Diabetes melitus 2. typu (DM2T) je velice rozšířený syndrom s vzestupnou incidencí. Kromě toho, že postihuje stále větší část populace, jedná se i o prekancerogenní stav nádoru slinivky břišní, která je zcela neléčitelná. Nejvíce rozšířenou lěčbou DM2T je metformin, látka ze skupiny bisguanidů. Metformin je nyní také testován jako potenciální léčivo proti nádorům slinivky břišní. Ukazuje se, že jak protirakovinný, tak i anti-diabetický účinek metforminu je závislý na inhibici mitochondriálního respiračního komplexu I. Proto jsme metformin modifikovali specifickou reakcí tak, aby nová látka měla povahu delokalizovaného kationtu, a proto se hromadila v interfázi mezi vnitřní mitochondriální membránou a mitochondriální matrix. Tato látka má proti nádorům štítné žlázy a proti symptomům DM2T asi o tři řády vyšší účinek než parentální látka, metformin. Tento projekt se bude soustředit na pochopení molekulárního mechanismu účinku nové látky a testování na preklinických modelech obou chorob. Očekáváme velmi významné výsledky základního a hlavně aplikovaného, translačního výzkumu.; Type 2 diabetes mellitus (T2DM) is widespread and has increasing incidence. Besides affecting a growing portion of the population, it is considered a pre-cancerous state for pancreatic cancer, which cannot be treated at present. The most frequent drug against T2DM is the bisguanide compound metformin. This agent is now also tested as a potential drug against pancreatic cancer. It has been shown that both anti-cancer and anti-diabetic effects of metformin are dependent on the mitochondrial respiratory complex I. Therefore, we modified metformin using a specific approach to target it to the interface between mitochondrial matrix and inner membrane. The effect of this compound against pancreatic tumours and symptoms of T2DM is some 10e3 higher than that of the parental agent, metformin. This project will focus on understanding the molecular mechanism of the effect of the new compound and on its testing in pre-clinical models of both pathologies. We expect very important findings relevant to basic research and, in particular, linked to applied, translational research.
