiologie zatím není zcela objasněna. Vznik ageneze je ve většině případů podmíněn genetickou poruchou, pouze malé procento agenezí je zapříčiněno vlivem zevního prostředí. Mohou se uplatnit somatická onemocnění, jako rubeola, syfilis, spála, rachitis nebo nutriční poškození plodu během těhotenství a dětství. Chybění zubů může způsobit také ozařování kraniální oblasti v období raného vývoje organismu, poškození mateřského organismu ozářením, chemickými látkami a léky (thalidomid, cytostatika). Z místních příčin jsou nejčastější různé druhy traumat, dále nádory a osteomyelitis. Hypodoncie se může objevit jako izolovaná vývojová vada (nesyndromická hypodoncie) nebo jako symptom komplexních syndromů (syndromická hypodoncie). Dosud popsanými příčinami nesyndromické hypodoncie uvedenými v databázi OMIM jsou mutace v genech MSX1 (muscle segment homeobox gene 1), PAX9 (paired box gene 9), AXIN2 (axis inhibition protein 2), EDA (ectodysplasin A), WNT10A (Wingless-type MMTV integration site family, member 10A) a LTBP3 (latent transforming growth factor beta binding protein 3). Publikovány byly také případy, kdy byla ageneze podmíněna mutacemi v genech EDARADD (EDAR-associated death domain), NEMO (nuclear factor-kappaB essential modulator), KRT17 (keratin 17) a TGFA (transforming growth factor-alfa). Jednotlivé geny se liší jak z hlediska počtu identifikovaných mutací, tak z hlediska počtu dokumentovaných pacientů. Tyto mutace vysvětlují vznik poruchy jen u části postižených jedinců, u většiny nebyly identifikovány žádné defekty v těchto genech. Cestou pro selekci dalších genů, které zodpovídají za nesyndromické formy hypodoncie, může být identifikace genů, které zapříčiňují vznik syndromů, mezi jejichž symptomy patří hypodoncie.
Background: Tooth agenesis represents the most common anomaly of dental development, which according to Online Mendelian Inheritance in Man (OMIM) database, affects approximately 20% of the population. Although the anomaly is so common, the ethiology is still undisclosed. In most cases the agenesis is caused by genetic disorder, only a few develop due to external factors. Some of the external factors are rubeolla, syphilis, vitamin D deficiency or nutritional damage during pregnancy and early childhood. Other harmful factors are radiation therapy in orofacial area in early stages of the development of the patient, harms the mother caused by radiation, chemical substances or drugs (e.g. thalidomide, cytostatics). Local factors include various types of injuries, tumors and osteomyelitis. Hypodontia can occur as an isolated condition (non-syndromic hypodontia) or can be associated with a systemic condition or syndrome (syndromic hypodontia). Despite the fact that, tooth agenesis is so common, little is known about the genetic defects responsible for this complex condition. To date, the genes associated with the non-syndromic form of tooth agenesis, listed in OMIM, are MSX1 (muscle segment homeobox gene 1), PAX9 (paired box gene 9), AXIN2 (axis inhibition protein 2), EDA (ectodysplasin A), WNT10A (Wingless-type MMTV integration site family, member 10A) and LTBP3 (latent transforming growth factor beta binding protein 3). Cases with selective tooth agenesis caused by mutation in genes EDARADD (EDAR-associated death domain), NEMO (nuclear factor-kappaB essential modulator), KRT17 (keratin 17) and TGFA (transforming growth factor-alfa), were also published. All these genes vary both in terms of number of identified mutations and in terms of number of documented patients. These mutations explain the formation of tooth agenesis in only a part of affected individuals. Most patients have no defects in these genes. To select other genes, that are responsible for non-syndromic forms of hypodontia, the identification of genes that cause syndroms with symptoms of hypodontia, seems as reasonable direction of further research.

