Výsledky klinické studie AETHERA ukázaly, že konsolidační terapie brentuximab vedotinem po vysokodávkované chemoterapii a provedení autologní transplantace kmenových hematopoetických buněk signifikantně prodlužuje přežití bez progrese u pacientů s klasickým Hodgkinovým lymfomem. V této multicentrické retrospektivní analýze sedmi hematologických center České republiky jsme analyzovali klinická data z reálné klinické praxe a výsledky léčby u 39 pacientů léčených konsolidační terapií brentuximab vedotinem od ledna 2015 do prosince 2021. Brentuximab vedotin nebyl nemocným podáván v předchozí léčbě lymfomu a pacienti měli nejméně jeden definovaný rizikový faktor pro další relaps. Medián podaných cyklů brentuximab vedotinu byl 8 (1-16), 82 % pacientů dosáhlo během léčby kompletní remise. Medián sledování byl 28 měsíců, dvouleté přežití bez progrese bylo 66,2 % (95% CI 0,52-0,85) a dvouleté celkové přežití 95 % (95% CI 0,82-1,00). Nejčastějším důvodem ukončení léčby bylo kompletní podání 16 léčebných cyklů (20,5 %), relaps/progrese lymfomu (15,4 %) a stejně tak rozvoj symptomatické periferní neuropatie, která byla v různém stupni závažnosti pozorována u 38,5 % pacientů. Naše data potvrzují deklarovanou účinnost a bezpečnost konsolidační terapie brentuximab vedotinem v reálné klinické praxi.
The results from the clinical trial AETHERA showed that consolidation therapy with brentuximab vedotin after high-dose chemotherapy followed by autologous stem cell transplantation increases progression free survival in high-risk patients with relapsed/refractory classic Hodgkin lymphoma. In this multicenter retrospective analysis from seven hematological centers in the Czech republic we have analysed real-life data of 39 patients treated between January 2015 and December 2021. Brentuximab vedotin was not administered in the previous treatment and the patients were at increased risk of subsequent lymphoma relapse with at least one defined risk factor. The median cycles of brentuximab vedotin administered was 8 (1-16), 82 % of the patients achieved complete remission during the treatment. The median follow-up was 28 months, the 2-year progression-free survival and overal survival were 66,2 % (95 % CI 0,52-0,85) and 95 % (95 % CI 0,82-1,00), respectively. The most common reason for treatment discontinuation was completion of 16 cycles administered (20,5 %) relapse/progression of the lymphoma (15,4 %) and symptomatic peripheral neuropathy as well, that was observed in various severity grades in 38,5 % patients overall. Our data have confirmed efficacy and safety of brentuximab vedotin consolidation therapy in the real-life clinical practice.

Voľná cirkulujúca nádorová DNA (circulating tumour DNA – ctDNA) je typ extracelulárnej voľnej DNA (circulating cell-free DNA – cfDNA) uvoľňovaná do krvi pacientov s onkologickým ochorením apoptózou a nekrózou nádorových buniek. Predstavuje alternatívny biomarker, ktorého kvalitatívna i kvantitatívna analýza môže, doplňujúc zobrazovacie vyšetrenie, spresniť hodnotenie liečebnej odpovede u pacientov s Hodgkinovým lymfómom. Ako dynamický parameter by si svoje uplatnenie mohla nájsť pri upresňovaní diagnózy, v skríningu mutácií a pri vstupnej stratifikácii pacienta podľa rizika, v priebehu liečby a po jej ukončení pri hodnotení jej účinnosti aj v ďalšom sledovaní za účelom včasnej predikcie prípadného relapsu.
Circulating tumour DNA (ctDNA) is a type of extracellular circulating cell-free DNA (ccfDNA, cfDNA) released into the blood of cancer patients by apoptosis and necrosis of tumour cells. It represents an alternative biomarker for the qualitative and quantitative analysis that may together with PET/CT imaging refine the assessment of treatment response in patients with Hodgkin lymphoma. As a dynamic parameter, it could be applied for refining diagnosis, initial screening for mutations and patient risk stratification. During treatment and at its the end, it could be used to evaluate treatment efficacy and may also be useful for further monitoring and relapse prediction.

