V příspěvku jsou prezentovány možnosti využití hipoterapie u dětských pacientů s dětskou mozkovou obrnou. Hlavní důraz je kladen na seznámení s uceleným metodickým postupem, jenž byl vypracován na základě dlouholetých praktických zkušeností, včetně ověřování v praxi, a dále s využitím prvků používaných v různých hipoterapeutických přístupech v zahraničí. Prezentovaný postup vznikl i z důvodu absence uceleného vzdělání v oblasti hipoterapie a také za účelem vytvoření standardních, v současné době velmi variabilních postupů, v provádění hipoterapie. Metodický postup striktně rozděluje dva principy tzv. funkce koňského hřbetu – princip senzomotorické stimulace a princip diferenciace. Pracuje s proměnnými i neproměnnými faktory, které zajišťují dle potřeb klienta jeden z uvedených principů. Tyto prezentované postupy rozšiřují nabídku fyzioterapeutických přístupů v léčbě dětí s dětskou mozkovou obrnou a nabízejí širší možnosti komplexní léčby těchto dětí.

In the following presentation, we will show different possibilities of using hippotherapy for treating children with cerebral palsy. The main emphasis is laid on a unified methodological approach, which has been created based on many years of practical experience, including verification of effectiveness during active practice combined with the use of therapeutic approaches from around the world. The presented approach has also been created due to the absence of a comprehensive education in the hippotherapy field and also for the purpose of creating a standardized approach among today's many highly variable ones. The methodology is strictly separated into two principles of horseback functionality, that of rimotor stimulation and differentiation. It operates with variable and non-variable factors, which ensures the treatment of one of the principles according to the needs of the client. These presented approaches expand the possibilities of physiotherapeutic approaches for the treatment of cerebral palsy and offers wider possibilities for complex treatment of these children.

Cooperation of occupational therapy, physiotherapy and speech therapy in the therapy of children with cerebral palsy

Aim: The project is focused on the problem of the cooperation of physiotherapy, occupational therapy, and speech therapy during the therapy of children with cerebral palsy. Materials and methods: In the study, 20 children with cerebral palsy were chosen. According to the physiotherapy, occupational therapy and speech therapy entrance examination, an eight-day rehabilitation intervention was assigned. Individual physiotherapy and occupational therapy treatment took place three times per week for 45 minutes. Speech therapy intervention was once a week under the direct charge of a speech therapist, and the practice was three times per week as part of an occupational therapy treatment, where the occupational therapist worked according to the instructions of the speech therapist. Result: Individual therapy was connected in the following order. First there was physiotherapy, with a focus on the stabilization of the truncal muscles and the improvement of breathing habits that leads to better posture. This was followed by logopedics oriented on motorics of the orofacial area. Finally there was occupational therapy, which connected elements of physiotherapy and logopedics during the activity of daily life training with a focus on basal activity of ADL. It was confirmed that the motivation of clients and their parents plays a big role in accomplishing positive results. Conclusion: Coordination of care is essential if a patient is to benefit from multiple health care services provided by various health care professionals. The therapy in children with cerebral palsy is a long-term and lifelong process. The results of our project confirmed the necessity of close cooperation between the physiotherapist, the occupational therapist and the speech therapist with a possible continuity and unification of the therapy.
