The primary objective is a clinical evaluation exploring the value of PET as a predictor of early histopatological response to preoperative chemotherapy in patients with locally advanced esophagogastric cancer. The multimodal treatment consisting of surgery, chemo- and radiotherapy is a standard treatment option in a locally advanced esophagogastric cancer. The prognostically unfavorable patient not responding to preoperative chemotherapy is treated with the same schedule after sugery irrespective of a histopathological response. Early identification of unfavorable group is needed for early change/intensification of treatment strategy. Metabolic non-response is associated with a poor prognosis and is a marker of low histopatological response. CZIBIT in non- randomized multicentric phase II trial exploring prospectively a predictive value of PET and retrospectively the predictive and prognostic significance of miRNA. The primary endpoint is to evaluate the role of PET in changing of preoperative treatment algorithm. The analysis of pharmacokinetics is planned in subgroup of patients.

Cílem grantu je klinické hodnocení zkoumající význam PET a miRNA jako prediktoru časné histopatologické odpovědi v rámci předoperační chemoterapie u pacientů s lokálně pokročilým adenokarcinomem žaludku a gastroesofageální junkce. Standardní léčba lokálně pokročilého adenokarcinomu žaludku a GEJ je komplexní, stávající z operace, chemoterapie a případně radioterapie. Prognosticky nepříznivá skupina pacientů neodpovídající na chemoterapii je pooperačně léčena stejným schématem bez ohledu na histopatologickou odpověď. Časná identifikace této skupiny pacientů je významná ke změně léčebné strategie a případné intenzifikaci léčby. Malá nebo žádná změna metabolické aktivity na PET je markerem špatné histopatologické odpovědi. CZIBIT je nerandomizovaná multicentrická studie fáze II zkoumající prospektivně přínos PET a retrospektivně přínos biomarkeru miRNA jako prediktoru histopatologické odpovědi a prognostického markeru. Primárním cílem studie je zhodnotit význam PET ve změně předoperačního léčebného algoritmu. U vybrané skupiny pacientů bude provedeno vyšetření farmakokinetiky.

First published in 2020, this toolkit is intended for clinicians working in acute care, managing adult and paediatric patients with acute respiratory infection, including severe pneumonia, acute respiratory distress syndrome, sepsis and septic shock. The main objective is to provide key tools for use in the care of critically ill patients – from hospital entry to hospital discharge. The 2022 updated version includes new tools and adapted algorithms, checklists, memory aids for COVID-19 and influenza, and the latest clinical evidence regarding clinical management of SARI.