Somatic mutations of genes involved in NF-κB, PI3K/AKT, NOTCH, and JAK/STAT signaling pathways play an important role in the pathogenesis of Hodgkin lymphoma (HL). HL tumor cells form only about 5% of the tumor mass; however, it was shown that HL tumor-derived DNA could be detected in the bloodstream. This circulating tumor DNA (ctDNA) reflects the genetic profile of HL tumor cells and can be used for qualitative and quantitative analysis of tumor-specific somatic DNA mutations within the concept of liquid biopsy. Overall, the most frequently mutated gene in HL is STAT6; however, the exact spectrum of mutations differs between individual HL histological subtypes. Importantly, reduction of ctDNA plasma levels after initial treatment is highly correlated with prognosis. Therefore, ctDNA shows great promise as a novel tool for non-invasive tumor genome analysis for biomarker driven therapy as well as for superior minimal residual disease monitoring and treatment resistance detection. Here, we summarize the recent advancements of ctDNA analysis in HL with focus on ctDNA detection methodologies, genetic profiling of HL and its clonal evolution, and the emerging prognostic value of ctDNA.

Voľná cirkulujúca nádorová DNA (circulating tumour DNA - ctDNA) je typ extracelulárnej voľnej DNA (circulating cell-free DNA-ccfDNA) nachádzajúci sa v periférnej krvi pacientov s onkologickým ochorením. Jej neinvazívna analýza vo forme tzv. tekutej biopsie (liquid biopsy) má potenciálne využitie v klinickej praxi predovšetkým v situáciách, keď nie je dostupný dostatok vhodného materiálu pre podrobné genetické testovanie nádoru, prípadne by bol odber takéhoto materiálu technicky komplikovaný či pre pacienta zaťažujúci. Hodnotenie ctDNA z kvantitatívneho aj kvalitatívneho hľadiska môže pomôcť spresniť diagnostiku a manažment daného ochorenia a prispieť k personalizácii terapeutického prístupu. Uplatnenie v manažmente Hodgkinovho lymfómu by mohla nájsť pri diagnóze v screeningu mutácií, počas liečby a po jej ukončení ruka v ruke spolu so zobrazovacím vyšetrením aj v ďalšom sledovaní. Významným objavom je možnosť sledovania klonálnej evolúcie nádorových buniek pri liečbe checkpoint inhibítormi spojenej s prípadnou rezistenciou na tento moderný typ liečby.
Circulating tumour DNA (ctDNA) is extracellular DNA which can be found in peripheral blood of patients suffering from malignancies. A liquid biopsy as a method of its non-invasive assessment might be potentially useful in clinical practice especially in cases when there is no available source of tumour DNA for genotyping or when a sampling could be problematic due to technical issues or a patient´s condition. Quantitative and qualitative analysis of ctDNA might improve diagnostics and management of a disease and contribute to more personalised healthcare. It can be used in diagnostic screening for mutations, during therapy and at the end of treatment hand in hand with PET/CT imaging, as well as during follow-up. Better understanding of clonal evolution might help to identify patients resistant to checkpoint blockade.

PURPOSE: Many important details of health-related quality of life (HRQoL) after diagnosis and treatment of Hodgkin lymphoma (HL) are still unknown because large longitudinal studies of HRQoL are rare. Therefore, we analyzed a systematically assessed, comprehensive range of HRQoL domains in patients with HL of all stages from diagnosis up to 5 years of survivorship. PATIENTS AND METHODS: We included patients with HL age 18-60 years at diagnosis from the German Hodgkin Study Group trials HD13, HD14, and HD15. We analyzed HRQoL using all functional and symptom scales of the European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire C30 including deviations from reference values. We estimated the effect of different disease, patient, and treatment characteristics using multiple regression and repeated measures analysis and computed correlations of HRQoL scores. RESULTS: We analyzed 4,215 patients with any HRQoL assessment within 5 years after treatment. Higher tumor burden at diagnosis was associated with impaired baseline scores in many HRQoL domains. During survivorship, cognitive, emotional, role, and social functioning and fatigue, dyspnea, sleep, and financial problems were severely and persistently affected. From year 2 on, mean deviations from reference values ranged between 12 and 29 points, with 10 points being a commonly used margin of clinical relevance. In all 3 trials, HRQoL domains 2 and 5 years after therapy were significantly influenced by baseline scores and age but not by randomized treatments. Fatigue was most closely correlated with other symptoms and scales. CONCLUSION: Our results show a high and persistent amount of different HRQoL deficits in survivors of HL that are largely independent of the applied chemotherapies. Our analysis underscores the high, unmet medical need of these rather young survivors of HL regarding the psychosocial adverse effects of the cancer experience.
