BACKGROUND: The authors present a case study which describes the development of bilateral optic neuropathy as a complication of allogeneic hematopoietic stem cell transplantation (HSCT) in a patient who underwent a transplant for B-cell acute lymphoblastic leukemia (B-ALL). The patient, who was in remission with regard to the underlying hematological disease, developed edema of both optic discs and maculas three months after transplantation. The morphological finding regressed after treatment with corticoids and comprehensive systemic anti-infective therapy. However, the loss of function was not entirely restored. CASE REPORT: One year after the healing, the atrophy of the optic discs persisted, with corresponding findings in vessel density (VD), retinal nerve fibre layer (RNFL) and visual field changes. Electrophysiological examination by pattern electroretinogram (PERG) showed an alteration in retinal ganglion cells in the left eye, but with significant damage to nerve fibres on both sides. Visual evoked potential (VEP) verified bilateral non-inflammatory neurogenic lesions. This finding was also confirmed by functional magnetic resonance imaging (fMRI). Examination by structural magnetic resonance imaging (MRI) showed inflammatory changes in the optic nerve sheaths over time and a consequent marked narrowing of them. CONCLUSION: The authors believe that edema of the optic discs and maculas was caused by a combination of several factors. Firstly, MRI showed inflammatory changes in the optic nerve sheaths, which led to a blockade of axoplasmic transport. Another factor that may have played a part in the outcome was endothelial damage to blood vessels with impaired microcirculation supplying the optic nerve fibres, which contributed to the occurrence of macular edema.
PURPOSE: A case report of a 40-year-old patient with tuberculosis treated with ethambutol is described. Within six months of starting treatment, there was a painless sudden decline in visual function. Despite the known complications of ethambutol treatment, it was discontinued after another three months. METHODS: In the case report, we highlight the damage to the dominantly peripheral visual pathways. Using electrophysiological examinations, we showed a significant alteration in the optic nerves. Optical Coherence Tomography (OCT) showed progressive loss of vessel density and nerve fibre layer of retinal ganglion cells. Perimetric examination showed both a central decrease in sensitivity and mainly scotomas in the temporal parts of the visual fields. Although there was improvement in visual fields over time, OCT findings indicated progression of ethambutol-induced optic neuropathy (EON). Magnetic Resonance Imaging confirmed the alteration in the peripheral part of the visual pathway (intraorbital, intracranial parts of optic nerves, chiasma, and optic tracts). CONCLUSION: Even though EON is not an unknown complication, new cases still occur and, unfortunately, with an irreversible course. Therefore, it is important to draw attention constantly to this complication and to consider it not only in ophthalmologists' surgeries.

Optic disc swelling is a potentially life- and sight-threatening symptom and its diagnostic procedure often requires the cooperation of a neurologist and an ophthalmologist. A good knowledge of individual optic nerve diseases, including findings that mimic optic disc swelling but do not require immediate investigation, is essential for a correct and effective differential diagnosis of this finding. In this article, we summarize the basic causes according to the mechanism of involvement (inflammatory changes, vascular changes, increased pressure and systemic disorders) and the specific diagnoses leading to optic disc swelling.
Otok terče zrakového nervu je potenciálně život a zrak ohrožující příznak a jeho diagnostický postup často vyžaduje spolupráci neurologa a oftalmologa. Ke správné a efektivní diferenciální diagnóze tohoto nálezu je nutné mít dobrou znalost jednotlivých onemocnění zrakového nervu včetně nálezů, které otok terče jen napodobují, avšak nevyžadují okamžité vyšetření. V článku shrnujeme základní skupiny příčin dle mechanismu postižení (změny zánětlivé, cévní, útlak a systémová onemocnění) a konkrétní onemocnění vedoucí k otoku terče zrakového nervu.

Jamka terče zrakového nervu je vzácná vrozená anomálie terče zrakového nervu. Vyskytuje se sporadicky, při bilaterálním výskytu může jít o autozomálně dominantní typ dědičnosti. Oftalmoskopicky má vzhled oválné deprese v místě optického disku, v případě unilaterálního postižení je disk větší ve srovnání s nepostiženou stranou. Lokalizována může být kdekoliv v oblasti disku, temporální lokalizace může být doprovázena makulopatií. Současné možnosti terapie zahrnují laserkoagulaci sítnice a pars plana vitrektomii, prognóza quo ad visum je však nepříznivá. Práce dokumentuje případ oboustranného nálezu jamky terče u bezpříznakového 9-letého chlapce. Pacient byl odeslán na naši kliniku s podezřením na incipientní odchlípení sítnice, vyšetřením se prokázal nález oboustranné jamky terče. Vzhledem k dobré zrakové ostrosti pacient dále zůstává ve sledování, zatím bez indikace k chirurgické intervenci.
An optic disc pit is a rare congenital anomaly of the optic nerve disc. It occurs rarely, and in case of its bilateral occurance, it can be caused by an autosomal dominant inheritance pattern. Ophthalmoscopically, the disc pit appears as an ovoid depression of the optic nerve disc. When identified unilaterally, the optic nerve disc is usually larger than the disc of the other healthy eye. Optic disc pits can be located anywhere in optic disc. Moreover, when located in the temporal margin, they can be accompanied by a maculopathy. The latest therapeutic possibilities include the retinal laser photocoagulation and pars plana vitrectomy, however the prognosis quoad visum neverthelles remains poor. This report deals with bilateral occurance of the optic disc pit in case of 9-year-old asymptomatic patient. The patient had been transferred to our ophthalmology department with suspected retinal detachment. Following the examination, the optic disc pit was diagnosed. The patient remains subject to further observation, however, owing to his current satisfactory vision without the need for a surgical intervention.

