OBJECTIVE: To evaluate the effects of dipyrone on sensitivity to aspirin (acetylsalicylic acid [ASA]) in patients who underwent peripheral artery vascular reconstruction. SUBJECTS AND METHODS: Impedance aggregometry and light transmission aggregometry were used to determine the effects of dipyrone on ASA treatment in 21 patients. Blood samples were drawn in a 7-day period after the surgery. The cut-off value for high on-treatment platelet reactivity (HTPR) was set at < 65% of aggregation inhibition for impedance aggregometry. For light transmission aggregometry the cut-off value for arachidonic acid-induced aggregation was set at > 20% of aggregating platelets, and the cut-off value for epinephrine-induced aggregation was > 44% of aggregating platelets. The cut-off value for each method was derived from a large number of patients treated with a daily dose of 100 mg of ASA. RESULTS: We found HTPR in 14 (67%) of the 21 patients. None had primary resistance to ASA, i.e., after the addition of ASA in vitro all samples showed antiplatelet efficacy. Regression analysis showed a possible correlation between lower efficacy of ASA treatment and higher daily doses of dipyrone (p = 0.005 for impedance aggregometry, p = 0.04 for light transmission aggregometry), higher platelet count (p = 0.005 for impedance aggregometry), and shorter time from surgery (p = 0.03 for impedance aggregometry). CONCLUSION: HTPR occurs in 67% of ASA-treated patients after lower limb vascular surgery. The occurrence of HTPR correlates with the daily dose of dipyrone. Therefore, dipyrone should not be used as a postoperative analgesic in ASA-treated patients after peripheral artery revascularisation due to its influence on the effectiveness of ASA.

OBJECTIVE: To present a case of survival without visual and central nervous system sequelae at a formate concentration of twice the reported lethal level. CLINICAL PRESENTATION AND INTERVENTION: This was a case of a 33-year-old man who ingested 1 liter of a toxic mixture of methanol and ethanol. Upon admission, he presented with anxiety, tachycardia and hypertension and had a serum formate level of 1,400 mg/l (normal range 0.9-2.1 mg/l), a methanol level of 806 mg/l (normal range 2-30 mg/l), an undetectable ethanol concentration and a normal lactate level. A 10% solution of ethanol and folinic acid was administered intravenously and two 8-hour sessions of intermittent hemodialysis were performed. The patient was discharged on the fifth day without sequelae of poisoning. The follow-up examinations 3 months and 2 years later revealed no damage to the basal ganglia. The patient had normal visual-evoked potential and findings on optical coherence tomography. The genetic analysis revealed a rare minor allele for the gene coding CYP2E1 enzyme of the microsomal ethanol oxidizing system. CONCLUSION: The patient survived acute methanol poisoning without long-term sequelae despite a high serum level of formic acid upon admission.

OBJECTIVE: To describe histological and ultrastructural changes of cardiomyocytes in experimental rats following subplantar administration of carrageenin. MATERIAL AND METHODS: In adult rats, an acute inflammatory reaction was induced by subplantar injection of 0.1 ml of 1% sterile carrageenin solution. In a total of 10 rats, which developed gangrene of tails in 5- to 12-cm-long segments, were killed and their internal organs fixed in 10% formaldehyde solution and subsequently processed for paraffin embedding. Later, blocks of the ventricular heart tissue were refixed and reprocessed for Araldite embedding and ultrastructure observation. Similarly, the cardiac muscle of control, carrageenin-injected rats which did not develop vascular thrombosis was processed. RESULTS: The cardiomyocytes of rats injected with carrageenin showed focal dystrophic alterations, enlarged mitochondria with densely packed concentrically oriented cristae, and many dense and irregularly shaped deposits with microgranular helicoid organization. Normal cardiomyocytes were observed in control rats. Complicating thrombosis of tail blood vessels leading to extensive tail necroses were also histologically confirmed. CONCLUSION: These findings demonstrate specific pathogenic effect in the cardiovascular system of the carrageenin-treated rats. 2007 S. Karger AG, Basel