Předmět sdělení: Ektodermální dysplazie je vzácná geneticky podmíněná porucha. Vyznačuje se vrozeným postižením dvou nebo více ektodermálních struktur, přičemž alespoň v jednom případě musí jít o defekt ve vývoji potních žláz, zubů, vlasů nebo nehtů. Odhaduje se, že postihuje asi sedm z 10 000 narozených dětí. Na molekulární úrovni jsou příčinou rozvoje tohoto onemocnění mutace genů, které se uplatňují v Eda-NFκB signální dráze. Klinicky rozlišujeme dvě hlavní formy onemocnění: hypohidrotickou a hidrotickou ektodermální dysplazii. Větší část případů hypohidrotické ektodermální dysplazie je způsobena mutací genu kódujícího ektodysplazin-A, který je umístěný na chromozomu X v pozici mezi 12 a 13. Menší část případů je autozomálně recesivní nebo autozomálně dominantní s mutacemi v genech pro EDAR (Eda receptor) a EDARADD (EdaR associated death domain) s genovými lokalizacemi 2q12.3 nebo 1q42-q43. Malou podskupinu tvoří pohlavně vázaná recesivní hypohidrotická ektodermální dysplazie s deficitem imunity. Je spojována s mutacemi genu IKK-gamma kódujícího nuclear factor-kappaB essential modulator (NEMO) s genovou lokalizací Xq28. Pro hypohidrotickou ektodermální dysplazii je charakteristická hypotrichóza (prořídlé vlasy a ochlupení), hypohidróza (snížená schopnost se potit) a hypodoncie (vývojová anomálie počtu zubů). Nejčastější je x-vázaná recesivní forma onemocnění. Postihuje častěji muže, ženy jsou obvykle přenašečkami a mají variabilní fenotyp. U mužů typicky nacházíme snížený počet a anomální tvar zubů. Vlasy jsou tenké, málo pigmentované a pomalu rostou. Kůže je tenká, lesklá, hladká a suchá, málo odolná k infekci a k ekzémům. Asi polovina postižených má abnormální nehty. Jsou tenké, roztřepené, lámavé. Druhým typem je autozomálně dominantní hidrotická ektodermální dysplazie, Cloustonův syndrom, s genovou lokalizací 13q12. V kontrastu s pohlavně vázanou hypohidrotickou ektodermální dysplazií je zachována normální funkce potních a mazových žláz.
Background: Ectodermal dysplasia is a rare, genetically determined disease, which is characterized by alterations in two or more ectodermal structures, at least one of these involving alterations in hair, teeth, nails, or sweat glands. It is estimated that about seven in 10 000 births are affected with a form ectodermal dysplasia. The cause of the disease is mutation of genes, which can be found in Eda/NF-KB (nuclear factor kappa B) pathway. We recognize two main forms of the disease: hypohidrotic ectodermal dysplasia and hidrotic ectodermal dysplasia. Most cases of hypohidrotic ectodermal dysplasia are related to the sex and caused by the mutation in EDA with gene localization Xq12-q13.1. Lower number of cases is autosomally recessive or dominant with the mutation in EDAR or EDARADD with gene localizations 1q42.2-q43 or 2q11-q13. The small subclass is sex related recessive hypohidrotic ectodermal dysplasia with immunodeficiency. It relates to the IKK-gamma gene mutations which encodes nuclear factor-kappaB essential modulator (NEMO) with the gene localization is Xq28. Hypohidrotic ectodermal dysplasia is characterized by hypotrichosis (sparseness of scalp and body hair), hypohidrosis (reduced ability to sweat), and hypodontia (tooth agenesis). In the X-linked recessive form of hypohidrotic ectodermal dysplasia, males are usually more severely affected, and females show variable severity, ranging from mild to severe. The most characteristic findings in man are the reduced number and abnormal shape of teeth. The scalp hair is thin, lightly pigmented, and slow-growing. Skin is thin, glossy, smooth, and dry with hypohidrosis. Another type is autosomally dominant hidrotic ectodermal dysplasia, Clouston syndrome with the gene localization 13q12. In contrast to the above described hypohidrotic ectodermal dysplasia ED1, the normal function of sweat and sebaceous glands is maintained.