Cílem práce je zhodnocení zrakových funkcí nedonošených dětí, které prodělaly perinatální mozkové poškození. Do souboru byly zahrnuty děti s intraventrikální hemoragií, periventrikulární leukomalacií, atrofií mozku a posthemoragickým hydrocefalem, který vyžadoval operaci. Při očním vyšetření jsme se zaměřili kromě základního vyšetření na zhodnocení vízu, refrakční vady, strabizmus. Normální nejlépe korigovanou zrakovou ostrost mělo pouze 35,4 % dětí. Refrakční vada se vyskytla v 55,8 %, převažovala hypermetropie (85,7 % dětí s refrakční vadou). Strabizmus jsme zjistili v 47,9 %. Z výsledků vyplývá nutnost pravidelných očních kontrol nedonošených dětí s mozkovým postižením a časná kompenzace jejich zrakové vady.
The aim of this research is to evaluate visual functions in premature children who have suffered perinatal brain injury. This study includes children with intraventricular haemorrhage, periventricular leukomalatia, brain atrophy and posthaemorrhagic hydrocephalus, who required surgery. In addition to the general assessment, the following parameters were also evaluated during ophthalmology examination: visual acuity, refractive errors, strabismus. Only 35.4% of children had normal visual acuity. Refractive errors were identified in 55.8% of patients, hypermetropy prevailed (85.7% of all children with refractive disorders). Strabismus was found in 47.9% of children. These results suggest that premature children with brain injury should have regular ophthalmology assessment with early correction of their visual defects.

PURPOSE: To compare the thickness of the retinal nerve fibre layer (RNFL) in hydrophthalmic glaucomatous eyes in children with age-matched healthy controls using scanning laser polarimetry with variable corneal compensation (GDxVCC). METHODS: Twenty hydrophthalmic eyes of 20 patients with the mean age of 10.64 ± 3.02 years being treated for congenital or infantile glaucoma were included in the analysis. Evaluation of RNFL thickness measured by GDxVCC in standard Temporal-Superior-Nasal-Inferior-Temporal (TSNIT) parameters was performed. The results were compared to TSNIT values of an age-matched control group of 120 healthy children published recently as referential values. The correlation between horizontal corneal diameter and RNFL thickness in hydrophthalmic eyes was also investigated. RESULTS: The mean ± SD values in TSNIT Average, Superior Average, Inferior Average and TSNIT SD in hydrophthalmic eyes were 52.3 ± 11.4, 59.7 ± 17.1, 62.0 ± 15.6 and 20.0 ± 7.8 μm, respectively. All these values were significantly lower compared to referential TSNIT parameters of age-matched healthy eyes (p = 0.021, p = 0.001, p = 0.003 and p = 0.018, respectively). A substantial number of hydrophthalmic eyes laid below the level of 5% probability of normality in respective TSNIT parameters: 30% of the eyes in TSNIT average, 50% of the eyes in superior average, 30% of the eyes in inferior average and 45% of the eyes in TSNIT SD. No significant correlation between enlarged corneal diameter and RNFL thickness was found. CONCLUSIONS: The mean values of all standard TSNIT parameters assessed using GDxVCC in hydrophthalmic glaucomatous eyes in children were significantly lower in comparison with referential values of healthy age-matched children.

An automatic method of segmenting the retinal vessel tree and estimating status of retinal neural fibre layer (NFL) from high resolution fundus camera images is presented. First, reliable blood vessel segmentation, using 2D directional matched filtering, enables to remove areas occluded by blood vessels thus leaving remaining retinal area available to the following NFL detection. The local existence of rather faint and hardly visible NFL is detected by combining several newly designed local textural features, sensitive to subtle NFL characteristics, into feature vectors submitted to a trained neural-network classifier. Obtained binary retinal maps of NFL distribution show a good agreement with both medical expert evaluations and quantitative results obtained by optical coherence tomography.
