Hypervirulentní kmeny Klebsiella pneumoniae (hvKP) mohou způsobovat atypické multilokulární infekce u jinak zdravých pacientů. Diagnostika infekce vyvolané hvKP je založena především na klinickém nálezu a laboratorních výsledcích včetně detekce genů virulence. Typicky se projevuje jako jaterní absces s metastatickým šířením. Léčba je založena na chirurgickém řešení v kombinaci s cílenou antimikrobiální terapií. Výskyt infekce hvKP je relativně častý v Asii. V Evropě je sice stále vzácný, ale incidence onemocnění se zvyšuje. Cílem článku je poskytnout stručný přehled problematiky a upozornit na možný výskyt infekcí hvKP.
Hypervirulent strains of Klebsiella pneumoniae (hvKP) can cause atypical multilocular infections in otherwise healthy patients. Diagnosis of infection caused by hvKP is based mainly on clinical findings and laboratory results, including detection of virulence genes. It typically manifests as hepatic abscess with metastatic spread. Treatment is based on surgical intervention in combination with targeted antimicrobial therapy. The occurrence of hvKP infection is relatively common in Asia, and while still rare in Europe, incidence is increasing. The article aims to provide a short overview of the issue and increase awareness of the possible occurrence of hvKP infections.

Necrotizing fasciitis is a life-threatening skin and soft tissue infection associated with high morbidity and mortality in adult patients. This infection can present as either type 1 infection caused by a mixed microflora (Streptococci, Enterobacteriacae, Bacteroides sp., and Peptostreptococcus sp.), most commonly developing in patients after surgery or in diabetic patients, or as type 2. The latter type is monomicrobial and, usually, caused by group A Streptococci. Rarely, this type can be also caused by other pathogens, such as Vibrio vulnificus. V vulnificus is a small mobile Gram-negative rod capable of causing 3 types of infections in humans-gastroenteritis, primary infection of the vascular bed, and wound infections. If infecting a wound, V vulnificus can cause a life-threatening condition-necrotizing fasciitis. We present a rare case of necrotizing fasciitis developing after an insect bite followed by exposure to the seawater. Rapid propagation of the infectious complication in the region of the right lower limb led to a serious consideration of the necessity of amputation. Due to the clearly demarcated necroses and secondary skin and soft tissue infection caused by a multiresistant strain of Acinetobacter baumannii, we, however, resorted to the use of selective chemical necrectomy using 40% benzoic acid-a unique application in this kind of condition. The chemical necrectomy was successful, relatively gentle and thanks to its selectivity, vital parts of the limb remained preserved and could have been subsequently salvaged at minimum blood loss. Moreover, the antimicrobial effect of benzoic acid led to rapid decolonization of the necrosis and wound bed preparation, which allowed us to perform defect closure using split-thickness skin grafts. The patient subsequently healed without further complications and returned to normal life.

Chirurgické infekce kůže a měkkých tkání jsou bakteriální infekce postihující primárně kůži a měkké tkáně, u kterých je velmi často nutná chirurgická léčba. Tyto infekce zahrnují celou řadu patologických stavů, od jednoduchých povrchových infekcí, které jsou velmi časté a mají příznivou prognózu, až po hluboké a velmi závažné nekrotizující infekce, které jsou sice vzácnější, zato však způsobují trvalé funkční postižení a jsou potenciálně život ohrožujícím stavem. Dokument shrnuje současné poznatky ohledně diagnostiky a léčby těchto onemocnění a prezentuje tak klinikovi aktuální standardy péče o tyto pacienty v souladu s mezinárodními doporučeními. Zároveň jsou však zohledněna i regionální specifika, takže předkládaný diagnosticko-terapeutický management pro jednotlivé klinické formy těchto infekcí vždy respektuje i reálnou klinickou praxi v ČR. Dokument je zpracován na základě konsenzu oslovených odborníků z univerzitních pracovišť napříč celou ČR.
Surgical skin and soft tissue infections (SSTIs) result from microbial invasion of the skin and underlying soft tissues, often requiring surgical treatment. SSTIs encompass a variety of pathological conditions, ranging from frequent simple superficial skin infections with very good outcomes to rare, rapidly progressive necrotizing infections associated with long-lasting morbidity and high mortality. The document summarizes current knowledge of the diagnosis and therapy of these diseases and provides clinicians with current standards of care of these patients based on international guidelines. Additionally, regional specific aspects are also reflected, and thus in all cases, this paper on diagnostic-therapeutic management of individual clinical forms respects the actual clinical practice and epidemiology in the Czech Republic. The document has been prepared based on multidisciplinary consensus of experts from universities all over the Czech Republic.

Background: Necrotizing fasciitis is a major health problem throughout the world. The purpose of this review is to assist providers with the care of these patients through a better understanding of the pathophysiology and management options. Methods: This is a collaborative review of the literature between members of the Surgical Infection Society of North America and World Society of Emergency Surgery. Results: Necrotizing fasciitis continues to be difficult to manage with the mainstay being early diagnosis and surgical intervention. Recognition of at-risk populations assists with the initiation of treatment, thereby impacting outcomes. Conclusions: Although there are some additional treatment strategies available, surgical debridement and antimicrobial therapy are central to the successful eradication of the disease process.
