INTRODUCTION: Belimumab is a fully humanised mAb against B lymphocyte stimulator (B-LyS). It is the first biological drug licensed and approved by the US FDA and the European Medicines Agency (EMA) for use in combination with standard immunosuppressants in autoantibody-positive systemic lupus erythematosus (SLE). Areas covered: This manuscript evalues the recent data concerning belimumab's safety and clinical effectiveness and its current place in the treatment of SLE. This includes an overview of the recent data coming from the post-hoc analyses of the BLISS trials, real-life experience with belimumab and the accumulating data on its safety. Attention is also paid to the progress made in the identification of predictors of response to belimumab, recent phase I/II/III studies with subcutaneous belimumab, and experience with B cell modulation coming from recent trials with other B cell modulating drugs. Expert opinion: Belimumab currently has its established role in the treatment of patients with SLE with serologic activity and clinical activity namely in mucocutaneous and musculoskeletal domains despite standard of care treatment. Better identification of patients who can benefit from treatment with belimumab is warranted. Data from ongoing studies in lupus nephritis and other data from observational studies are eagerly awaited.

PURPOSE: Patients with DiGeorge syndrome suffer from T-lymphopenia. T-cells are important for the maturation and regulation of B-cell function. Our aim was to characterize the B-cell compartment in DiGeorge syndrome patients. METHODS: B-cell subset phenotypization using flow cytometry. Serum BAFF (B-cell activating factor) and serum anti-alpha-galactosyl IgM measurement using ELISA. Serum IgG measurement using nephelometry. RESULTS: We observed a significantly increased number of naïve B-cells and decreased number of switched memory B-cells in DiGeorge patients. Furthermore, we observed increased BAFF levels and a trend toward hypergammaglobulinemia later in life. Surprisingly, we detected a decrease in marginal zone-like (MZ-like) B-cells and natural antibodies in DiGeorge patients. CONCLUSION: The maturation of B-cells is impaired in DiGeorge patients, with high naïve and low switched memory B-cell numbers being observed. There is a clear trend toward hypergammaglobulinemia later in life, coupled with increased serum BAFF levels. Surprisingly, the T-independent humoral response is also impaired, with low numbers of MZ-like B-cell and low levels of anti-alpha-galactosyl IgM natural antibodies being detected.

Systémový lupus erythematodes (SLE) je prototypem autoimunitního onemocnění s rozsáhlými imunologickými abnormalitami a velmi různorodým spektrem klinických manifestací. V roce 2011 byl schválen první biologický lék v terapii SLE, belimumab, a další biologické léky jsou ve fázi klinických studií. Nejslibnější se jeví cesta terapeutického ovlivnění B-lymfocytů (rituximab), ať už přímo, či prostřednictvím kostimulačních molekul (belimumab, blisibimod, tabalumab, atacicept a další). Tato práce si klade za cíl podat stručnou charakteristiku uvedených biologických léčiv a přiblížit jejich úlohu v terapii SLE.
Systemic lupus erythematosus (SLE) is a prototypical autoimmune disorder with extensive immunological abnormalities and a rather varied spectrum of clinical manifestations. In 2011, the first biological agent for the treatment of SLE, belimumab, was approved, and other biological agents have entered the phase of clinical studies. The most promising appears to be the approach consisting in influencing B cells (rituximab), either directly, or through the mediation of co-stimulatory molecules (belimumab, blisibimod, tabalumab, atacicept, and other). This works intends to provide a brief characterisation of the above biological drugs and describe their role in the therapy of SLE.

Mnohočetný myelom (MM) a Waldenströmova makroglobulinemie (WM) jsou nádorová onemocnění B lymfocytů, pro která je charakteristická nadprodukce monoklonálního imunoglobulinu a infiltrace kostní dřeně (KD) maligními buňkami. Tyto patologické jevy vedou posléze k typickým klinickým příznakům onemocnění, především ke vzniku osteolytických lézí a poruchám krvetvorby. U obou těchto onemocnění hraje velmi významnou roli mikroprostředí KD, jelikož interakce nádorových buněk s buňkami KD usnadňuje růst a přežívání těchto maligních buněk. Tyto interakce jsou zprostředkovány mimo jiné také cytokiny. Jejich produkce umožňuje růst, proliferaci a přežívání nádorových buněk, a tak významně přispívají k patogenezi MM i WM. V předkládaném přehledovém článku jsme se zaměřili na funkci nejdůležitějších cytokinů u obou onemocnění.
Multiple myeloma (MM) and Waldenström macroglobulinemia (WM) are malignant disorders of B lymphocytes. These diseases are characterized by monoclonal immunoglobulin production and bone marrow infiltration, which further lead to disease manifestation mainly via osteolytic lesions and disruption of hematopoiesis. The bone marrow microenvironment plays a crucial role in pathogenesis of both of these diseases, as it is well known that interaction between malignant cells and bone marrow cells facilitates both survival and growth of these tumor cells. The interactions are mediated by several different factors, including cytokines. Their production leads to tumor cell growth, proliferation and survival contributing to pathogenesis of MM and WM. In this review, we focus on function of the most important cytokines in both these diseases.
